The Account Manager manages a portfolio of residential and healthcare partner accounts, including assisted living homes, group homes, center partners, outpatient clinics, substance abuse facilities, and other community-based settings. Serving as a primary liaison between Genoa Healthcare, pharmacy operations, and client partners, this role focuses on relationship management, proactive account support, and timely resolution of service, operational, and billing issues to maintain and grow the business. The Account Manager works closely with pharmacy, billing, operational, and business development teams to support onboarding, retention, service continuity, growth metrics. Responsibilities include maintaining a quarterly outreach cadence through site visits and remote follow-up, vaccination support, assisting with invoice and past-due account follow-up, and helping deliver a responsive, relationship-centered client experience. The aim of Community First is to create an upstream diversion from arrest and justice system involvement by building supportive relationships with people in need of services and creating linkages to mental health services, housing support, substance use services, and other kinds of social services as needed. Community First is a partnership between Midtown Community Justice Center, Breaking Ground, Fountain House, Times Square Alliance, Housing Works, and Ryan Health. Reporting to the Clinical Director, the Community Navigator will be tasked with engaging community members frequenting the Times Square and Midtown Manhattan area who are experiencing housing insecurity or homelessness and often have co-occurring behavioral health challenges. In keeping with the Community First model, the Navigator will build trusting relationships with those individuals, and facilitate linkages to social services and other community-based resources. The Community Navigator will also work with clients of MCJC’s onsite programs, including court-based and diversion programs, who are unstably housed. Community Navigators are peers who share some lived experience with clients and use their own experience in establishing trusted relationships. THE OPPORTUNITY Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court and community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in so doing, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC seeks Case Managers for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Managers, Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Reporting to the Outreach Worker Supervisor, the Violence Interrupter will work individually and as part of a team to prevent neighborhood shootings and killings. They will provide conflict mediation services and serve as conduits to the program’s Outreach Workers who work with program participants on a long-term basis. THE OPPORTUNITY Launched in early 2020, Manhattan Justice Opportunities (Manhattan Justice) seeks to rethink justice in Manhattan and to build a justice system that is more responsive to individuals’ and communities’ needs. Manhattan Justice is a centralized, court-based screening and referral hub that provides judges, prosecutors and defense attorneys at New York County Criminal Court and Supreme Court with a wide array of social service-based sentencing options in misdemeanor and felony cases. Manhattan Justice is seeking a Case Manager to work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to Senior Social Worker, the Case Manager will be part of a team that provides case management services to the ATI Court. The Case Manager will be responsible for providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Case Manager’s role will be close collaboration with key stakeholders. They will work regularly with the judge and court staff from the New York County Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based clinical work is an exciting, unique and demanding area of practice, and the Case Manager's role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks Case Managers for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Managers, Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, Case Managers will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks a Case Manager for the borough’s Supervised Release Program (SRP) Brownsville/East New York office. Reporting to Supervising Case Manager, Case Manager is the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, the Case Manager will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. BronxWorks Legal Services provides free, high-quality legal representation, and advice in the areas of Crime Victims law, Domestic Violence, Immigration, and Evictions. BronxWorks uses its diverse resources to offer holistic representation and advice with a particular focus on serving low-income and underserved populations RESPONSIBILITIES • Must attend and actively participate in all weekly unit meetings in-person to ensure collaboration, case coordination, and alignment on program goals., • Work collaboratively with the program staff to ensure critical needs of survivors are appropriately addressed by coordinating intake processes that will accommodate survivors who approach the program directly or through referrals from partner organizations., • Carry a caseload for which the attorneys provide representation in either administrative processes or litigation., • Develop training for the ongoing professional development of program staff., • Conduct outreach to underserved and other communities in need., • Ensure that data reporting requirements are met., • Working with a variety of third-party stakeholders, including partner social service agencies., • Maintain accurate and timely data entry in program databases., • Prepare monthly and quarterly reports for funders and internal tracking., • Attend internal and external meetings, including contract and agency trainings., • Provide coverage and program support as needed, including classroom and workshop assistance., • Participate in community outreach events, workshops, and Know Your Rights sessions., • Maintain detailed attendance and service records in compliance with program requirements., • Work at other BronxWorks sites as needed., • In-person work is an essential function of this position and may include, but is not limited to, intake, client meetings, conferences, CLEs, community outreach events with elected officials, and Know Your Rights trainings., • Perform other duties as assigned. QUALIFICATIONS • Bachelor’s degree and at least two years of relevant program or administrative experience., • Experience working with survivors of trauma, immigrants and/or underserved communities is required., • Familiarity with legal advocacy and representation in family court seeking orders of protection., • Familiarity with legal advocacy and representation on immigration matters related the Violence Against Women Act., • Excellent oral advocacy, litigation, and writing skills., • Ability to work independently and in a team., • Ability to work collaboratively and across issue area with diverse communities and institutions, • Fluency in another language commonly spoken in NYC is preferred., • Strong organizational skills and ability to meet deadlines in a high-volume environment., • Proficiency with modern office technology, including Microsoft Office, databases, email, and standard office equipment., • Demonstrated commitment to social justice and working in low-income communities and communities of color to advance equity. RESPONSIBILITIES • Assist with the outreach and engagement of community members in the E Robert Moore and Courtlandt PACT-RAD development, • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up, • Manage a case load, • Monitor and document client progress toward service plan goals, • Document all interactions with, or on behalf of, clients, • Maintain familiarity with program resources available on- and off-site, • Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers, • Complete program reports on a monthly basis, or more frequently as required, • Act as program contact person for inquiries concerning clients, • Provide collaboration and support with securing benefits, especially as it relates to access to benefits services and eviction prevention efforts, • Report to, and meet with, supervisor on a monthly basis, • Conduct home visits and other field work, as required, • Coordinate and facilitate monthly activities that fit the needs of the PACT-RAD community, • Perform additional administrative duties as assigned, • Assist with community events as needed and serve as a liaison with community vendors QUALIFICATIONS • Bachelor’s degree with case management experience or High School Diploma with 2 years of case management experience, • Experience using Access HRA and securing HRA entitlements and benefits preferred, • Mental health background preferred, • Bilingual preferred, • Crisis prevention and mental health first aid training is required within first 6 month, • Strong oral and written communication, time management and organizational skills are necessary., • Proficiency in Microsoft Office suite and other standard business technology is required.
