Job Description JOB DEFINITION: Provide administrative supervision to a dynamic team of residential staff providing housing and services to people with psychiatric disabilities. Responsible for the overall functioning of the programs and ensuring compliance with agency policies and procedures as well as applicable local, state and federal regulations. EDUCATIONAL REQUIREMENTS: Minimum of Bachelor's degree in human services, Master's Degree preferred. EXPERIENCE REQUIRED: Two years supervisory experience in a mental health related field. Knowledge of the needs of the Co-Occurring D/O population. Minimum of two years of experience in a residential program and familiarity with Office of Mental Health regulations strongly preferred. DUTIES AND RESPONSIBILITIES: • Oversee and supervise all aspects of assigned residential programs and assure that appropriate and necessary services are delivered., • Ensure all program guidelines, protocols, policies, procedures and regulations are followed., • Participate in the recruitment, hiring, training and ongoing evaluation of residential staff under the supervision of the Asst. Director/ Director., • Provide ongoing structured weekly supervision to housing supervisors and ensures that all staff are receiving adequate training and supervision., • Review and approve all time off requests, time sheets and travel expenses for supervisors within 48 hours of submission., • Meet with the program supervisor and review the staff schedule on a bi-weekly basis. Provide oversight of staffing patterns at all assigned programs., • Ensure that residents receive restorative and/or rehabilitative and tenancy support services appropriate to their needs. In a supervisor's absence, services and billing should be completed by the program manager. In addition, the billing report and accuracy should be reviewed by the program manager after the housing supervisor completes it., • Assure that residents in supported housing and/or apartment treatment programs are receiving regular visits and relevant services from staff, if applicable., • Oversight of medication monitoring procedures in all applicable programs. Program Managers are to review the medication logs/EMAR on at least a monthly basis to ensure all procedures are being followed appropriately., • Monitor occupancy rates and ensure maximum occupancy in cooperation with the management team., • Conduct intakes for potential residents in cooperation with the admissions committee., • Ensure that appropriate discharge planning activities occur for all residents., • Participate in the development, implementation and following of all program policies and procedures in conjunction with the Assistant Director and Director., • Responsible for routine (at least bi-monthly) monitoring of all client records to ensure that records are up to date and include all required documentation in compliance with applicable individual program regulations and policy manuals., • Assist housing supervisors in maintaining consistent and professional collateral contacts to ensure that all program participants are receiving coordinated and adequate services. This includes participation in case conferences when necessary., • Oversight of the sites to ensure a clean, safe and homelike environment. Ensure proper maintenance and upkeep of the site in collaboration with the facilities and management team., • Act as liaison with Property Specialist and/or landlords to ensure all physical sites are maintained and meet safety standards., • Provide direction, guidance and support to staff during business hours., • Be On-Call one week (wed-wed) after business hours and weekends on a rotating basis (approximately every 3-5 weeks) to answer any calls, texts and Microsoft Teams messages in which a supervisor may need to report pertinent information, need assistance, handle a crisis situation or incident/accident., • Ensure that all incidents/accidents/grievances are reported in a timely manner in accordance with applicable regulations., • Ensure that Quality Assurance standards are met or exceeded in cooperation with the QI Specialist and Director., • Oversight of monthly Medicaid Billing in Licensed programs and RTS service delivery in Unlicensed programs., • Oversight of program finances including resident benefits, petty cash accounts, personal needs allowance and other financial issues., • Assume the responsibilities and tasks of all staff roles in their absence. Job Description Under the supervision of the Assistant Director for your region support area, the Patient Services Specialist will be responsible for evaluating and resolving patient grievances and providing conflict management techniques to dialysis providers in order to avert involuntary discharges and access to care issues. The Patient Services Specialist serves as a first level patient advocate for End-Stage Renal Disease (ESRD) dialysis patients and a resource for all outpatient dialysis providers on any ESRD patient questions or concerns. The Patient Services Specialist works with facilities and patients via phone, email, and fax to collect information, evaluate, and work to mitigate challenges between patients and facilities that are not able to be resolved at the facility level. This includes providing advice, technical assistance, and guidance such as conflict resolution, mental health, and socio-economic services and resources. Additionally, the Patient Services Specialist is responsible to document case activity and mail letters and resources to patients who they are working with to resolve a grievance. Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. Job Description JOB DEFINITION: The Psychiatric Nurse Practitioner is to provide Psychiatric Assessments and medication management to children and adults experiencing a behavioral health crisis within a 24 hour per day, 7 day per week to Crisis Stabilization Center. EDUCATIONAL REQUIREMENTS: • Masters' degree in advanced nursing practice in psychiatry from an accredited program., • New York State license and registration as a Nurse Practitioner in Psychiatry., • Current DEA registration., • Certification required in Medication Assisted Treatment (MAT) (i.e., suboxone). EXPERIENCE REQUIRED: Substantial clinical experience with the diagnosis and treatment of the entire spectrum of mental disorders, in outpatient and/or inpatient settings. Experience will include management of seriously and persistently ill clients as well as those with psychiatric and co-occurring substance abuse disorders. Familiarity with current DSM criteria. DUTIES AND RESPONSIBILITIES: • Provides psychiatric and substance abuse care and medication to appropriate clients applying standards of care consistent with good medical judgment and current practice in the psychiatric community. This settlement requires the state to assist residents of impacted adult homes identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap-around services are in place after the move. An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. They will be responsible for making recommendations about the type of housing and the types of services—physical and behavioral health—as well as non-medical services like Meals on Wheels, etc. The Social Work Assessor will complete assessments for Queens Adult Home and SIBN Adult Home Supported Housing Programs. The Social Work Assessor will complete the housing application's psychiatric evaluation and psychosocial portion. As a Clinical Services Administrator, you will: • Assist the Assistant Director to train and support new hires, • Support the Behavior Intervention Specialists by conducting observations, providing onsite support, attending clinical meetings as requested, assisting with the development of behavioral supports, and monitoring behavioral data, • Provide coverage in absence of a BIS, • Oversee the delivery of all psychological services and ensure that services and program environments are of the highest quality, • Design and establish monitoring systems which promote organizational efficiencies as well as compliance with ACLD policies, • Provide BIS services within assigned caseload, • Coordinate and facilitate Psychology Review and special case review meetings and generate meeting agendas and minutes, • Participate in behavioral interventions, including the implementation of SCIP-R maneuvers when necessary, • Participate in all OPWDD surveys of all assigned programs, • Perform quality assurance for the department including tracking due dates of paperwork and completing initial reviews of behavior plans, • Maintain regular communication with the residential administration at the respective programs, • Organize and track, review and submit Human Rights Committee documentation, • Other duties as assigned We require: • Master’s degree from a program in a clinical or treatment field of psychology, social work, school psychology or applied psychology as it related to human development and clinical interventions and documented trainings in assessment techniques and behavior support plan development; or a National Board certification in behavior analysis (BCBA) and a Master’s degree in behavior analysis or a field closely related to clinical or community psychology that is approved by OPWDD; or a New York State license in mental health counseling:, • Minimum of five years of experience working directly with the I/DD population, including the development, implementation and monitoring of behavior support plans; and/or providing supervision and training to others in the implementation of behavior support plans, • In lieu of 5 years’ experience will consider candidates that have obtained OPWDD approved training or experience in functional assessment techniques and behavior support plan development, • Minimum of three (3) years supervisory experience preferred, • Complete required trainings in SCIP (Strategies for Crisis Intervention and Prevention)and CPR, • Clean, valid NYS driver’s license.
