Weekend Field RN Key Responsibilities • Perform home care assessments to determine eligibility and develop care plans., • Provide skilled nursing care, including wound care, medication management, and disease education., • Coordinate and communicate with patients, families, physicians, and interdisciplinary teams., • Complete documentation in HCHB clinical software within required timeframes., • Supervise and orient home health aide personnel as needed., • Identify needs for additional services (e.g., PT, OT, MSW, speech therapy) and coordinate accordingly., • Monitor patient progress, adjust care plans as needed, and participate in case conferencing., • Ensure compliance with infection control standards and agency protocols.
