Position: Housing Care Manager – 584 Main Street Position Type: Full time General Description: The Housing Care Manager provides supportive services to individuals and families within their assigned program. Responsibilities include intake and assessment, crisis intervention, developing and monitoring goal-oriented service plans, providing counseling and guidance, facilitating referrals, advocating for participants, and helping them access needed services and resources to achieve their goals. Principal Duties & Responsibilities • Conduct intakes, assess participant needs, and develop individualized service plans., • Provide crisis intervention, counseling (individual, group, and family), and ongoing support., • Facilitate referrals and advocate for participants to access services, entitlements, and supports., • Coordinate care and share information with internal staff and external service providers., • Lead workshops, support groups, and activities that promote life skills and healthy relationships., • Monitor participant progress, compliance, and goal achievement; adjust plans as needed., • Maintain accurate and confidential case records and reports., • Assist with discharge and aftercare planning and maintain follow-up contact., • Participate in case reviews, team meetings, training, and program evaluation efforts. Education: Bachelor’s degree in a human services field, or equivalent education and experience. Experience: Minimum of 2 years working with youth or related populations. Skills: Strong communication, organization, and documentation skills. Attributes: Compassionate, ethical, dependable, and able to work both independently and collaboratively. Cultural Competence: Ability to engage respectfully with diverse populations Valid NYS Driver’s License and reliable transportation (with insurance if using personal vehicle). Ability to climb stairs and lift up to 25 lbs. Access to a home or cell phone and basic computer skills. Participation in ongoing training as required. Specific background clearances may be required depending on assignment.
