This includes the free-standing day habilitation site, individual day habilitation cases and day habilitation without walls programs. The Day Habilitation Coordinator shall oversee the development and growth of the programs with an emphasis on quality and diversity of individualized activities and experiences and shall be responsible for the safety and wellbeing of all the people we serve while in attendance at the program or in the community. There is a direct relationship of the success and growth of the program to the variety and creativeness of the program activities developed. The Day Habilitation Coordinator will embrace the forward thinking ideologies of Adapt of the Hudson Valley?and OPWDD in the movement of individuals from more restrictive and isolative environments to those which foster free thought, individuality and independence. ALL plans developed by the Coordinator of Day Habilitation will embody these tenets. II.?ORGANIZATIONAL RELATIONSHIP The Day Habilitation Program Coordinator oversees two Assistant Coordinators, two shift leaders and 18 Direct Support Professionals (DSP). These staff work on-site and off-site in “without walls” community integrated programs. Requirements: II.REQUIRED DUTIES 1.Shall work with the individuals’ therapeutic team in the development of habilitation plans, behavior plans and safeguards. 2. Shall act as QIDP in the function of approving and certifying habilitation plans., 3. Shall be sensitive to and respect the individual rights, physical and emotional needs of each person and recognize the importance of communicating these needs both verbally and in writing to the appropriate support staff.’, 4.
