(WSFSSH) is a community-based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops, manages, and provides housing for low- income older persons, many of whom live with mental illness and/or have experienced homelessness. WSFSSH currently owns and operates nearly 2,400 units in 29 buildings located in Manhattan and the Bronx. The Claremont is a model housing program and is located in the Claremont neighborhood of the Bronx. It has 90 supportive housing units for persons who are 50 and older most of whom have experienced homelessness, have a serious mental illness and/or substance use disorder.. In addition, there are 24 units of affordable family housing. Job Description: Under the supervision of the Managing Director of Congregate Care, the Claremont Director provides leadership and is responsible for establishing and maintaining a supportive housing community. We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 255-unit residence will provide housing for 153 individuals and families referred by the New York City HIV/AIDS Services Administration (HASA) and the rest 102 units will be apartments for moderate-income households. The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population. Key Responsibilities: • Maintain a caseload and provide supportive case management services to these individuals, • Maintain resident records in compliance with agency and funding standards, • Conduct and review assessments, • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency, • Monitor and ensure residents are linked to healthcare providers, • Provide counseling and crisis intervention as necessary, • Assist residents in securing and maintaining benefits, • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs, • Facilitate groups and activities that promote health and independent living skills development, • Monitor and assist residents with rent payments and budgeting, • Conduct regular unit inspections, • Educate residents about approaches to mental health, • Attend ongoing training to enhance the services provided to the residents, • Assist with the completion of reports as needed, • Bachelor’s Degree in Social Work or relevant field, • Minimum of three years of post-bachelor experience, • Ideal candidate must have a working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness, • Excellent case management skills including knowledge of benefits in the NYC Metro area, • Sensitivity to community issues and needs, • Outstanding interpersonal and communication skills, both oral and written, • Strong ethical standards, confidentiality, and personal integrity Submit your application for review.
