Senior LTSS Assessor (RN) Highlights • The pay for this position is $35.49 – $63.79 per hour, • Monday–Friday, field-based role in the Bronx area, • Must be willing to conduct 2–4 UAS Assessments/Reassessments daily, • Field-based position, • Perform care management duties to assess and coordinate all aspects of medical and supportive services for complex/high-acuity members, • Conduct in-home and community-based UAS assessments to create personalized long-term care plans, • Evaluate service needs and recommend optimal care solutions based on clinical findings, • Develop and continually update care plans/service plans in collaboration with care management teams, • Coordinate providers, specialists, and community resources to ensure members receive appropriate person-centered care, • Monitor member status, clinical symptoms, and progress toward care plan goals; revise plans as needed, • Assess for potential transitions to higher levels of care or waiver eligibility, • Review member data and referral information to identify trends and ensure compliance with state and federal regulations, • Provide education to members and caregivers on conditions, care options, benefits, and community resources, • Document all assessments, care management activities, and member interactions in accordance with regulatory guidelines, • Serve as a liaison and advocate between members, families/caregivers, and providers, • Coordinate service authorizations (e.g., transportation, meals, ADL support, housing resources), • Participate in leadership efforts to improve quality of care and service delivery, • May precept clinical new hires and assist with onboarding, competency checks, coaching, and field training, • May conduct 1+ monthly home/site visits as required, • Active and unrestricted NY Registered Nurse (RN) license, • Residency in the Bronx Area (required for field-based travel), • Minimum 4 years of relevant RN experience , preferably with high-acuity populations, • UAS Assessment experience required, • Ability to conduct home/site visits daily (2–4 visits per day), • Strong assessment, critical thinking, and care coordination skills, • Excellent communication and education abilities to work with members, families, and caregivers, • Ability to collaborate with care management teams and develop comprehensive care plans, • Valid driver’s license and personal transportation preferred (public transportation considered), • Experience in care management , preferably in a Managed Care Organization (MCO), • Knowledge of community resources and service coordination, • Experience working with complex/high-acuity populations, • Familiarity with state and federal long-term care regulations, • Benefits are available to full-time employees after 90 days of employment, • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
