MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • Knowledge in three or more of the following areas:, • Experience in supervision of social workers, clearly written and capture assessment findings and recommendations, • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being, • Ensure that Client Care Coordinators are providing short-term counseling, outreach services, crisis intervention, risk assessment, safety planning, and psycho-education for clients, • Collaborate with ACS and/or prevention services agencies, and participate in conferences to advocate on behalf of clients as needed, • Ensure that Client Care Coordinators are providing clients with appropriate referrals and linkages between mental health providers, government agencies, and other related community-based services as needed, • Serve as agency/program liaison between DHS, government agencies, community-based organizations and/or groups, • Develop and Implement group work to be facilitated by Client Care Coordinators to address family issues, enhance life skills etc., • Ensure that Client Care Coordinators are completing all documentation in a timely manner, • Ensure that Client Care Coordinators assess each client/family for supportive housing, complete supportive housing applications when applicable, and accompany residents to supportive housing interviews when needed, • Ensure that Client Care Coordinators complete and discuss exit summaries with each client, and provide clients/families with referrals and supportive services identified in their new community, • Participate in family meetings with case work and housing teams to discuss planning efforts and progress, • Networking and establishing successful linkages with government and community-based organizations to create resources for both families and staff, • Work in collaboration with the Director of Social Services and the Housing Coordinator to ensure that all services lead to permanent housing placement, • Complete special projects and assignments as needed or identified by leadership, • Must work one late a week from 11 am -7 pm to accommodate both staff and families, • Provide on-call crisis intervention support to Client Care Coordinators, • Perform other duties as assigned CLIENT POSITION OVERVIEW Under supervision of the Program Director the Clinical Supervisor will provide clinical oversight and quality assurance to all unlicensed and licensed staff clinicians who provide substance and alcohol abuse services in an OTP program setting. The Clinical Supervisor is responsible for adherence to all applicable licensing, supervision, billing, documentation requirements and overseeing the timeliness and quality of a staff member’s clinical work. The Clinical Supervisor is responsible for client case record reviews, performance improvement and quality improvement to include all required monthly reports. KEY ESSENTIAL FUNCTIONS • Responsible for the daily operations and performance of the OTP program, • Manage OTP clinic operations to budgeted and planned results., • Work closely with staff via weekly supervision to ensure the completion of performance goals. Provides ongoing, regular feedback and performance evaluation to clinicians on the quality and timeliness of their clinical work as well as their caseload. Also solicits feedback from clinicians regarding supervision and whether needs are being met., • Monitor caseloads and ensure the supervisees comply with all agency, clinical, legal and ethical practices. Review and co-sign supervisees’ clinical documentation to include but not limited to methadone dose change request forms, bio-psychosocial history, initial treatment plans and treatment plans reviews., • Becomes familiar with APG and ICD-10 billing codes to include DSM V diagnosis codes., • Oversight and monitoring of billable hours of service required. Provide support, training, and remediation where needed to assist supervisees in meeting program’s billing expectations and caseload requirements, charting, recordkeeping, documentation standards and all other quality assurance standards., • Communicate any concerns about quality of clinician’s work to Program Director in a timely manner., • Become familiar with ACACIA policies and procedures and implements in OTP program. Job Description The RN (Registered Nurse) Case Manager is responsible for providing Rebekah Certified Home Health Agency patients with quality care in a safe and judicious manner. The Case Manager will assist in facilitating and coordinating all services that the Home Health Agency patients require. He or she will be responsible for promoting quality, patient access care, cost-effectiveness within a well integrated healthcare delivery system. CAMBA’s The Landing Family Shelter is funded by New York City Department of Homeless Services and offers families a supportive, structured therapeutic, safe, and drug-free facility. It provides comprehensive case management services to 169 families who are in a transitional residence program for homeless families. The three story facility has rooms of various sizes, common areas and provides on-site security and social services Position: Client Care Coordinator Reports To: Client Care Supervisor Location: Queens, NY What The Client Care Coordinator Does: • Maintain professional relationships with clients and client confidentiality., • Practice Universal Precautions/Standard Protocol & Procedures., • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information., • Assist clients in completing all CAMBA intake applications, screenings and forms., • Create and maintain client files and make file copies., • Conduct initial risk assessment of clients and clients' families., • Prepare initial biopsychosocial evaluation within 30 days of client entry and update according to regulations., • Provide a range of “instant response” services when a client’s immediate well-being and safety is threatened., • In collaboration with clients and case managers, assist in the preparation of initial Independent Living Plan (ILP), including short-term and long-term client goals., • Assist clients in attaining their goals by identifying community resources and by referring clients to appropriate services both within and outside CAMBA (i.e., medical services and psychiatric services, etc.)., • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization., • Consult with others both inside and outside of CAMBA to determine causes of client problems and effect solutions., • Act as advocate on behalf of clients and client families' to arrange for medical, psychiatric, educational and other tests and examinations that may disclose causes of client difficulties and indicate remedial measures., • Attempt to alter clients' and their families' attitudes and behaviors that cause or aggravate problems., • Facilitate or participate in case conferences with all stakeholders in clients life to ensure collaborative approach to services., • Provide all required information for weekly/monthly/quarterly/annual reports., • Lead group counseling sessions and psychoeducation to enhance social development and psychological education of individual clients and provide peer support., • Monitor clients' progress toward their goals via regularly scheduled face-to-face contacts/sessions., • Document client contact via progress notes within 48 hours., • Periodically reassess clients and update services plans and goals to reflect current status., • Follow-up with clients for a period of time after successful completion of their primary goals to assure client stability (aftercare)., • Ensure that all clients’ charts contain all required documentation., • Reach out and market the program internally to the shelter community in order to recruit clients., • Complete weekly unit inspections for clients whom have newborns and/or have ACS involvements, • Complete pre-employment requirements such the State Central Registry (SCR) clearance, The Register of Substantiated Category One Cases of Abuse or Neglect aka Staff Exclusion List (SEL) clearance and Criminal history information checks via fingerprinting with New York State Division of Criminal Justice Services prior to start of employment., • TB test required., • Ability to maintain clearances throughout the duration of employment., • Required to become First Aid/CPR certified., • Required to become certified in overdose prevention., • Bilingual Preferred Status: Full-time (35 hours per week) Benefits: CAMBA offers a comprehensive benefits package including health insurance, dental insurance, 403(b) retirement plan with employer match, paid time off (vacation, personal, and sick time), and paid holidays.
