The selected candidate will be an employee of Public Health Solutions, a nonprofit organization which is the fiscal and administrative manager of the grant but will be supervised by the NYC Health Department. This is a grant-funded position ending in January 2027 The Division of Disease Control performs identification, surveillance, treatment, control, and prevention of 90+ infectious diseases. It is one of the largest Divisions within the Health Department with seven bureaus including the Bureau of Communicable Diseases. The Bureau of Communicable Diseases is responsible for the surveillance, prevention, and control of over 70 infectious diseases, including foodborne, waterborne, respiratory, zoonotic and vector-borne diseases. Within the Bureau of Communicable Diseases, the Community Outreach and Health Education Unit is a collaborative and cross-cutting team that is in development. The team works across diseases on planning outreach and engagement with communities impacted by infectious diseases and on developing health education materials for preventing and controlling infectious diseases. Job Description The NYC Health Department has an opening for an outreach coordinator and health educator. This position will help develop and foster relationships with members of the agricultural and animal health community in NYC and develop health education materials for various public and provider audiences. They will report to the Community Outreach and Health Education Unit lead. Duties • Coordinating health education initiatives related to avian influenza and respiratory diseases, in collaboration with the Respiratory Pathogens Unit, as well as assisting with initiatives on other diseases and conditions managed by the Bureau of Communicable Diseases (BCD) including foodborne, waterborne, zoonotic and vector-borne diseases., • Arrange and conduct in-person visits to poultry markets in NYC to develop and foster relationships in order to facilitate communication with owners and staff., • Establish and foster relationships with backyard flock groups in collaboration with partners at the NYS Department of Agriculture and NYC Parks Community Garden program,, • Explore ways to engage and share information with the public and people at high risk of exposure to infected birds or animals, and people at greater risk of severe disease., • Create materials for the provider community as needed to support rapid identification and appropriate testing of patients at risk of avian influenza testing,, • Working with disease subject matter experts to develop and implement theory- and evidence-based health education materials or initiatives with a focus on populations most at-risk for zoonotic and vectorborne diseases. Materials and initiatives may include instructional activities and other strategies to change individual health behavior, as well as contributing to bureau and agency driven organizational efforts, policy directives, mass media, and community-level programs., • Collaborating with subject matter experts in BCD to maintain disease specific webpages, fact sheets, and existing health education materials for both a public and provider audience., • Developing social media proposals that promote health using innovative, targeted, and timely posts. Evaluating metrics to inform future social media posts., • Understanding and addressing cultural differences, health inequities, and disabilities when creating content and materials. Qualifications: • Undergraduate or graduate degree in field related to health education., • Previous experience working in fields related to health education., • Capacity and willingness to travel within NYC and be in the field 50% of the time., • The ideal candidate for this position must be a pro-active and self-motivated individual with the ability to work in teams and in a highly dynamic environment with multiple stakeholders and timelines. BroSis provides holistic and long-term support services to youth who range in age from eight to twenty-two. BroSis offers wrap-around evidence-based programming such as four-six year rites of passage programming, thorough five-day a week after school care, school and home counseling, summer camps, job training, college preparation, employment opportunities, community organizing training, and month-long international study programs to Africa and Latin America. Based in Harlem, NY but with a national reach, we publish assorted curricula and collections of our members’ writings; train educators from throughout the nation on our approach; and organize and advocate for social change. We are the caretakers of the NYC Parks Department Frank White Memorial Garden located adjacent to our headquarters and have been responsible for developing the 6,000 sq/ft lot into an Environmental Learning Center that includes a functional Urban Farm (producing more than 30 varieties of fruits and vegetables), a Green House, an aquaponics system, a Group Challenge Course, as well as recreational seating and performing areas. All of these features have been co-designed and constructed by our youth members and local community residents. We also manage a weekly youth-run farmers market, and engage our members in city-wide efforts to improve the local environment. EP teen members learn environmental skills in 4 interdependent Core Groups: Horticultural Science, Sustainable Design, Food Empowerment and Community Organizing. POSITION SUMMARY We are seeking an experienced youth development worker and environmental educator with a love for environmental Justice and community organizing. The Facilitator will report to the Senior Manager of the Environmental Program and work collaboratively with the Environmental Program team to ensure the smooth operation of all programmatic activities. The Facilitator will develop and facilitate a wide array of interactive and educational activities geared toward youth in middle and high school. They must be a dedicated and charismatic youth worker who is able to create fun and innovative workshops, and is able to work to holistically develop and guide youth. Job Description Join The Doe Fund in our mission to move New Yorkers from homelessness to stable housing, lasting employment, and economic opportunity. Our flagship workforce development program, Ready, Willing & Able (RWA), operates across 660 transitional housing beds and combines housing and comprehensive supportive services with paid work training, career preparation, and occupational skills training. The Doe Fund’s innovative social enterprise model creates additional opportunities for New Yorkers to gain paid, hands-on experience while delivering essential services across the city, including supplemental sanitation, graffiti removal, and rain garden maintenance. The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs. We are seeking a dedicated Case Manager/Mental Health Counselor for the Webster House Permanent Supportive Housing Program, which provides long‑term residential care and 24‑hour support services for homeless men and women living with HIV/AIDS. The program is designed to maximize residents’ independence while ensuring their physical and emotional security within a supportive community environment. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households. The Case Manager/Mental Health Counselor is responsible for delivering supportive case management services to HASA‑funded residents. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a deep understanding of the unique needs of individuals living with HIV/AIDS. Key Responsibilities: • Maintain a caseload and provide regular case management services to these individuals, • Maintain resident records in compliance with agency and funding standards, • Conduct and review assessments, • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency, • Monitor and ensure residents are linked to healthcare providers, • Provide counseling and crisis intervention as necessary, • Assist residents in securing and maintaining benefits, • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs, • Facilitate groups and activities that promote health and independent living skills development, • Monitor and assist residents with rent payments and budgeting, • Conduct regular unit inspections, • Educate residents about approaches to mental health, • Compile resources on mental health information and treatment strategies, • Attend ongoing training to enhance the services provided to the residents, • Assist with completion of HASA’s weekly and monthly reports as well as any other relevant reports, • Other duties as assigned The Ideal Candidate: • You have a Master’s Degree in Social Work or related field; a license in Clinical Social Work or Mental Health Counseling is preferred., • You have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness, • You have at least 5 years of experience in the mental health field, • You have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues, • You have solid computer skills – internet, Microsoft Office, and charting database, • You have great organizational, writing, and communication skills Interested?
