Job Description Registered Nurse (RN) Case Manager for Foster Care Westchester & Westchester, NY Schedule & Compensation: • Monday–Friday, 9:00 AM–5:30 PM, • Pay: $58–$60/hour Job Summary: The RN Case Manager monitors each child’s health status, medical treatments, and follow-up care. The nurse provides support, counseling, and training to foster parents regarding the medical and health needs of the children. Responsibilities include coordinating services with outside medical providers, performing monthly home visits, accompanying children to specialty clinic appointments, and maintaining accurate and compliant medical records. Key Responsibilities: • Conduct Foster Care visits in the clinic or at a home visit, attend specialty medical appointments as needed., • Monitor and follow up on each child’s health, treatment plans, and medications., • Provide formal and informal health training to foster and biological parents, including medication administration and general health practices., • Communicate and coordinate with families/guardians regarding medical conditions, appointments, and treatments., • Participate in community visits (ER, hospitalizations, specialty clinics, home visits) as needed., • Obtain and maintain health records, including NBS, birth certificates, immunizations, and mental health documentation., • Ensure all records and program practices meet internal, external, and regulatory requirements., • Review ACS triage material and participate in staff meetings, case reviews, FTCs, trainings, and workshops., • Administer PPDs as part of the employee health screening process. Qualifications: Required: • Associate’s Degree in Nursing., • Current NYS Registered Nurse License., • Valid NYS Driver’s License., • Minimum 1–2 years of ICU experience (pediatric experience strongly preferred)., • Experience in acute/sub-acute care or visiting nurse services is a plus., • Bachelor’s Degree in Nursing or related field.
