Job Description Founded in 1974, Community Access continues to uphold and operate as a pioneer of supportive housing and social services in NYC for people with mental health concerns. Facilities management is an organizational function that ensures the comfort, functionality, and safety of the CA’s premises to create a conducive living and working environment for everyone. Position Overview Under the supervision of the Area Superintendent, the Maintenance Mechanic assists with maintaining agency apartments and facilities as needed and directed by the Area Superintendent. Assistance includes cleaning, disinfection, trash removal and other building maintenance tasks, as well as minor repair in the areas of masonry, carpentry, electrical, plumbing, painting and overall maintenance of residential building systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. Community Access’ IMT Teams serve individuals who are street homeless or living between institutional settings and housing. IMT team members share responsibility for the people they serve, helping individuals attain housing and other personal and safety goals that may be of a health, social, work or education nature. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Program/Department Description Community Access’s OMH licensed Intensive Crisis Residence (ICR) is inspired by the Soteria model and is designed as an alternative to psychiatric hospitalization for people going through an acute psychiatric crisis and who would benefit from a respite period in a stabilizing, home-like environment with 24-hour peer support and treatment services. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. Position Overview The Assistant Program Director is responsible for supporting the Program Director in the overall administration and the day-to-day operation of the ICR. This includes providing leadership, guidance and support, policy and procedure development and implementation, scheduling, staff recruitment, training, supervision and evaluation, management of physical plant, quality assurance and quality improvement activities, internal and external reporting and billing. Job Description Summary: The Staten Island Community Program Clinical Director utilizes outstanding management skills and effective leadership to oversee Safe Horizon’s community-based program located on Staten Island. The Clinical Director will play a key leadership role on Staten Island, acting as a voice for Safe Horizon with public officials, city agencies, and other service providers. Internship Description/Outcome: The Howie the Harp Internship is an opportunity for an individual to learn and refine skills needed to participate in the field of social services within New York City. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. Position Overview: The Program Director is responsible for the overall administration and the day-to-day operation of the Soteria Intensive Crisis Respite (ICR), ensuring all contractual obligations are met and all operations are mission-aligned. Program/Department Description Howie the Harp Advocacy Center (HTH), which is run by professionals who have personal experience with the mental health system, offers an array of services that help individuals with mental health concerns find meaningful, permanent employment and develop careers in human services. All HTH programs are based on the core values of self-determination and peer expertise. Through the Comprehensive Peer Training Program, Assisted Competitive Employment Program (ACE), and other initiatives, the Academy offers a wide range of services including training, internship experience, job placement and support, and continuing education services. Our Ongoing Integrated Supportive Employment (OISE) program provides extended job coaching and career support services that are necessary to help people maintain employment or to place them in a new job if a job is lost or when appropriate for career development. These services provide extended mental health- focused employment supports related to helping sustain employment for individuals who have achieved job stability. Ongoing meetings with the individual are required for routine interventions to monitor their work performance and may include worksite job coaching or meeting with the employer regularly if requested by the participant. Position Overview The Employment Specialist provides person-centered employment support for individuals with mental health concerns. The individual in this role will primarily be supporting people who are presently employed but will also be supporting individuals in the process of securing new employment, in instances when participants are looking to connect to new employment opportunities. The Employment Specialist will also be responsible for reviewing and responding to OISE applications and admitting and discharging individuals from the program. Through a customized array of services delivered both on site and in the field the Employment Specialist develops connections to community employers and provides support and skill development toward success and satisfaction in the participant’s chosen field of employment. Key Performance Indicators All key performance indicators below are expected to be met fully. • All contacts are documented in Progress Notes within two business days of the contact., • All participants will have at least one monthly meeting., • At least 50% of participants will retain their jobs for 90 days. Health and Wellness initiatives at Community Access aim to improve the overall health and wellness of participants and reduce isolation and loneliness. Program activities include urban farming (growing vegetables, herbs, flowers, fruits), cooking, farm to table community meals, universal movement groups and classes, DIY workshops and activities, Plant Access, group bicycle rides, outing exploration groups, overall skill-building and skill sharing workshops, etc. The Health & Wellness team centers the work on trauma informed, peer expertise to create a CA wellness culture that is driven by lived experience and self-determination, harm reduction and health justice. Department Overview The Residential Crisis Support Program, also known as the Crisis Respite Center, provides a short-term peer staffed alternative to psychiatric hospitalization in a home like setting. We aim to create a warm, friendly, and supportive environment where guests, as the participants are called, can learn to use new recovery and relapse prevention skills. This is achieved through 24-hour peer support, self-advocacy education, self-help training, and mutual understanding. The program offers people experiencing a psychiatric or emotional-related crisis with a short-term stay (starting at 14 days and no more than 28) that allows individuals to maintain daily activities and connect with community-based services and supports.
