High School diploma or equivalent and three years’ experience working with aging community. RESPONSIBILITIES: Conduct interviews (initial/ reassessment) or follow up interview and phone calls with clients. Coordinate resources to meet all case management needs of program registrants. Provide clients with educational information, case assistance, entitlement counseling referral services supportive contact, and advocacy. Maintain statistical records of client contact for annual cost reports. Under the supervision of the Program Director develop case management care plans. May include provision of short and long-term goal oriented counseling services to clients and specific concrete needs. Be knowledgeable with and entitlement programs and procedures. Maintain and complete current and accurate written records of contact with clients. Develop and maintain liaison with other agencies on behalf of the registrants. Keeping Program Director informed of any significant information or changes on behalf of the client. Responsible for periodically updating client information. Refer to and/or communicate to Program Director or/and any other disciplines any relevant information which impact as registrant. SUPERVISION RECEIVED Respond directly to the Adult Day Care Program Director. CONTACT WITH OTHERS: Regular internal and external contacts involving considerable tact and discretion. PHYSICAL DEMANDS: Light work requiring a limited expenditure of physical effort. WORKING CONDITIONS: Good.
