This role offers the opportunity to utilize your clinical knowledge in various Skilled Nursing and Ambulatory Care settings. Seeking hardworking individuals for 35-40 hours per week. We focus on developing relationships with Nursing and Facility Administration, while maintaining quality care. Candidate should be be an assertive, outgoing, personable and driven. Must be willing and able to commute to the Northern Ocean, Monmouth, Middlesex County areas. Qualifications • A current New Jersey pharmacist license that is in good standing is required. New York License a plus., • 2+ years of experience in a clinical pharmacy practice preferably in skilled nursing consultant pharmacy work., • Pharm.D required., • Excellent Excel and Electronic Health Record skills Job Requirements • Unit Inspections, • Perform Medication Regimen Reviews, • Psychoactive Monitoring analysis, • Controlled Substance Review - including knowledge of using DEA 222 forms. Education • PharmD degree required, • Certification in geriatrics (BCGP) preferred, • Achieve a Certified Consultant Pharmacist (CCP) certification within 1 year of starting. Medication Regimen Review (MRR) • Perform monthly Medication Regimen Reviews (MRRs) in accordance with CMS Appendix PP, state regulations, and facility policies., • Review resident charts for:, • Medication appropriateness / Medication times, • Therapeutic duplication, • Adverse drug reactions, • Drug-drug interactions, • Laboratory monitoring, • High-risk medications, • Psychotropic medication utilization, • PRN medication compliance, • Gradual Dose Reduction (GDR) opportunities, • Anticoagulation management, • Pain management regimens, • Diabetes management, • Antibiotic stewardship, • Make physician recommendations and nursing recommendations., • Review hospital discharge summaries, medication reconciliations, physician orders, laboratory values, nursing documentation, and progress notes., • Monitor for medication-related trends, recurring concerns, and potential adverse outcomes. Skills/Other Requirements • Knowledge of Windows 10 PRO, Excel and Word is a requirement. We require you know how to use a computer., • Excellent time management, communication and interpersonal skills., • Own issue resolution and communicate early warning signs of potential issues., • Knowledgeable with state and federal SNF, ALF regulations., • Must be a professional willing to keep up with clinical knowledge.
