Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a part-time Nurse Practitioner to join its busy team at our St. Ann's Health Center located in Bronx, NY. A Nurse Practitioner, under the direction of the of a physician, is responsible for planning, evaluating and delivering the health care of patients on assigned service. Emphases preventative health measures and education of patients. She/he will perform other duties as assigned and in compliance with systems and job specific standards. Essential Duties and Responsibilities for a Nurse Practitioner: • Takes histories, performs physical examinations, gives immunizations (as necessary) and does patient education and counseling., • Orders appropriate lab work draws blood for lab work if needed and performs some lab tests such as wet prep microscopic exams., • Prescribes treatment for minor illnesses, and consults with physician regarding medication, chronic illness and referrals when appropriate., • Maintains legible and appropriate documentation and reviews records periodically. Does follow-up of abnormal pap smears and reportable STDs., • Attends case conferences involving patients., • Attends continuing education programs to increase skills and knowledge., • Attends provider meetings and participates in quality improvement programs and team assignments., • Identifies and categorizes each patient's age-specific grouping of needs, such as those infants, adolescent, or geriatric patients., • Follows OSHA standards of infection control under CDC guidelines endorsed by department, organization, and overseen by Infection Control Officer. Minimum Work-Related Experience: 5-8 years; 4 years of business office experience, preferably in a large medical practice with increasing levels of responsibility.
