    POSITION OVERVIEW The counselor will provide assessment, counseling, advocacy, and referral services to child witnesses of gender-based violence and survivors of teen dating violence. They will work with the clinical team to provide holistic, trauma-informed, survivor-centered clinical services. They will utilize a trauma-informed approach to promote clients’ rights and provide services in a manner that emphasizes safety, respect, and dignity. They will provide strength-based guidance and work to resolve the impact of gender-based violence in their young lives. RESPONSIBILITIES -Complete clinical assessments and treatment plans; -Provide mental health services for children and teens (from 0-21 years old) including short term individual counseling and crisis intervention with individuals and their families; -Collaborate with staff at other Sanctuary for Families sites to provide holistic services; -Advocate for and assist families/individuals in the negotiation of service delivery system to ensure receipt of appropriate clinical and related services; -Demonstrate competence in evidence-based modalities such as CPP (Child Parent Psychotherapy) and TF-CBT (Trauma Focused Cognitive Behavioral Therapy); -Develop effective relationships with children and families; -Facilitate appropriate clinical groups for clients, including, but not limited to, parenting skills groups; -Participate in team and agency meetings, individual supervisory meetings, case conferences and other meetings as requested; -Complete all appropriate paperwork, including case notes and reports, in a timely manner; -Assist with children’s room coverage to support team in providing services to families seeking services; -Conduct outreach and training related to gender based and domestic violence; -Perform other duties as assigned by the Clinical Supervisor; -Provide support and supervision to MSW interns as needed. -Hybrid work requirement with both remote and in-person formats. REQUIREMENTS -LMSW, LCSW, LMHC, LCAT, LMFT or similar degree. -A minimum of one (1) year of relevant work experience with victims of domestic violence, including work with children. -Bilingual English and Spanish required. -Experience providing counseling and supportive services to domestic violence survivors and their families required. -Experience facilitating support groups and dynamic community workshops required. -Ability to work flexible hours, a mix of hybrid and remote work, and at least 2 evenings a week. -Interest in working with clients from diverse backgrounds. -Computer literate in Microsoft applications required. As a member of the Pathway Home multi-disciplinary team the Peer Specialist position will work under the supervision of the Team Leader. The role will involve facilitating transition from one level of care to the community by addressing the preparatory needs of participants in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through team and individual work in the community. The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Peer Specialist engages participants in peer-based services to identify and overcome barriers to community integration, utilizing a peer relationship to foster connection, trust, understanding and validation with community providers and supports. The role will require on call coverage and a willingness to work flexible hours. Job Responsibilities: • Engagement beginning from referring location with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the participant;, • Individual sessions with participants emphasizing prevention and preparing for independent community living and the promotion of optimum mental and emotional health. Responsibilities: • Responsible for reading, knowing and complying with all appropriate governing documents to include contracts, agency policies and procedures, State or Federal standards, specific program operations manuals and circulars both internal and external., • Ensures compliance with the agency’s Operations and Personnel Manuals and the Division of Developmental Disabilities Standards Manual., • Completes and submits in a competent, and timely manner all reports, forms, and documentation as required by the agency and Division of Developmental Disabilities., • Understands basic developmental disabilities and learning styles, using motivation techniques to help clients improve in areas with behavioral or medical challenges., • Must complete all CDS trainings and refreshers as directed., • Attend staff meetings, all other relevant meetings and training as directed., • Assist clients with goal outcomes established on the Individual Service Plan (ISP)., • Responsible for reading the ISP and working within the documented expectation for client development., • Provides a consistent means of relaying accurate and complete information to direct supervisors., • Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors according to plans that are in place., • Provides training and supervision to clients in all aspects of community experience living., • Serves as a positive role model for clients and other staff., • Provides safe transportation in agency vehicles for clients as necessary., • Maintains confidentiality and professionalism when communicating with family members or group home staff, Support Coordinators and any outside agency personnel., • Maintains active, positive engagement with clients at all times., • Always ensures the safety and well-being of clients., • Provide direct care and support to individuals with disabilities or special needs., • Proactively accompany and support clients during medical appointments, ensuring relevant information is provided to the healthcare professional. Clearly and accurately communicate details from the appointment to the Residential Coordinator, Medical Compliance Officer, and/or designee., • Follow and demonstrate understanding of medication policy and procedures in the monitoring and administration of the client’s medications. Completes Medication Error Report when needed., • Ensures home, grounds, yard and van are maintained in clean, safe, and sanitary condition., • Ensures meals are prepared and planned based on clients’ individual diet, client input, and the planned menu for the month., • Ensures that the Recreation Calendar is carried out as per client input and as per the planned calendar completed each month., • Responsible for competing all required documentation to include but not limited to, daily logs, communication logs, and behavior reports., • Ensures line of sight for the individual(s) being supported is maintained and completes reports accordingly., • Responsible for adequately and completely cleaning the home during any shift worked., • Ensures that a client’s dignity is not compromised in any way., • Responsible for reporting all emergencies or unusual incidents to direct supervisor., • Cooperates with the licensee and Department staff in any inspection, inquiry or investigation.
