Our staff members are culturally and linguistically competent and identify with the communities they serve. Family Connect receives referrals from health and social service providers for pregnant and newly parenting NYC residents, screens them for needs and interests, and refers them on to home visiting, doulas, and many other services and supports. Program Description: Contracting and Management Services (CAMS), a division of PHS, administers millions of dollars in public health funding including Federal, State and City government funding, to improve public health and human services for vulnerable communities. Since 2002, PHS has supported the New York City Department of Health and Mental Hygiene (NYC Health Department) as its bona fide fiscal agent for the Public Health Emergency Preparedness grant. As the bona fide fiscal agent PHS receives and expends grant funds on behalf and at the direction of the NYC Health Department, procures goods and services, recruits staff, maintains the budget, prepares and submits to grantors required fiscal reports and budget modification requests. Summary of Position: Reporting to the Portfolio Manager, the Contract Coordinator is responsible for supporting the management and administration of all funding agreements and works in close collaboration with Government Programs Department colleagues and managerial staff but not limited to all other CAMS and internal PHS departments as required for contract payments, compliance, purchase requisitions and contract administration. Under the direct supervision of the Education and Disabilities Coordinator, the Home Visitor shall be responsible for: • Maintaining a caseload of (12) participants, • Providing 90-minute weekly home visits to support parents/guardians in their roles as primary caregivers and to facilitate the child’s individual development within their home environments, • Providing regularly scheduled home visits with enrolled pregnant women to ensure access to comprehensive prenatal and postpartum care and to provide prenatal education on topics such as fetal development, labor and delivery, postpartum recovery (including maternal depression), and the benefits of breastfeeding., • In collaboration with the Education Coordinator and assigned staff, planning and implementing developmentally appropriate, parent/child focused Group Socializations twice per month, • Assisting parents in identifying strengths and goals leading to school readiness, • Working in collaboration with families to identify goals for themselves, their families and their child, and developing objectives for meeting those goals for the Family Partnership Agreement, • Record Keeping: Enters child and family information into the approved child tracking system database (COPA & ACS/DOE systems). Assist with collecting child health medicals and dentals along with entering health related data into COPA. Submits reports as needed and requested, such as monthly summary tracking reports, etc. Maintains and provides data for the annual Program Information Report (PIR).
