Key Responsibilities: Conduct client assessments to identify needs, goals, and available resources. Develop, implement, and monitor individualized service or care plans. Prepare reports and participate in team meetings and case reviews. Requirements: Bachelor's degree in Social Work, Psychology, Human Services, Nursing, or a related field (preferred). Previous experience in case management, social services, healthcare, or a related role is preferred. Strong interpersonal, communication, and organizational skills. Ability to manage multiple cases and prioritize tasks effectively. Knowledge of community resources and support services. Proficiency with electronic record-keeping systems and Microsoft Office applications. Ability to maintain confidentiality and professional boundaries. Preferred Qualifications: Case Management certification or relevant professional licensure. Experience working with diverse populations. Bilingual skills are a plus. Work Environment: Office, healthcare facility, community-based organization, or remote setting. May require local travel for client visits, meetings, or community outreach activities.
