Job Description The Mental Health Association is seeking Per Diem Peer Support Specialist for our new program, 988 Mobile Crisis Outreach Team, operating out of Hudson county. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shifts are on an as-needed basis for this position. Summary Job Description Coaches are paired with up to 15 young people and work with each of them from 9th grade (or age 14, if they are disconnected from school) to age 21, regardless of their academic status. Through the course of their work with assigned youth, coaches’ general functions include the following: • Building relationships with up to 15 young people, • Provide ongoing social/emotional support to the youth on their caseloads, • Work 1:1 with young people to develop individualized academic and career development goals based on their interests and strengths and take measurable steps towards their goals, • Connect young people to quality schools, programs, resources, and opportunities in line with their goals, • Help young people persist in these settings, navigate any challenges that arise, celebrate successes, and plan/prepare for the next step on their journey, • Should a youth not be successful in any particular school/program setting, Coaches help young people reflect on the experience and work with them to set new goals and transition to the next opportunity, • Collaborate with Case Planners, Tutors, Specialists, and foster/birth parents, as needed, to help youth achieve their goals and ensure their overall well-being, • Connect young people to peer group supports and build positive relationships, • Ensure young people who are aging out can access and maintain stable, affordable housing and build independent living skills Principle Responsibilities: ACADEMIC • Re-engage disconnected youth and help them re-enroll in an academic setting, • Assess the fit between the young person and their current academic setting and help them transfer into a better-fit high school or high school equivalency (HSE) program, if needed, • Visit the young person’s school/program and build relationships with school staff, review transcripts and academic data, provide educational advocacy, and discuss progress towards graduation/HSE obtainment, • Ensure young people are attending school and have the supports and resources they need to be successful, such as tutoring, individualized educational plans (IEPs), 504 Plans, or other special accommodations, • Expose students to post-secondary pathways, such college and/or accredited vocational programs, starting in the 9th grade (or as early as possible), • Assist students with post-secondary planning, along with the guidance counselors and/or College Specialists and Career Development Specialists, • Connect the student to the College Specialist to apply to colleges, opportunity programs, and all forms of financial aid, funding, and scholarships, • Provide college persistence coaching, including ensuring students are receiving all of the financial and academic supports they need to succeed each semester, • Assist students with planning for apprenticeship and vocational education paths, as appropriate, • Collaborate with Case Planners, guidance counselors, Specialists, Tutors, teachers, family members, and caregivers CAREER DEVELOPMENT • Assist young people in obtaining working papers, if needed, • Conduct career exploration activities with young people, • Assist ALL young people in selecting at least one career development experience each year that is in line with their interests and academic situation/background, • Work with the young person and Career Development Specialist to identify potential longer-term career pathways based on the young person’s interests, strengths, and academic/vocational plans, • Connect the young person to the Career Development Specialist, if needed, to explore workforce development programs, vocational programs, and for assistance with job applications, • Connect young people, as needed, to agency-based job readiness and/or internship programs (e.g., YA WORC or the Mentored Internship Program), • Provide persistence coaching to young people enrolled in any external program by checking in with them regularly HOUSING AND INDEPENDENT LIVING • Ensure that the young person coordinates with a Housing Specialist (and/or Case Planner, if the agency does not have a Housing Specialist) to ensure that all young people’s housing applications are submitted as early as possible (by age 19 if their goals is to age out), • Accompany the young person to visit supportive housing residences, when needed, • Help young people move in and obtain furniture, • Help young people maintain their housing by providing independent living and life skills supports such as budgeting, obtaining a bank account, and advocating with landlords, if needed Minimum Requirements: • Qualified applicants for the Coach positions will be required to have a Bachelor’s degree and at least three years of experience working with at-risk or system-involved young people or an Associate degree and 5+ years of working with at-risk or system-involved young people, • Strong written and verbal communication skills, • Strong time-management, organizational skills, and attention to detail, • Ability to troubleshoot, advocate, and navigate obstacles where there is not always a clear-cut pathway, • Open to learning how to use and interpret data to inform decision-making, • Ability to effectively collaborate in team settings and build relationships with external partners, • Ability to use Motivational Interviewing techniques, • Ability to use trauma-informed techniques, • Experience working with vulnerable youth, • Knowledgeable about youth resources and services, • Ability to facilitate therapeutic peer groups, • Competence in planning, organizing, scheduling, case documentation, and strong oral and written communication abilities. Some experience in public relations preferable, • Must possess strong human relations skills to communicate and work effectively within a culturally diverse work environment and community, • Valid NYS Driver’s License if using personal vehicle or agency vehicle to drive on agency business. Park Slope Shelter for Women serves 100 MICA (mentally ill chemical abusing) women at the Park Slope Armory in Brooklyn. Position: Engagement Coordinator Reports To: Director of Social Services Location: 1402 Eighth Avenue, Brooklyn, NY 11215 What The Engagement Coordinator Does: • Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders., • Provide direct interventions with the clients that are culturally appropriate and take into account the client’s communication styles, values and life experiences., • Provide non-direct interventions that support the shelter team and increase communication between functions., • Conduct and document one-on-one supervision with one or more of the following staff; Recreation Coordinator, Adult Literacy Specialist, Peer Counselors and the Employment Specialist in order to engage clients with the skills to obtain employment or enroll in vocational training programs with the ultimate goal to secure housing., • Reinforce, support and integrate trauma informed care, de-escalation techniques and motivational interviewing interventions., • Track and analyze incidents and assist in incident debriefings including; providing crisis management assistance, interventions, and follow up with clients who have patterns of maladaptive behaviors, and education for staff on how to manage incidents from a trauma informed perspective., • Develop workshop curriculum for one or more of the following; Recreation Coordinator, Adult Literacy Specialist, Peer Counselors and Employment Specialist programming components., • Develop community based resources and supports towards agency housing and engagement efforts., • Conduct Crisis Prevention (CPI) trainings for staff members to better inform them of tactics to use to deescalate and appropriately handle incidents within the shelter., • Ensure attainment of funding objectives and performance measures., • Conduct field visits to increase community and citywide resources., • Create and implement incentive system to increase client participation in on-site and community activities., • Plan and organize program activities to maximize program contract's goals and performance targets as outlined in DHS’ Performance Incentive Plan and grant contracts., • Motivate, coach and counsel direct reporting staff to excel., • Ensure that all direct reporting staff completes all necessary case management documentations including individual and group case management notes, housing applications, individual living plans (ILP), and referrals to providers including psychiatric referrals are completed as required and up to date., • Encourage teamwork among direct reporting staff as well as among peers., • Troubleshoot client and direct reporting staff program problems and make decisions in accordance with program policies, procedures, and protocols., • Administer constructive discipline to direct reporting staff, as needed., • Input client data using DHS’s CARES database, as needed, • Monitor clients' progress toward their goals (dates achieved) and document via progress notes., • Review all documentation related to clients' progress for accuracy, completeness, and clarity., • Review and sign time sheets., • Prepare performance appraisals for directed reporting staff., • Conduct chart auditing of client files (i.e., paper and electronically)., • Ensure staff utilization of electronic database and conduct oversight to ensure data integrity., • Manage own time effectively and coordinate program activities to maximize time of direct reporting staff., • Lead and participate in administrative and staff meetings as requested., • Provide all required information for weekly/monthly/quarterly/semester/annual reports to CAMBA management and/or to funders., • May conduct orientation sessions for new clients arriving in shelter to orient the client to the rules and regulations of the shelter., • May ensure the monthly event calendar is inclusive and includes both on-site and external activities, • May prepare marketing materials for the program., • May conduct initial intake or assessment of clients and/or clients' families and periodic reassessments., • May have direct client service/program responsibilities in addition to the above., • Bachelor's degree (e. g., B.A., B.S.W.) or equivalent relevant professional experience, with a minimum of two years of applicable experience; Master’s Degree preferred. (A license is required of these Master's Degrees: Social Work (LMSW or LCSW), Mental Health Counseling, Counseling/Guidance, Art Therapy).Other Requirements:, • May be required to become First Aid/CPR certified., • May be required to become certified in overdose prevention., • Obtain Train the Trainer CPI certification in Non-Violent Crisis Intervention within 90 days of hire, • Obtain NYS Peer Specialist Certification within 90 days of hire, • Bi-lingual preferred.
