Job Description JOB DESCRIPTION Part-Time Public Health NutritionistLocation: Bronx, NY New York Common Pantry is one of the city’s largest hunger relief organizations, providing food assistance, case management services, senior food provision, nutrition education, and hygiene services. NYCP is seeking qualified Part-Time Public Health Nutritionists to join our growing Live Healthy! team. The Part-Time Public Health Nutritionist is responsible for planning activities, events, and workshops, and producing outputs that will lead to healthier communities, focusing on serving the SNAP population. The support is needed for Direct Nutrition Education, Farmer’s Market Tours, Farmshare, Food MD, Recipe Club, and many other community nutrition experiences and opportunities. As a part of the SNAP-Ed III Program, LIVE HEALTHY! offers virtual and in-person interactive nutrition education workshops to children, seniors, and adults throughout New York City, aimed at improving the overall quality of life. The Part-Time Public Health Nutritionist will report to the Nutrition Program Manager at the 788 Southern Blvd, Bronx, NY,10455 location. This is a non-exempt, part-time position, up to 28 hours per week including some Saturdays. Extensive travel on public transit is required for program activities (a monthly MetroCard is covered by the grant). Please note that this is a grant-funded position with funding anticipated up to September 30th, 2026. Continuation of this position is dependent on contract renewal. Responsibilities: * Implement SNAP-Ed III Nutrition Education and PSE Activities throughout Upper Manhattan and the Bronx area. * Lead all aspects of preparation, implementation, and take-down for all assigned SNAP-Ed III activities. * Initiate and engage in communication with community partners about class scheduling and organization. * Ensure that there is adequate supply and materials for each activity. * Confirm that all attendance records and reporting methods are completed accurately. * Assist with the organization and implementation of SNAP-Ed III activities within each project. * Assist with data collection, organization, and data entry and ensure proper data collection techniques are used in acquiring evaluation data and ensure the integrity and validity of the data are secure. * Participate in required meetings, including SNAP-Ed required meetings and weekly team meetings. * Plan and organize work responsibilities according to priorities developed with the immediate supervisor. * Work both independently and collectively to effectively collaborate with LIVE HEALTHY! team members, and our community. * Communicate frequently and professionally with all team players and have a solution-oriented approach to program planning. * Collaborate on other program-related or agency-related activities as assigned by the Director of Nutrition. * Work flexible hours, which may include weekends, as scheduled. * Other responsibilities as assigned. Program Description: PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve. They help clients navigate through what can be a complicated and confusing application process, step by step. Each year, we help over 22,000 people apply for or renew free or low-cost health insurance coverage, 14,000 people to apply for SNAP, and 35,000 eligible pregnant and nursing women and children up to five years of age to enroll in the New York State WIC (Women Infants and Children) Program. Position Summary: Under the general supervision of the ABD Sr. Program Manager and Director of Health Insurance Enrollment, Home Visitor Facilitated Enrollers will engage the aged, blind, and disabled (ABD) populations through home visits, outreach, and at community-based sites. This role will engage the ABD populations who are homebound and assist clients with the application process for public health insurance through home visits 5 days a week (including weekends per request) across the 5 NYC boroughs. Home Visitor Facilitated Enrollers will educate consumers about insurance options, benefits, length and terms of coverage and application process as well as about managed care, health plans, and primary/preventive care. The Direct Support Professional provides direct care to individuals with intellectual and/or developmental disabilities in residential, site-based settings and/or community; assists and inspires individuals to tend to their personal daily living needs and develop the vocational, nutrition, social and personal skills necessary to achieve the highest possible level of independence in the least-restrictive environment. The Direct Support Professional promote productivity, inclusion, independent and individuality. Job Description The RN (Registered Nurse) Case Manager is responsible for providing Rebekah Certified Home Health Agency patients with quality care in a safe and judicious manner. The Case Manager will assist in facilitating and coordinating all services that the Home Health Agency patients require.
