The Care Manager will work with youth up to age 21 who reside in Monroe County and surrounding counties who are Medicaid eligible. This work will be done in a community based setting and will consist of regular home-visits. Regular collaboration with family and team members is required. Director and camp physician on call., • Confers with parents as necessary., • Screens incoming campers for any illnesses, infections, bruises, rashes and other health concerns., • Notifies the appropriate staff members of camper allergies, medications and health concerns., • Establishes a system for health screening for arriving campers and staff, as well as a system for health screening
