    Position Summary The Care Manager provides services within the Care Management programs, including Health Home Care Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. This position may support Willowbrook Class Members. The core responsibility of the Care Manager is to oversee and coordinate access to services for people with intellectual and developmental disabilities. The Care Manager works with the member, their family and/or representative, and providers to develop, implement, and monitor an integrated and person-centered driven Life Plan, following the completion of a comprehensive assessment process. The Life Plan is the foundation upon which service delivery is built. The Life Plan identifies services that meet medical and behavioral health needs, community, social supports, and other necessary services to support them to live their healthiest and most meaningful life. A key function of this role is being a strong advocate in supporting the member to access needed services to reach their identified goals and live a meaningful and quality life. ACA/NY is a 501(c)(3) not for profit organization that has been designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. ACA/NY is dedicated to meeting the needs of people with Intellectual and Developmental Disabilities by providing comprehensive care management and coordination of service. ACA/NY supports 25,000+ people in its program with support services spanning New York City, Long Island, and the Lower Hudson Valley. Duties & Responsibilities • Deliver person-centered care management services in compliance with regulatory standards and in alignment with the agency’s quality management plan, policies, and standard operating procedures., • Responsible for the completion of a comprehensive assessment/reassessment process., • Identify gaps in service provision and make referrals when appropriate. Advocate on the member’s behalf, to reach their identified goals and live a meaningful and quality life., • Develop, implement, and monitor member Life Plans within required timeframes, by leading an interdisciplinary team planning process, with the person at the center., • Develop strategies that address conflict or disagreements in the person-centered planning process and working with the interdisciplinary team to resolve those conflicts in a timely manner., • Complete all required service documentation with stated timeframes. Ensure existing benefits and other entitlements are maintained., • Ensure a current and accurate information sharing consent is present within the electronic health record and updated as necessary when changes occur or are requested by the member and/or representative., • Coordinates and provides access to high quality healthcare services, inclusive of medical, behavioral health, specialized services. Provides regular communication, monitoring, and action oriented follow up on critical and acute healthcare needs., • Identifies, coordinates, and provides access to preventative and health promotion services as needed., • Coordinates transitional care inclusive of appropriate follow up from inpatient to other settings, discharge planning, facilitating transfers within the healthcare system, residential settings and aging out of childhood services to adult services., • Use health information technology in the delivery of care management services, included but not limited to the use of the electronic health records and programs to facilitate telehealth services for members. For a law firm Responsibilities:  Facilitate and coordinate logistics during negotiations, arbitrations, trials, and settlement discussions; create trial and witness notebooks, identify and organize exhibits, coordinate witness schedules, maintain trial calendars, set up trial spaces, and travel for trial as needed.  Organize and prepare pleadings and discovery documents and support litigation by reviewing, finalizing, and compiling documents.  Assist and manage the discovery process, prepare for depositions, and prepare for expert discovery.  Speak with clients and potential clients to pursue investigations and facilitate case administration.  Assist in case investigations and development by conducting factual and legal research.  Draft, proofread, and format routine correspondence and litigation documents and conforming to applicable rules of federal court, state courts, administrative agencies, and arbitration forums.  Perform and oversee cite-checking, Shepardize case law, confirm case quotes, and proofread briefs and other legal documents.  Maintain electronic case files and manage large document discovery databases.  Interact and liaise with clients and vendors.  Train more junior members of the Litigation Operations team.  Assist with administrative tasks and ad hoc assignments for attorneys.  Proactively assume other duties as necessary. k Qualifications: The successful candidate will have a combination of employment, educational, and life experience that demonstrates the qualities and skills below:  Education and Experience: You have a minimum of 5 years of experience as a paralegal or case manager at a law firm. You have a strong familiarity with federal and state court litigation procedures and experience with trial technology (e.g., CaseMap, TextMap, Sanction). You have a proven track record of supporting attorneys in all aspects of litigation in a highly collaborative environment. Paralegal certification or equivalent preferred.  Leadership: You have experience building, mentoring, and developing a team and cultivating a respectful work environment. You lead by example.  Clear Communication and Attention to Detail: You express yourself clearly and concisely, always highlighting the most important information. You proactively communicate the status of your work and share updates with colleagues when necessary. You ensure that your work is mistake free.  Confidentiality: You must be able to adhere to Firm policies regarding the protection of confidential data and demonstrate sensitivity to (and good judgment in connection with) confidential attorney, personnel, and Firm matters.  Flexibility and Organization: You are versatile and able to respond quickly and positively to shifting demands and opportunities. You are comfortable in an environment where your responsibilities may vary from day to day. You demonstrate a “no job too big, no job too small” attitude, and take a collaborative, team-focused approach to rejuggling priorities and contributing to Firm tasks. You are comfortable balancing tasks of varying levels of urgency and complexity and work well under pressure with tight deadlines.  Ownership, Initiative, and Teamwork: You take ownership of your projects and follow through on their completion. You proactively anticipate, communicate, and work to independently resolve obstacles you encounter in your work. You possess a strong client- service orientation and ability to work well individually and as part of a team.  Physical Requirements: Prolonged periods sitting at a desk and working on a computer.     Help Elect Jasmin Sanchez for New York State Assembly Politics doesn’t change because politicians decide to change it. It changes when people organize and demand it. Our campaign for New York State Assembly in District 65 is building a grassroots movement across Lower Manhattan to fight for housing justice, healthcare for all, climate action, and a government that puts people over profit. We’re looking for interns and volunteers ready to help organize the community and build the future of New York. No experience required. Just commitment and curiosity. What You’ll Do Interns will be part of the campaign’s organizing team and will gain real hands-on experience in electoral politics. You’ll help with: • Door-to-door canvassing, • Petitioning to get on the ballot, • Phonebanking and textbanking, • Social media and digital organizing, • Community outreach in NYCHA and neighborhood organizations, • Campaign events, rallies, and volunteer organizing You will be talking directly with voters and helping grow a grassroots movement.
