The Quality Assurance (QA) Radiological Technologist position encompasses responsible, administrative and clinical activity of varying degrees of latitude. Clinical level judgement is needed in the daily supervision of radiological functions and patient care, and departmental projects. Responsibilities include: • Review exam orders, consult with radiologist, schedule patients and arrange for transportation as indicated., • Identify patient and document all information pertinent to final report in the RIS and on images (PACS)., • Prepare all patients (infants, children, adolescents, adults and geriatric) for radiological procedures requested by the clinician. Arrange for proper protection of patients, public and staff from infectious diseases/materials., • Perform all general diagnostic imaging and routine examination in CT and Angiography., • Assists in performing daily checks of radiographic imaging equipment, immobilization devices and other necessary accessories., • Enters data and maintains documents and logs as required and mandated by the department QA guidelines and various regulatory agencies., • Maintains adequate stocks of supplies and materials needed for use in the department., • Checks equipment for defects prior for use and assures proper use of such equipment through instruction and review., • Report any unexpected patient incidents in accordance with the occurrence report protocol., • Maintain and exercise all mandated radiation safety regulations. Maintain basic care of all the imaging equipment. All malfunctions are to be reported to management staff whether mechanical, electrical or suspect and service request documentation completed. Maintain a clean and safe work environment for both patients and staff., • Perform other duties as deemed necessary (by supervisory or administrative staff) in the event of emergencies or disasters., • Continue professional growth and development of job-related skills on an ongoing basis. Participate in continuing education activities., • Ensure all examinations are accessioned, documented and sent to PACS for reading. Monitor PACS and RIS incomplete worklists to ensure all exams are completed., • In accordance with HIPAA (the Health Insurance Portability and Accountability Act), staff members will exercise due diligence in ensuring patient privacy and confidentiality., • Identifies activities that the network has implemented to promote patient safety and takes all necessary precautions to ensure a safe environment for patients, visitors and staff. In addition to the Radiology Technologist job functions, the QA Technologists is responsible for the following: • Organize and file maintenance reports for each building in regards to dentistry and radiology x-ray equipment and ensure dentistry and x-ray equipment follow the Department of Health codes, policies and procedures., • Participates in quality assurance activities, including coordination, control and maintenance of technical equipment., • Establish a daily/weekly/monthly quality control routine for monitoring, evaluating, and maintenance of X-ray equipment to ensure optimal performance and stability., • Documenting and maintaining records for the quality control program in accordance with applicable regulations, legal requirements, accrediting agencies and recommendations from equipment manufacturers., • Performs research and participates in special projects involving evaluation of equipment and delivery of service., • Work with our radiologists to obtain updated documentation of primary diagnostic monitors., • Distribute/collect dosimeter badges from each employee/facility that works with x-ray equipment., • Recommends area assignment of personnel to coordinate activities within the department with other activities and services and represents the department in interdepartmental operations and meetings., • May be assigned duties and responsibilities of X-ray Director in regards to radiology department workflow. Minimum Qualifications: Assignment Level III 1. For Supervisory Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and two (2) years of Radiologic Technologist experience performing duties consistent with Assignment Levels I and II, one (1) year of which must have been in a supervisory capacity., 2. Duties & Responsibilities Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Unit Chief is the mental health administrative lead in each facility or designated area, including general population and therapeutic housing units. Under the direction of the Director of Mental Health Administration, this full-time position coordinates, and helps manage, mental healthcare delivery in one or several of the jail facilities in New York City. This includes ensuring provision of services within the timeframes and quality benchmarks outlines by CHS MH policies, providing administrative support to staff who are assigned to their clinical area, maintenance of patient records and compliance with monitoring requirements. S/he will act as the principle administrative liaison between mental health disciplines, as well as pharmacy, medical and nursing. As a member of the facility management team, the Unit Chief will collaborate closely with the Department of Correction (DOC) leadership and CHS Operations in order to ensure service delivery and meet patient needs. A major component of this position will be providing effective, inspiring, and clinically-informed administrative leadership and organization. Responsibilities Include: • Manage the daily operations of the program and site; ensure that patient and program goals are met in accordance with established H+H policies and procedures, • Ensure adequate multi-disciplinary mental health staffing in the facility, • Coordinate therapeutic services; oversee the referral, intake and assignment of cases, • Participate in relevant quality improvement initiatives; assure completion and accuracy of all data completion, report progress and plan of action in monthly QI meeting, • Provide orientation and training to all new hires, • Provide administrative and clinical support to mental health staff; proactively identify and resolve performance issues and manage personnel issues with discipline supervisors, • Support staff development; teach and ensure completion of departmental trainings designed to enhance the skills of staff relevant to their position, • 10% of time must be devoted to direct clinical care, some of which may include intake evaluation, suicide watch assessment, and clinical follow up’s as needed., • Organize and facilitate monthly mental health staff meetings with other MH discipline leadership, • Attend and actively participate in multiple interdisciplinary meetings per week, both within the mental health department and with other services, including DOC, • Supervise the collection and appropriate dissemination of statistical reports, staffing patterns, bed utilization, timesheets, payroll activities and HR paperwork., • Maintain on-call availability for urgent or emergent administrative issues that arise off-hours, • Other duties as assigned by the Director of Mental Health Administration or her/his designee Minimum Qualifications 1. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and, 2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity. The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. Administrative Responsibilities: • Offer direct administrative and clinical supervision for the infectious disease service, encompassing ID clinicians and the Medspan team., • Oversee staffing requirements for the infectious disease service within the facility., • Direct the formulation and execution of workflows and protocols pertinent to the infectious disease service., • Organize and lead regularly scheduled rounds and meetings for the ID team., • Facilitate conferences and seminars with other clinical services and both CHS-based and external re-entry programs., • Provide supervisory clinical guidance to the SMD and staff of the communicable diseases unit (CDU)., • Oversee admission and discharge criteria for patients with confirmed or suspected communicable diseases within the CDU., • Lead patient tracking initiatives within the CDU, monitoring clinical conditions, medication regimens, and laboratory results., • Collaborate with and provide clinical consultation to the infection control team., • Engage in initiatives focused on system-wide education, quality improvement, and population health., • Direct projects, analytical efforts, patient tracking, and quality improvement initiatives for the infectious disease service., • Cooperate with relevant external agencies to ensure high-quality, data-driven continuity of care for patients with communicable diseases admitted to or discharged from correctional facilities. Medspan Directorship: • Provide supervisory direction and guidance for the Medspan Program., • Oversee the creation and implementation of staffing, workflows, and protocols for the Medspan team., • Direct regularly scheduled rounds and meetings for the Medspan team., • Lead research initiatives, patient tracking, and quality improvement efforts for the Medspan team. Minimum Qualifications Applicants must have all of the following: 1. Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. 1. Completion of an acceptable internship in an approved hospital., 2. A license to practice medicine in the State of New York., 3. Certification by American Board in field of specialization or a faculty appointment of Associate Professor or higher in a medical school recognized by the American Medical Association., 4. Eight (8) years of satisfactory full-time paid experience in field of specialization including at least three (3) years in a supervisory or administrative capacity.(Training and experience required for certification will be credited toward required specialized experience.) New York City Health and Hospitals Corporation Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Additionally, while CHS seeks the most qualified individuals for these positions, preference will be given to equally qualified, internal candidates. • Time: 8:00 AM - 4:30 PM, • Days: Monday to Friday, • Location: OTxHU-Bellevue/Rikers Island Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to patients with complex chronic disease in the New York City jail system, under the supervision of physician leadership. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services. The Physician Assistant will participate in an interdisciplinary team under supervision of a Site Medical Director for patients who have chronic medical conditions or physical disabilities who do not require acute inpatient admission. Responsibilities include: • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated., • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable., • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicare about 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards., • Maintain clinical competency by participating in all CME and CHS training and in-service requirements., • Maintain your schedule as directed with particular attention to punctuality and timely notification of absences., • Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed., • Complete tasks as delegated by a Site Medical Director or other supervising clinical team member., • Maintain and provide documentation of all required credentials., • Maintain professional attitude and appearance., • Adhere to Occupational Health Services requirements. Minimum Qualifications 1. Valid limited permit to practice as a Physician Assistant issued by the New York State Education Department (NYSED); or, 2. Position Overview Under the general direction of the Director of Safety & Environmental Health, or designee, the Deputy Director of Safety & Environmental Health is responsible for developing, implementing, and monitoring adherence to CHS policies and procedures to ensure a safe working environment. This role oversees occupational safety training and on-going education to staff while working collaboratively with Human Resources, Facilities, Clinical Leadership, and external regulatory bodies to mitigate safety risk and reduce potential environmental hazards. Key Responsibilities • Implement and maintain safety standards and protocols that comply with local, state, and federal regulations. This includes conducting routine safety inspections and risk assessments to identify potential hazards., • Conduct in-service education to healthcare personnel on safety procedures, including but not limited to situational awareness, workplace violence prevention, and emergency response actions., • Represent CHS as needed in internal or external committees and workgroups where Safety representation or perspective is required., • Compile workplace violence and staff injury reports for distribution to required entities., • Assist in maintaining workplace violence, occupational injury, and other safety-related databases and logs., • Analyzes data to identify trends and areas for improvement in safety practices., • Respond on-site to serious incidents of workplace violence, occupational injury, or environmental hazards, to gather factual information and develop a detailed report of findings., • Assist in conducting post-incident well-being checks for employees who were involved in a workplace violence incident or who may have sustained a severe work-related injury., • Conduct and prepare written summaries of safety rounds performed at Rikers Island, community warehouses, outposted sites, and court-based facilities., • Aid in establishing methods to improve safe clinic operations while reducing the risks associated with workplace violence incidents, environmental hazards, and overall staff safety., • Performs regularly scheduled and unannounced walkthrough of clinic facilities and other work areas to identify, document and address potential workplace concerns in a timely manner., • Assist in facilitating monthly and quarterly workplace violence and safety meetings with key stakeholders., • Other relevant assignments as required. This full-time position reports either directly or through a Program Director to the Clinical Director of Psychiatry in the Division of Correctional Health Services and is responsible for the clinical oversight, management, and innovative development in specific mental health treatment areas, including standard mental observation (MO) units, specialty MO units and general population clinic settings in one of the 12 jail facilities in New York City. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Clinical Supervisors who are qualified for and interested in providing care on the PACE or CAPS units (specialty MO units) for individuals with serious mental illness will report to a Program Director for direct supervision. Responsibilities Include: • Formulate and implement procedures for operating, monitoring, and evaluating mental health programs and services in assigned area of responsibility, • Develop and lead staff training programs and exercises to maintain a culture of learning and clinical excellence, • Serve as the clinical leader for the assigned area of responsibility and provide weekly individual and in-person supervision for all assigned mental health clinical staff; review diagnostic, treatment, and level of care decisions and documentation; ensure adherence to policies and procedures; and conduct clinical case conferences, • Lead daily multi-disciplinary meetings with all mental health staff in the assigned area of responsibility, including DOC when appropriate., • Provide quality assurance and quality improvement as directed by the Clinical Director of Psychiatry, • Coordinate re-entry services with the social work staff in the assigned area of responsibility, • Meet regularly with the Clinical Director of Psychiatry or assigned supervisor to provide meaningful input into program design and policy formation, • Personnel management of the clinical staff, with assistance from the Clinical and/or Medical Directors as needed, including recruiting, mentorship, supervision, and disciplinary procedures, • 20% of time must be devoted to direct clinical care, some of which may include supervision of trainees. Timely and clear documentation of supervision and clinical care is expected. Minimum Qualifications: 1. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and, 2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Under general supervision of the Medical Director, the Senior Psychiatrist is the psychiatric leader of the facility or program and works in close collaboration with other discipline leadership to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong clinical support and oversight, leadership and mentorship for existing staff. Responsibilities • Develops workflows, policies and quality metrics for their dedicated area in close coordination with the Medical Director and Chief of Mental Health, • Recruits, hires and trains staff to provide care in their dedicated area, • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with other leadership: design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. JAIL POPULATION REVIEW PROGRAM The Jail Population Review Program is an ambitious initiative in New York City that is led by the Center’s New York Legal Policy (NYLP) Team–court practitioners, clinical staff, and research and policy experts focused on developing and implementing critical operational and policy reforms and improving coordination between justice system stakeholders. The Jail Population Review Program reviews incarcerated individuals for opportunities for pretrial release or case resolution. The team provides critical information to prosecutors, defense attorneys and judges at the earliest possible juncture so they can engage in meaningful dialogue before an individual spends weeks or months in jail. THE OPPORTUNITY This postdoctoral fellowship in forensic psychology is a collaboration between the Center and Columbia University that focuses primarily on forensic assessment with an opportunity to provide psychotherapy to individuals re-entering the community from incarceration. Postdoctoral fellows conduct forensic assessments mainly for the Jail Population Review Program. The Jail Population Review Program provides a number of clinical consultation services, including independent clinical assessment, review of medical records, and psychological testing when indicated. The postdoctoral fellowship is a one to two-year, full-time program providing advanced training at the intersection of psychology and law. Fellows gain supervised experience conducting psychological evaluations in the criminal court context, primarily brief and comprehensive clinical assessments of incarcerated people, as well as violence risk assessments (general violence, intimate partner violence, and sexual violence) using the Structured Professional Judgement (SPJ) approach, criminogenic risk and needs assessments, and mitigation psychological evaluations. Fellows also conduct psychological evaluations and testing to assist the courts in developing treatment plans for those involved in court-mandated treatment, such as mental health courts or other alternative-to-incarceration programs. In collaboration with Columbia University, fellows may conduct psychotherapy with one participant of the Columbia Reentry Initiative under the supervision of Columbia’s faculty or audit a graduate level forensic psychology course at Columbia University (Forensic Assessment or Correctional Psychology). The Columbia Reentry Initiative at the Dean Hope Center for Educational and Psychological Services at Teachers College is a no-cost, evidence-based and trauma-informed psychological treatment program designed to support adults reentering to the community following incarceration. Postdoctoral Psychologists attend weekly multidisciplinary case discussions and monthly didactic seminars covering topics such as case law relevant to forensic psychology, report writing for legal audiences, cultural considerations in forensic work, and other topics related to forensic assessment. Postdoctoral Psychologists report to the Supervising Forensic Psychologist and receive clinical supervision from the Supervising Forensic Psychologist and the Senior Director of Forensic Psychology in order for the candidate to become licensed within 18 months of employment. Of note, this is a postdoctoral position and therefore a full-time position after licensure is not guaranteed. The Postdoctoral Psychologist will work with stakeholders, including judges, defense attorneys, prosecutors, special jurisdiction courts, ATI programs, Correctional Health Services, and the Department of Correction. Responsibilities Role and Responsibilities: Working alongside our Licensed Practical Nurse (LPN), you’ll play a vital part in providing the highest level of nursing care to inmates/detainees while putting a special focus on maintaining our safe and secure workplace. • Develop nursing care plans with guidance from the RN., • Perform complete physical assessment and medical history., • Provide nursing care within your capacity and refer other medical problems to the appropriate physician., • Make sure prescribed medications are administered as instructed., • Periodically evaluate patients' symptoms, reactions, and progress, and report problems to management., • Securely administer medication, taking precautions against drug hoarding and trafficking., • Follow industry and company accreditation policies., • Document all detainee/inmate contacts and keep medical records confidential. Operates within legal constraints and contract agency stipulations. Primary Duties and Responsibilities • Ensures that the equipment necessary for each watch is operational including radios, flashlights, and keys prior to relieving the watch. Prepares shift documentation including shift reports, work orders, security reports, and unusual incidents that are legible, concise and that meet facility’s standards and contracting agency requirements., • Receives necessary information from previous shift personnel pertaining to events that transpired during the shift such as resident medical problems, disciplinary problems, or new arrivals and any special instructions. Relieves the watch and informs necessary personnel that the watch has been relieved. Makes entries into the security log showing the time the watch was relieved noting name and title., • Conducts radio checks documenting results in the security log. Conducts facility counts at the prescribed times and ensures the logbooks in the buildings are accurate as to count., • Frequently patrols building to observe and listen to individual residents and groups of residents and monitors the state of the building.
