Work Shifts Time: 9a-5p Days: Mon-Fri Facility: Various Locations on Rikers Island Duties & Responsibilities This full-time position reports either directly or through a Program Director to the Clinical Director of Psychiatry in the Division of Correctional Health Services and is responsible for the clinical oversight, management, and innovative development in specific mental health treatment areas, including standard mental observation (MO) units, specialty MO units and general population clinic settings in one of the 12 jail facilities in New York City. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Clinical Supervisors who are qualified for and interested in providing care on the PACE or CAPS units (specialty MO units) for individuals with serious mental illness will report to a Program Director for direct supervision. Responsibilities Include: • Formulate and implement procedures for operating, monitoring, and evaluating mental health programs and services in assigned area of responsibility, • Develop and lead staff training programs and exercises to maintain a culture of learning and clinical excellence, • Serve as the clinical leader for the assigned area of responsibility and provide weekly individual and in-person supervision for all assigned mental health clinical staff; review diagnostic, treatment, and level of care decisions and documentation; ensure adherence to policies and procedures; and conduct clinical case conferences, • Lead daily multi-disciplinary meetings with all mental health staff in the assigned area of responsibility, including DOC when appropriate., • Provide quality assurance and quality improvement as directed by the Clinical Director of Psychiatry, • Coordinate re-entry services with the social work staff in the assigned area of responsibility, • Meet regularly with the Clinical Director of Psychiatry or assigned supervisor to provide meaningful input into program design and policy formation, • Personnel management of the clinical staff, with assistance from the Clinical and/or Medical Directors as needed, including recruiting, mentorship, supervision, and disciplinary procedures, • 20% of time must be devoted to direct clinical care, some of which may include supervision of trainees. Timely and clear documentation of supervision and clinical care is expected. Minimum Qualifications 1. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and, 2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicare about 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards., • Maintain clinical competency by participating in all CME and CHS training and in-service requirements., • Maintain your schedule as directed with particular attention to punctuality and timely notification of absences., • Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed., • Complete tasks as delegated by a Site Medical Director or another supervising clinical team member., • Maintain and provide documentation of all required credentials., • Maintain professional attitude and appearance., • Adhere to Occupational Health Services requirements. Minimum Qualifications 1. Valid license and current registration to practice as a Physician Assistant issued by the New York State Education Department (NYSED); and, 2. Current certification issued by the National Commission on Certification of Physician Assistants (NCCPA); and, 3. One (1) year of full-time, satisfactory clinical experience as a Physician Assistant; and, 4. Possession of a valid Life Support certification (e.g., Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS)) is required in the appropriate practice area through the American Heart Association (AHA) within sixty (60) calendar days of appointment to the title. Summary of position The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Participants are either mandated or encouraged to voluntarily attend this class as part of their supervised release program. The class offers participants an overview of basic principles of power and control dynamics and uses an interactive format to introduce strategies for making healthy choices in intimate relationships. The Facilitator will provide this class as part of a larger IPV supervised release track, with the goal of increasing compliance with court mandates and return to court. The Facilitator will also train in other IPV and domestic violence groups and classes as the team seeks to expand programming. The facilitator will primarily teach online and but will also teach classes in person throughout New York City's five boroughs as the program expands. The class will be offered in several separate groups: Spanish-speaking, male, female, and LGBTQ+ defendants utilizing different curricula. Classes are offered in other languages with an interpreter on an as-needed basis. The position assumes full responsibility and authority in areas delegated. May direct a group of services across multiple jail facilities. Responsibilities include: (Detailed Task) • Serve as the clinical director for Complex Care, providing clinical supervision to clinicians (1 NP, 1 RN) who supporting care delivery and coordination for the more medically complex patients in the jails., • This role encompasses the following responsibilities:, • Establishing clinical criteria for inclusion., • Defining the overarching vision and program objectives., • Attending weekly Complex Care rounds to ensure the effective delivery of care., • Consulting on Complex Care patients and offering clinical guidance and support to front-line providers within jail facilities., • Providing direct patient care at least 50 percent of schedule, • Serving as a clinical point of contact for colleagues from other departments as needed., • Delivering patient education related to complex diagnoses., • Ensuring that patients are assigned to medically appropriate housing areas on Rikers Island., • Providing clinical input and support for medically appropriate discharge plans for patients being discharged to the community, including necessary medications and follow-up appointments., • Ensure safe transitions and coordination of care between the jail and external facilities, including the following tasks:, • Obtaining and reviewing external medical records for comprehensive patient assessment., • Reviewing the daily hospital census of Correctional Health Services (CHS) patients., • Collaborating with the Complex Care Nurse Practitioner to schedule appropriate specialty appointments, track these appointments, and communicate care plans to all relevant parties within CHS to ensure proper execution., • Engaging with hospitalists and inpatient specialty teams when CHS patients are hospitalized and as they transition back to the jail or community., • Arranging expedited admissions to the hospital and ensuring the success of planned admissions for Complex Care patients., • Collaborate with other CHS teams regarding shared patients, with particular emphasis on those involved in Mental Health Discharge Planning and Re-entry and Transition Services., • Participate in clinical release work and support legal advocacy in concert with the Clinical Court Advocacy (CCA) team by:, • Acting as a resource to resolve clinical inquiries from CCA staff (social workers) and providing input regarding patient eligibility for CCA services., • Interfacing with legal representatives to furnish relevant clinical information pertinent to a patient’s legal case., • Composing letters that detail medical conditions for legal documentation., • Assuming on-call responsibilities for critically ill patients, ensuring timely communication with hospital staff, CHS leadership, and legal representatives regarding urgent cases., • Engage in the planning, development, and implementation of programs, policies, procedures, and evaluative monitoring techniques designed to fulfill the objectives established by CHS and Health and Hospitals Corporation (HHC) leadership. Summary of position The Senior Psychiatrist is the psychiatric leader of the facility or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Responsibilities Include (depending on location): • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals., • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction., • Ensure that medication renewals are completed in a timely manner., • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting. (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards., • In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program., • Conduct regular and multi-disciplinary case conferences., • Report on an as-needed basis to the CHS Morbidity and Mortality review committee., • Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education., • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary., • Lead at least monthly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief and Clinical Supervisor., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints., • At least 30% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees. For smaller facilities or programs with less supervisory need, more direct patient care will be expected., • On-call responsibilities for two weeks at a time, one to times per year, for off-hours issues that require high level clinical intervention. Position Overview Under the supervision of the Site Medical Director, Nurse Practitioners will deliver comprehensive, compassionate, and thoughtful care to patients with complex chronic diseases within the New York City jail system. This role involves collaboration with physician leadership to provide general primary care, which includes conducting detailed histories and physical exams, diagnosing and managing both acute and chronic illnesses, and assessing the necessity for consultative services. In addition, the Nurse Practitioner will engage as part of an interdisciplinary team, focusing on patients with chronic medical conditions or physical disabilities who do not require acute inpatient admission. Job Description HHC Payroll Title: Director of Service Functional/Office Title: Supervising Psychiatrist Summary of position: This full time position reports to the Medical Director of Mental Health in the Division of Correctional Health Services and is primarily responsible for the clinical oversight, management, supervision and innovative development of the clinical staff in one or more of the jail facilities in New York City, or in a designated multi-site specialty program that affects specific mental health populations. In addition, this position also includes direct clinical care. There are 11 facilities of the New York City jail system spread out through multiple boroughs; 25% of new admissions per year will receive care on the psychiatric service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face this population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong, ambulatory clinics, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that a mission-driven, high quality, and innovative standard of psychiatric care is provided to patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. In addition to the Radiology Technologist job functions, the QA Technologists is responsible for the following: • Organize and file maintenance reports for each building in regards to dentistry and radiology x-ray equipment and ensure dentistry and x-ray equipment follow the Department of Health codes, policies and procedures., • Participates in quality assurance activities, including coordination, control and maintenance of technical equipment., • Establish a daily/weekly/monthly quality control routine for monitoring, evaluating, and maintenance of X-ray equipment to ensure optimal performance and stability., • Documenting and maintaining records for the quality control program in accordance with applicable regulations, legal requirements, accrediting agencies and recommendations from equipment manufacturers., • Performs research and participates in special projects involving evaluation of equipment and delivery of service., • Work with our radiologists to obtain updated documentation of primary diagnostic monitors., • Distribute/collect dosimeter badges from each employee/facility that works with x-ray equipment., • Recommends area assignment of personnel to coordinate activities within the department with other activities and services and represents the department in interdepartmental operations and meetings., • May be assigned duties and responsibilities of X-ray Director in regards to radiology department workflow. Minimum Qualifications Assignment Level III 1. For Supervisory Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and two (2) years of Radiologic Technologist experience performing duties consistent with Assignment Levels I and II, one (1) year of which must have been in a supervisory capacity., 2. For Specialized Imaging Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and possession of a current advanced- level certification for the performance of these procedures. Department Preferences • Possession of a current valid license as a General Radiological Technologist issued by the State of New York., • A minimum of two years of experience as a Radiological Technologist., • A current member of the American Registry of Radiologic Technologists (ARRT)., • Valid BLS required., • Must possess a valid driver’s license., • Must be proficient in Microsoft Office Suite. Additional Salary Compensation All Radiologic Technologists in Assignment Levels II and III shall receive a differential of $7,500 per annum to be cross-trained on different modalities of radiologic imaging and to be cross-assigned (once trained) to different imaging settings depending on patient volume and staffing needs within a facility.
