Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. They participate in clinical reviews, incident reviews, human rights committee meetings, and other meetings as directed. As schedule allows, the Client Coordinator serves as primary chair for Life Plan meetings. They review documentation and provide technical assistance to program management and staff. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES The employee must be able to interact with individuals and staff professionally and ethically, understand and implement service plans, and complete required documentation clearly, legibly, and on time. Previous experience working with individuals with IDD/mental illness is required, as well as experience managing teams. Competency in Microsoft Word is required. Job Responsibilities Behavior Intervention, Assessments & Analysis • Maintain coordination of active treatment services for people being supported in QSAC Day Habilitation programs in accordance with OPWDD regulations, • Develop/Monitor/Assess monthly goals and objectives to teach the valued outcomes outlined in Life Plans, • Write/Design and revise all individual goals and observe goals implementation by Direct Support (DSP) Professionals, • Maintain clinical paperwork as regulated by New York State (OPWDD) regulations, • Provide written goal summary for and participate in interdisciplinary meetings, • Review data collected and provide monthly updates on progress, • Track due dates for required documents and reports, • Provide training to Direct Support Professionals (DSP) on skills and techniques to support people with autism by implementing the principles of applied behavior analysis (ABA), • Provide direct instruction for goal implementation and proactive and reactive strategies, • Assist with arrival and dismissal, fire drills and assist/support when typical staffing ratios are low, • Collaborate with program supervisors and help with staff disciplinary actions when necessary, • Ensure effective communication regarding behavioral issues of individuals, • Complete staff observations and provide feedback, • Participate and work as part of interdisciplinary team, • Maintain an open, friendly, professional relationship with all staff and families. Looking to fill as soon as possible!!” Job Summary The Habilitation Specialist (DSP), is responsible for assisting the people we support, while following established treatment plans. QSAC’s Residential program emphasizes the importance of promoting independence through the development of daily living/social skills and integration into the community through recreational outings. You will abide by the Direct Support Professional Code of Ethics at all times. The salary range for this position is $18.00 an hour. Shift Information: Flexible Schedule - Mornings, Afternoons, Evenings, Overnights - Weekdays and/or Weekends. May be required to assist with overnight outings.
