Summary This position is a member of our Clinical Department and will work directly with our Clinical Director and requires regular visits to our Residential Group Homes and Day Programs throughout Bergen, Hudson, Essex, Union, and Somerset counties along with meetings both virtually and at our main office in Somerset. The successful candidate will be self-motivated, detail-oriented, and have a strong ability to prioritize under strict timelines and competing priorities. Counties Covered could include but not limited to: Somerset, Mercer, Middlesex, and Monmouth Full-time, part-time, and per diem shifts available! What We Offer: • Comprehensive benefits, including Medical, Dental and Vision Insurance options, • Generous Paid Time Off, • 403B Retirement Savings Plan, • Life insurance, • Ongoing Training and Certifications (Leadership development, CPR, ASL, etc.), • Responsible to evaluate and monitor the health of the clients we serve to ensure their well-being, • Review medical needs of individual referral packets and interview potential clients when needed as member of the clinical team to determine eligibility for Everas services, • Participate as a member of the client’s Interdisciplinary Team (IDT) to ensure individuals receive the necessary medical supports and that their goals are appropriate to maintain their health and medical needs., • Coordinate medical services to enable individuals to live safely and successfully in the community, • Act as a liaison with clients medical and dental practitioners, • Develop protocols for specific client medical needs based on medical orders, train staff on protocols, and monitor to ensure protocols are being carried out correctly, • Responsible to visit all individuals at their homes and day programs to ensure appropriate resources and supports, as well as provide information and training to program staff on topics relevant to client care., • Bachelors of Science in Nursing (BSN), Associates degree in Nursing (ADN), Nursing Diploma, Certificate OR HS Diploma with Accredited Practical Nursing Certificate (LPN) plus 2 two years experience, • Valid NJ Driver’s License, • Required to maintain certification/licensure, • Required to achieve certification in and perform CPR and CMT Through our work to help people thrive and be empowered to contribute to their greatest capacity, we are making a positive difference in the lives of our clients, our employees, and the greater community.Everas provides comprehensive services advocating for the best possible outcome in all areas of a person’s life.
