Job Description HHA or PCA Coordinator Full-Time or Part-Time | Nassau County, NY | Weekend On-Call Required About Us We are a well-established, family-owned home care agency serving Nassau County for over 30 years. We are seeking an experienced HHC / PCA Coordinator to manage caregiver scheduling and match HHA / PCA with patients. This is a critical role responsible for keeping cases staffed, caregivers supported, and patients receiving consistent care. Position Overview As a Home Care Coordinator, you will be responsible for coordinating care between our HHAs and patients. This includes scheduling, case management, and ensuring all shifts are properly staffed. This is NOT an entry-level role. Prior home care agency experience is required. Key Responsibilities • Match HHA/PCA caregivers with patients based on needs, availability, and location, • Manage daily scheduling and fill open shifts quickly, • Coordinate new case starts and caregiver assignments, • Communicate with caregivers and patients to ensure coverage and satisfaction, • Handle call-outs, emergencies, and last-minute staffing needs, • Maintain accurate scheduling records and documentation, • Work closely with office staff to ensure compliance and continuity of care, • Minimum 2+ years experience in a licensed home care agency (REQUIRED), • Strong experience scheduling HHAs or PCA caregivers, • MUST know how to use HHA exchange software program, • Ability to manage multiple cases and urgent staffing needs, • Excellent communication and problem-solving skills, • Experience with Nassau County cases or NY home care environment, • Knowledge of HHA/PCA certification requirements (New York State), • Full-Time or Part-Time available, • Family-owned agency with strong reputation, • Paid holidays, • Paid sick days, • Stable, long-term opportunity, • Supportive team environment, • Opportunity to play a key role in patient care coordinationCompany DescriptionLong Island Care at Home is a locally owned and operated enterprise.
