JOB SUMMARY The mission of the Patient Advocate (PaD) is to facilitate and ensure continuity in patient care and serve as a conduit of care between the services of the health center and into the community. The PaD will strategically engage employees and their dependents before, during, and after visits to the center in order to ensure we are providing personalized, informative, and compassionate care. The PaD role is supported by and reports to the Director of Patient Care Navigation. The individual will work closely with the Client Relations, Clinical Operations team, Health Center Provider, and Client HR Team. ESSENTIAL DUTIES AND RESPONSIBILITIES Coordination of Care * Under the direction of the Provider, the PaD will coordinate care with external specialists, imaging centers, labs, medical equipment providers, counselors, community resources, etc. * Ensure electronic medical records are provided to Proactive MD’s Provider following any specialist visit and that Provider records are sent to specialists prior to patient visit. * Receive prior authorizations required by the insurance company for tests or procedures and, as appropriate to the Health Center, send prior notification to insurance regarding procedures or referrals. * Continuously refine and update list of value-based providers suitable for referral in the community the Health Center serves. * Assist patients in navigating the healthcare system. * Coordinate care when a patient is admitted and discharged from the emergency room or hospital. Ensure Proactive MD’s Provider is included in the aftercare process. * Visit hospitalized patients to ensure care coordination is directed by the Provider. * Coordinate with "Care Beyond the Walls RN Case Manager" with medically complex patients requiring significant out of Health Center care. Patient Engagement * Engage with patients to encourage use of the Health Center (high risk dashboard or patient engagement dashboard) * Encourage and coordinate follow-up visits at the Health Center. * Work in coordination with the on-site Provider, Client Relations and Client representative to develop outreach campaigns and educational resources. The PaD is responsible for the execution and success of these campaigns * Represent the Health Center at internal and external meetings and events. * Encourage completion of patient survey at end of each Health Center encounter. * Text/email/mail patient communications regarding significant events (birthdays, get well, condolences, congratulations, etc.) Patient Education * Be knowledgeable about and provide information to patients regarding available community services that could enhance their care. * Coordinate patient care and treatment plans; provide educational materials/consultation; follow up on patient well-being. * Assist patients using individual appointments in understanding benefits available to them through their health insurance program (EAP etc.) Health Center Synergy * Coordinate with Clinical Operations in times of high Health Center utilization to ensure that patients are greeted, checked in, and roomed, and phones are answered. * Engage with provider in role of “embedded” case manager for optimal utilization of skills and abilities. * Encourage patient completion of tablet based patient satisfaction survey. * Participate in any Health Center wide meetings to ensure excellent cross communication and collaboration. Job Description Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports, they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. Program Overview The Learning to Work (LTW) for Transfer Schools Program will provide services to students, ages 15-21 that are over-aged for grade, under-credited, and have spent at least one year in another high school to earn a high school diploma and gain valuable vocational and life skills supports. Using the Primary Person Model, each student will have a Young Adult Career Advisor who will: guide students towards removing barriers toward graduation, engage students in college preparatory activities, provide supportive services, and increase self-direction and self-sufficiency through meaningful career exploration experiences, including subsidized internship placements for 20% of the student population. Position: Internship Placement Specialist Reports To: Supervisor II Location: 999 Jamaica Avenue Brooklyn, NY 11208 What The Internship Placement Specialist Does: The person filling this position is expected, under general supervision, to: (1) develop and maintain employer relationships with the goal of identifying and meeting their HR needs; (2) match appropriate students with appropriate internship openings and arrange for interviews; and (3) inform prospective employers and student applicants of the features and advantages of CAMBA's internship placement services; including: • Maintain professional relationships with students and student confidentiality., • Develop, create and/or request internship and job opportunities for students (obtain as much information on the position as possible)., • Research and maintain accurate contact data for all existing and prospective employer/Internship contact information., • Regularly initiate contact with potential employers that are seeking interns in the NY Metro area., • Market CAMBA's internship placement services to new and existing employers., • Build and maintain a list of internship openings in various industries., • Share internship openings with other internship developers in the internship shared database system, if you are unable to fill position., • Identify students who meet the qualifications for open positions., • Match students to internships based on their qualifications., • Inform students of the job duties, responsibilities and all appropriate details regarding open positions., • Review students' resumes to ensure professionalism and accuracy and send resumes to potential employers., • Schedule interviews and coordinate between student and hiring manager., • Prepare students for interviews with specific employers including conducting mock interviews., • Facilitate Job Readiness Seminars, pre-placement orientations, and other workshops as needed., • Request feedback from employers regarding students’ interview., • Collect LTW Agreement Forms for each student., • Collect and send all new hire paperwork for each student to Human Resources Department., • Enter student’s internship start/end dates and internship hours worked into database (such as Client Track)., • Make bi-weekly phone calls to employers to ensure that CAMBA's students are satisfactory employees, to resolve problems that may interfere with performance, and to encourage employers to call CAMBA if a problem should arise., • Create narrative case notes regarding student progress at each point of contact using Client Track., • Update Internship bulletin board bi-monthly using creative and innovative methods., • Follow-up with students after interviews to get feedback and establish next steps., • Once a student is hired, periodically request documentation to verify placement and retention., • Create internship timesheet vouchers and submit voucher to payroll bi-weekly., • Visit each internship site at least one time per month to ensure that interns are satisfactory employees., • Participate in case conferences with program staff and students regarding student progress., • Provide ongoing communication with Young Adult Career Advisors concerning students., • Provide all required information for weekly/monthly/quarterly/annual reports., • Enter student information (resumes, interviews, employment, and retention) in computerized student tracking systems., • Conduct student interviews to evaluate work history, skills, education, training, compensation needs, and other qualifications., • Other duties as assigned. Ability to model the interviewing/presentation skills that students are expected to emulate., • Ability to apply concepts such as fractions, decimals and percentages to practical situations., • Ability to define problems, collect data, establish facts, and draw valid conclusions., • To perform this job successfully, an individual must have knowledge of Word Processing software; Spreadsheet software; Internet software and Database software. Pre and/or Post Employment Requirements: • Must obtain Dept. of Education fingerprint clearance., • Ability to maintain fingerprint clearance throughout the duration of employment., • General pre/post-employment requirements.
