Work Schedule: Full-time/40 Hours Position Type: On-site Branch Location: Westbury, NY This is NOT a remote or work-from-home position. You will sit on-site at our Westbury, NY branch location. As the RN Clinical Manager, you will oversee all direct care patient services provided by clinical personnel. You will be a focused on both home health clinical quality assurance and home health clinical operations plans. You will report to the Branch Director in the Westbury, NY branch location. • Develop, plans, implements, analyzes, and organizes clinical operations for a specific location managed., • Conduct/delegate the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while following Company, physician, or health facility procedures., • Manage the assignment of caregivers., • Responsible for and oversees the delivery of care to all patients served by the location. Receive case referrals. Review available patient information related to the case, including disciplines required, to determine home health or hospice needs. Accountable to ensure patients meet admission criteria and make the decision to admit patients to service. Assign appropriate clinicians to a case., • Instruct and guide clinicians to promote more engaging performance and delivery of quality home care services and is available during operating hours to assist clinicians., • Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing patient Plan of Care (POC)., • Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers. Ensure final audits/billing are completed and in compliance with Medicare regulations., • Coordinate communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning., • Work together with the Branch Director and Company Finance Department to establish location's revenue and budget goals., • Participate in sales and marketing initiative., • Oversee all clinical employees assigned to a specific location. Responsible for the coordination, and evaluation of the location. Manage supervisory responsibilities following Company procedures., • Handle necessary employee corrective action and discipline issues fairly and objectively, with the Human Resources Department and the Executive Director/Director of Operations., • Participate in the interviewing, hiring and development of direct care clinicians. Evaluate their performance relative to job goals and requirements. Coach staff and recommends in-service education programs, when needed. Ensure adherence to internal policies., • Assess staff education needs based on the review of clinical documentation in addition to feedback and recommendations by Utilization Review staff. Upon completion of the assessment, creates and conducts regular staff education., • Analyze situations, identifies problems, evaluate alternative courses of action through the use of Performance Improvement principles., • Responsible for the QA/PI activities. Work with Utilization Review staff relative to data tracking for performance review and outcomes of care analysis to determine efficiency, the efficacy of case management system and any other systems and process. Competently perform patient care assignments and staff management activities., • Provide direct patient care on an infrequent basis and only in times of emergency., • Step in as the Branch Director in their absence., • Interpret Company standards and Company procedures to ensure compliance with external regulatory authorities and ensure that caregiver clinical documentation meets internal standards., • Participate in performance improvement activities, maintain ongoing clinical knowledge through internal and external training programs. Provide interpretation of knowledge and direction to staff., • Graduate of an accredited School of Nursing., • Current state license as a Registered Nurse., • Proof of current CPR., • Valid driver's license, auto insurance and reliable transportation., • Two years as a Registered Nurse with at least one-year of management experience in a home health, hospice or equivalent environment., • Certified Home health Agency experience is required (NY state requirement) or four years' CHHA experience and six credit hours toward BSN., • Management and people leadership experience, preferred., • OASIS experience, required. OASIS certification (HCS-O, COQS, and/or COS-C), preferred., • Homecare Homebase (HCHB) experience, preferred., • CMS PDGM billing knowledge and/or experience, preferred. The Client Coordinator coordinates and oversees the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department and completion of a required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. The Client Coordinator accepts primary responsibility for development of program goals and methodologies and for supporting the DSPs to correctly implement and document such activities. The Client Coordinator is primarily responsible for ensuring the goals and activities are developmentally appropriate and are in response to identified prioritized needs, and to relay relevant information to the team. The Client Coordinator ensures provision of integrated services to individuals by fostering within each Team an awareness of the need for service integration, coordinating service delivery and planning activities with day program services, monitoring service provision by agency clinical staff, consultants and community based vendors, and ensuring services are in accordance with IDT treatment recommendations. The Client Coordinator ensures provision of a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to ensure provision of coverage levels consistent with the treatment needs of the individuals and regular monitoring of the day program setting to ensure maintenance of an atmosphere conducive to individuals' development and well-being, where the rights of each individual are upheld and respected. The Client Coordinator regularly records progress of individuals and documents all changes in Plans of Care / ISPs according to procedures established by the agency. The Client Coordinator reports regularly to the Supervisor on issues of program compliance, noncompliance, and remedial actions taken to correct deficiencies. The Client Coordinator identifies needs and requests supports, assistance or direction to meet service needs and ensure program compliance. The Client Coordinator participates in clinical reviews, incident review and human rights committee meetings and other meetings as directed by Supervisor. As schedule allows, the Client Coordinator is the primary chair on behalf of the agency in the Life Plan Meetings. The Client Coordinator reviews documentation and provides technical assistance or direction to program management and staff. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES: Employee must possess the ability to interact with individual and staff professionally and ethically, understand and implement service plans, and to complete all required documentation clearly, legibly in a timely manner. Previous experience working with individuals with IDD/Mental Illness is required and proven experience managing teams. Competency in Microsoft applications including Word. Previous experience in a Day Support Program for individuals with an IDD or Special Education teaching/learning environment is preferred. Reporting to the Senior Program Manager, the Clinical Specialist will provide evidence-based assessments and interventions, case management services, family engagement, restorative practices, and clinical services. The Uplift program is a trauma and healing program for young men between the ages of 16-24 who have experienced exposure to violence, with a special focus on individuals in the justice system. Program Description: PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve. They help clients navigate through what can be a complicated and confusing application process, step by step. Each year, we help over 22,000 people apply for or renew free or low-cost health insurance coverage, 14,000 people to apply for SNAP, and 35,000 eligible pregnant and nursing women and children up to five years of age to enroll in the New York State WIC (Women Infants and Children) Program. Summary of Position: Under direct supervision of the Center Manager, the Qualified Nutritionist is responsible for the assessment of the WIC eligibility of applicants, direct implementation of nutrition education components, participant-centered nutrition services and support to WIC clients. Specifically, the Qualified Nutritionist will: • Verifies and documents eligibility of WIC participants to enroll eligible applicants., • Provides general knowledge of WIC program and orientation to new participants., • Participates in the administration of the WIC program and evaluation of documents., • Develop and implement individual care plans (ICPs) and oversee high risk care to all participants., • Provide oversight and guidance to CPA and Nutrition Assistant staff., • Provides individual and facilitated group discussion using the Participant Centered Nutrition Education (PCNE) technique., • Ensures relevant and current information transfer regarding pregnancy, nutrition, and/or health concerns., • Assists in evaluating and developing literature, posters, videos and DVDs., • Conducts Participant Centered Nutrition Assessment (PCNA) to motivate WIC participants and help improve their health and nutrition status., • Performs client anthropometric and hematological measurements on site to prevent delay in benefits between physician visits., • Undergoes annual anthropometric and hematological training on site., • Participates in all other activities that contribute to the efficient operation of the WIC Center., • Ability to travel and work at other WIC centers in Queens, Brooklyn, and the Bronx., • Attends all mandatory trainings and meetings., • B.S. or B.A. in Food and Nutrition, including at least 18 semester credits in nutrition. Job Description Summary: As care manager, a human service professional to provide case management services to participants who are eligible for assistance through the Center of Treatment Innovation program. The role of our Case Manager is guiding participants through the treatment and recovery system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Case Manager provides direct services to participant's including the completion of needs assessments, care plans, periodic reassessments and overall comprehensive service coordination. Essential Duties and Responsibilities: Core duties and responsibilities include the following. Other duties may be assigned. • Communicate with participants, families, SUD treatment and medical personnel as necessary to facilitate a desired outcome., • Educate participants and facilitate procedures related to support benefits with a special focus on the process for transitioning into recovery., • Provide holistic and comprehensive case management services., • Participate in the intake and assessment process., • Assess eligibility for benefits and entitlements., • Goal setting and long-term case plan development., • Motivate others toward achieving goals., • Progress monitoring., • Maintain current contacts for advocacy and referrals., • Meet with or have telephone contact with participants on a bi-weekly basis., • Maintain knowledge of community resources and counseling/social work practices with high risk populations., • Connect families with needed and available community resources., • Maintain a caseload of participants/families Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
