Vascular Health Partners Community Care Physicians (CCP) | Capital Region, New York Position Overview Community Care Physicians (CCP) is seeking an experienced or highly motivated Nurse Practitioner or Physician Assistant to join Vascular Health Partners, one of the Capital Region's premier vascular surgery practices.
