The Client Care Coordinator is responsible for engaging individuals and/or families in long term services within the community. The LMSW/LMHC is supervised by the LCSW/LMHC Clinical Supervisor. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Senior Psychiatrist is the psychiatric leader of the facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic and evidence-based therapeutic interventions are provided for the individuals incarcerated in the New York City jail system. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for elderly and medically complex patients with comorbid mental illness., • Directly treat and oversee the psychiatric care of additional patients housed in NIC/WF/CDU, • Direct patients to the appropriate level of mental health care, within the facility or in the hospital as needed., • Close communication with psychiatric and medical providers at Bellevue Hospital to ensure continuity of care as patients transition between the two facilities., • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief and/or Senior Clinical Supervisor, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. New York City Health and Hospitals Corporation Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. She/he ensures the quality of treatment services, the fidelity of evidence-based service delivery and timely documentation of all group and individual counseling services and support services delivered by the facility’s clinical staff. She/he is responsible for oversight of job performance, guidance of all clinical staff members and for the direct supervision of each of the clinical Deputy Directors. She/he is responsible for maintaining OASAS regulatory compliance and assuring that programs operate in accordance with Odyssey House organizational standards. SPECIFIC DUTIES & RESPONSIBILITIES: 1. Implement, monitor and provide oversight of all aspects related to the delivery of residential substance use disorder services., 2. Conducts regular and direct observations of all clinical treatment activities., 3. Implement and ensure compliance with all clinical policies and procedures and protocols., 4. Ensure compliance with all referent communications, HRA STARS reporting, and HRA finger imaging verification., 5. Provide crisis intervention services., 6. Conduct utilization review, incident management and review and quality assurance reviews., 7. Assure that staff maintains accurate and confidential clinical records that comply with external monitoring agency standards and internal policy and procedures., 8. Prepare all proposals and reports as needed., 9. Assume responsibility for program operations in the absence of the Program Director., 10. Assure all clinical staff members are adequately trained., 11. Provide and implements formal orientation plan to newly hired direct subordinate staff., 12. Conduct regular clinical and administrative meetings with staff., 13. Provide direct supervision for designated staff as per Odyssey House policies. Prepare written annual evaluations on staff under his/her supervision. Review evaluations of all clinical staff members., 14. Provide ongoing training for clinical staff and participate in Odyssey House in-service trainings., 15. Lead clinical quality improvement activities in collaboration with Program Director., 16. Represent Odyssey House at community and public forums., 17. Other relevant duties as assigned by Program Director. REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES 1. Master’s Level Clinical Degree and five (5) years administrative or clinical experience working within a substance use disorder treatment setting required., 2. Two (2) years’ experience in an administration and/or clinical supervisory role required., 3. Qualified Mental Health Professional (e.g. LCSW, LMSW, LMHC, CRC, Ph.D., MSN, CASAC etc.) required., 4. Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Record and Outlook programs), 5. Must be able to work a flexible schedule., 6. Job Description Job Title: Psychiatric Nurse Practitioner (Certified in Psychiatry) Program/Department: Housing, Health and Homelessness Solutions & Services Reports to: Division Director Work hours: Full time 35 hours per week Location: Must be able to travel to various locations throughout Brooklyn Salary Range: $150,000 - $180,000 The Psychiatric Nurse Practitioner currently supports two Brooklyn Community Services Divisions – the Housing, Health and Homelessness Solutions Division which offers a variety of housing supports to people who are unhoused in Brooklyn including supportive housing programs, homeless shelters, and a street outreach program. Many of those clients experience co-occurring mental health conditions and substance abuse disorders and Services for People Living with Disabilities which provides strengths-oriented psychiatric rehabilitation programs devoted to helping adults increase independence and overall well-being. The PROS Counselor aids people living with persistent mental illnesses in developing life role goals, in managing symptoms, and in taking concrete steps toward a better life. The Psychiatric Nurse Practitioner will ensure the delivery of high quality mental health support in all aspects of service delivery to clients from vulnerable populations in accordance with funder and regulatory requirements; best practices; BCS policies and Core Values; and the Code of Conduct for Custodians of People with Special Needs. This is a service delivery leadership position and the nurse serves as a role model to all stakeholders. Responsibilities: 1. Direct Clinical Service & Treatment * Clinical Assessment and Evaluation: Conduct comprehensive psychiatric evaluations for program participants, including specialized evaluations for supportive housing applications. * Medication Management: Provide routine monthly medication management appointments, including the authority to prescribe psychiatric medications and administer necessary psychiatric injections. * Specialized Assessments: Complete psychosocial assessments for 2010E applications when case managers are unavailable. * Crisis and Support Care: Deliver immediate crisis mental health support and provide necessary mental health support post-discharge for members/residents. * Resource Navigation: Provide necessary mental health community referrals as indicated by client need. 2. Documentation and Compliance * Clinical Documentation: Ensure all psychiatric assessments, medication management notes, and missed appointment notes are accurately documented and submitted to the Program Director (or designee) by the end of each day. * Prescription Logistics: Efficiently utilize the e-prescribing system to submit and transmit prescriptions directly to the appropriate pharmacy. * Confidentiality: Maintain the strict confidentiality of all program files and evaluation documents. * Program Oversight: Oversee and ensure full compliance with all mandated protocols (e.g., HIV testing, harm reduction services) and funder requirements. * Licensure and Waivers: Maintain all required regulatory documents, including essential waivers, licenses, and registrations (e.g., CLIA waiver, temporary authorizations). * Logistics and Inventory: Oversee the distribution of syringes and Narcan, and maintain accurate, up-to-date inventory logs for all supplies. * Reporting: Collect and submit all required monthly and quarterly reports/deliverables to the Program Director in the specified format and timeframe. 3. Team Collaboration and Professional Development * Team Meetings: Attend required weekly team meetings (including specific 9:00 AM meetings) to review caseloads, bed rosters, and discuss program operations. * Case Coordination: Participate in case conferences with the Case Manager and Program Director to ensure seamless, coordinated care. * Staff Training: Ensure all program staff are properly trained on current program policies, procedures, and compliance standards. * Professional Development: Participate in all mandatory training and meetings as directed by funding agencies and/or the Program Director. * Community Engagement: Engage in and support community events and outreach activities to advance program goals. Qualifications:Required Education & Licensure * Active and unrestricted Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) issued by the New York State Education Department (NYSED). * Must possess prescriptive authority for all necessary medications within the scope of practice, including the ability to be designated in the program's registration for prescribing Naloxone (opioid overdose reversal medication). * Current and valid DEA registration. Essential Clinical Expertise * Proven experience providing psychiatric care to adults and young adults with Serious and Persistent Mental Illness (SPMI), including those who are unhoused (homeless) and those with active Substance Use Disorders (SUDs). * Expertise in utilizing integrated treatment models for co-occurring mental health and substance use disorders (Dual Diagnosis). This includes proficiency in evidence-based practices such as: * Motivational Interviewing (MI) * Cognitive Behavioral Therapy (CBT) * Trauma-Informed Care (TIC) principles * Demonstrated ability to perform the following core functions: * Conduct comprehensive psychiatric evaluations. * Provide expert medication management, including Pharmacotherapy for SUDs (e.g., Medication-Assisted Treatment - MAT). * Perform thorough risk assessments (suicide, violence, relapse). * Ability and willingness to provide psychiatric services in diverse community settings, including on a mobile outreach unit and other non-traditional environments.
