About HopeCentral HopeCentral is a faith-based 501(c)(3) non-profit clinic and serves all families in Southeast Seattle regardless of income, status, or belief system. We launched in 2014 as a primary care practice with the mission of “sharing the love of Jesus through excellent, enhanced primary care for the entire community.” Since 2015, we have been an Autism Center of Excellence for Washington state and have provided pediatric primary care with a focus on special healthcare needs. Job Summary At HopeCentral, we believe it is important to care for the whole person; mind, body, and spirit. Toward that end, we have adopted a multidisciplinary team-based approach to health and wellness. Our behavioral health clinician is a core member of the collaborative care medicine (CoCM) team, which works together with our pediatricians and a consulting psychiatrist to address children’s mental health care needs and to facilitate improved functioning. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers., • Screen and assess patients for common mental health disorders. Facilitates patient engagement and follow-up care., • Provide parent education and coaching about common behavioral health problems, home behavioral management strategies, and available treatment options., • Systematically track treatment response and monitor patients (in person or by telehealth) for changes in clinical symptoms and treatment side effects or complications., • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment., • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, RUBI, motivational interviewing, and other treatments as appropriate., • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care., • Develop and complete maintenance self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the registry. Care Coordination • Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers, schools., • Facilitate referrals for clinically indicated services outside of HopeCentral (e.g., mental health specialty care) Consultation • Participate in regularly scheduled (usually weekly) caseload consultation with the CoCM psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person., • Support physician team in identification and vetting of patient facing psychoeducational materials Chaplaincy • Serve as a counsellor and guide to the spiritual needs of HopeCentral staff and patients., • Seek to motivate and initiate meaningful engagement of patients’ spiritual beliefs and practices in the management of their difficulties. Program Summary Community Behavioral Health Services provides all mental health, WISe, and substance use disorder treatment for families and individuals enrolled in Rising Strong. Rising Strong serves families who are at risk for separation due to parental substance misuse by providing safe housing, substance abuse disorder treatment, case management, and other supportive wraparound services. By serving whole families, our aim is to reduce trauma in children, decrease the rates of foster care placements, and remove barriers that traditionally prevent parents from succeeding in substance abuse treatment. Job Summary This position helps serve as mental health therapist as a part of the WISe wraparound care of families enrolled at Rising Strong. The therapist is responsible for the intake and clinical treatment of at least one youth per family. This may include the use of evidence-based parent coaching, family therapy, and intensive individual treatment of the youth. They will operate under the oversight and leadership of the WISe Director and/or WISe Clinical Supervisor. This position will be responsible for providing trauma-informed insight to the WISe team and Rising Strong staff. This position will be expected to help staff make decisions on familys needs and will report to the WISe Director and RS leadership teams on items requiring their attention. The same person would be in the 1st percentile with scores of 10 inches of flexibility, 51.50% of body weight pushed and a 1.5 mile run in 22:00. Candidates must meet or exceed the 25th percentile.
