Job Description The Personal Training Recruiter is responsible for executing a comprehensive recruitment strategy to support Equinox’s Personal Training business and its primary revenue-driving positions. This role aims to increase applicant flow and headcount while implementing best-in-class interview and selection practices with Personal Training Managers. Reporting directly to the Talent Acquisition Manager overseeing all markets, the Personal Training Recruiter will promote Equinox as an employer of choice for Personal Trainings and serve as an expert in all areas of Talent Acquisition. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. CAMBA’s Flagstone Family Center provides comprehensive case management services to 160 families who are in a Tier II transitional residence program for homeless families. Putnam Court is a newly constructed residential building located on Putnam Avenue between Irving Place and Downing Street in the Clinton Hill neighborhood of Brooklyn. The project consists of a seven-story elevator building containing 59 residential units, with 58 units of rental housing and one superintendent’s unit. CAMBA, Inc. is the on-site services provider at Putnam Court. Thirty-four apartments, 58% of the units in the building, will be master leased by CAMBA and will house Population A chronically homeless single adults who are diagnosed with SPMI (seriously and persistently mentally ill) or MICA (mentally III and chemically addicted). The remaining 24 units are occupied by community residents, who may require some services. SUMMARY Provide on-going training and personal assistance to Community Habilitation participants with disabilities with the intent of promoting independence and community inclusion. Work with minimal supervision, provide training, role modeling, structured routines and specialized assistance as outlined in each participant’s habilitation plan. As much as possible, routines will be individualized, based on the participant’s choice of activities and aimed at empowerment and inclusion in the community. Job Description:\n\nTitle: Day Services Qualified Intellectual Disability Professional / QIDP Reports: Director of Day Services Department: Day Services Location: Day Services, NY Status: Exempt, Management POSITION SUMMARY: The Qualified Intellectual Disability Professional / QIDP is responsible to assist and ensure the overall operations and services are being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The role of the Qualified Intellectual Disability Professional / QIDP is responsible to assist in the overall coordinating and managing the overall person-centered services for assigned individuals. The Qualified Individual Disability Professional (QIDP) serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. QIDP also supervises the rehabilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan. The QIDP supervises the treatment process in the assigned day programs, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. The QIDP is responsible for ensuring service provision by all disciplines represented on the treatment team. The QIDP meets routinely with other Team Clinicians for coordination and integration of services. The QIDP coordinates with the Program Coordinator(s) and staff. The QIDP monitors provision of individual and group program services as well as the maintenance of a therapeutic milieu which is conductive to their health and well-being. The QIDP ensures provision of integrated services to individuals by fostering within each Team an awareness of the need for service integration; Coordinating service delivery and planning activities with day program services; and Monitoring service provision by agency clinical staff, consultants and community-based vendors, ensuring services are in accordance with IDT treatment recommendations. They report to the Clinical Coordinator, or Director of Clinical and Social Services, while working closely with their Building Manager. This relationship will enable the Social Worker I to provide a comprehensive range of social work services to the residents and enable the residents to establish and maintain safe and independent lifestyles in the community. WSFSSH strongly supports M.S.W. level social workers obtaining their licensing. We offer study groups for staff who are preparing to take their exam. WSFSSH will also pay for a study course if needed, and pays for up to 2 licensing exams, as well as registration/licensing fees. Once a MSW staff member has obtained their LMSW, WSFSSH will cover reasonably priced ceu credits in order to meet licensing requirements. In addition, WSFSSH will provide weekly clinical supervision by a LCSW, and because WSFSSH is a waivered agency with the NYS Education Dept, a LMSW's work experience will count towards their LCSW. Typically, after 3 years of full time employment, a LMSW will be eligible to apply for their LCSW. Similarly, WSFSSH will cover testing and licensing fees to obtain the clinical license, and when LCSW is obtained, WSFSSH will cover the cost of CEUs to maintain the license. Responsibilities • Assist residents in obtaining and maintaining appropriate medical and mental health services., • Assist residents in obtaining and maintaining entitlements., • Assist residents in obtaining and maintaining home care., • Provide care coordination for residents, as needed, between community service providers., • Assist residents in establishing and/or maintaining family and social relationships., • Through in-house and community referrals, assist residents in participating in socialization activities., • Assist residents with budgeting, income maintenance etc. to ensure on-time payment of rent., • Assist residents with money management as needed., • Follow up with resident and other service providers during out of facility stays such as hospitalizations and/or short-term rehabilitation placements., • Oversee the discharge planning process for residents as applicable, including the completion of all necessary discharge forms and/or applications to other facilities., • Provide individual counseling as needed., • Escort residents to appointments as needed., • Provide crisis intervention services as needed., • Provision of group work services for residents, including recreational, educational and/or clinical groupwork., • Participate in the intake process of prospective residents as assigned., • Maintain professional roles and boundaries., • Work cooperatively with other staff members., • Share information about resident progress, needs, and problems with other staff as appropriate, guided by the WSFSSH Confidentiality Policy., • Complete non-reportable and reportable incident reports as assigned., • Uphold standards of diversity, equity and inclusion in provision of services., • Maintain competency in software and data bases as required., • Implement emergency procedures as necessary., • Attend supervision meetings, team meetings and staff meetings., • Maintain thorough case records, including case notes, support plans and psychosocial histories within the time frames required by WSFSSH and the contract/regulatory agencies where applicable., • Other duties as assigned.
