New York City Health and Hospitals Corporation Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. This is an incredible opportunity to be part of a passionate and motivated team providing care to some of the City’s most marginalized, vulnerable people. *To help support continuity of operations and care, staff selected to work in the OTxHU may also be required to work in CHS locations within the jails. Additionally, while CHS seeks the most qualified individuals for these positions, preference will be given to equally qualified, internal candidates. • Time: 8:00 AM - 4:30 PM, • Days: Monday to Friday, • Location: OTxHU-Bellevue/Rikers Island Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to patients with complex chronic disease in the New York City jail system, under the supervision of physician leadership. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services. The Physician Assistant will participate in an interdisciplinary team under supervision of a Site Medical Director for patients who have chronic medical conditions or physical disabilities who do not require acute inpatient admission. Responsibilities include: • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated., • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable., • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicare about 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards., • Maintain clinical competency by participating in all CME and CHS training and in-service requirements., • Maintain your schedule as directed with particular attention to punctuality and timely notification of absences., • Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed., • Complete tasks as delegated by a Site Medical Director or other supervising clinical team member., • Maintain and provide documentation of all required credentials., • Maintain professional attitude and appearance., • Adhere to Occupational Health Services requirements. Minimum Qualifications 1. Valid limited permit to practice as a Physician Assistant issued by the New York State Education Department (NYSED); or, 2. Valid license and current registration to practice as a Physician Assistant issued by the NYSED; and, 3. Under general supervision of the Medical Director, the Senior Psychiatrist is the psychiatric leader of the facility or program and works in close collaboration with other discipline leadership to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong clinical support and oversight, leadership and mentorship for existing staff. Responsibilities • Develops workflows, policies and quality metrics for their dedicated area in close coordination with the Medical Director and Chief of Mental Health, • Recruits, hires and trains staff to provide care in their dedicated area, • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with other leadership: design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction, • Ensure that medication renewals are completed in a timely manner, • Ensure that the standard of care is being met or exceeded in all patient care, • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards, • In collaboration with other leadership, oversee the orientation of new staff to the facility or program, • Conduct regular multi-disciplinary case conferences and/or other educational programming, • Report on an as-needed basis to the CHS Morbidity and Mortality review committee, • Work in collaboration with other mental health leadership to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education, • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary, • Lead interdisciplinary mental health staff meetings in coordination with local MH leadership., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints, • Administrative tasks, including coordinating scheduling and assignment of patient care appointments, scheduling of staff, and management of referrals to mental health service, • Direct clinical care, amount determined by facility needs, which may include direct supervision of psychiatric trainees., • On-call responsibilities for 2 weeks at a time, one to two times/year, for off-hours issues that require high level clinical intervention, • Other duties at the discretion of the Medical Director of Mental Health or the Chief of Service., • Personnel management of the clinical staff, with assistance from the Clinical and/or Medical Directors as needed, including recruiting, mentorship, supervision, and disciplinary procedures, • Meet regularly with assigned supervisor to provide meaningful input into program design and policy formation Minimum Qualifications Applicants must have all of the following: 1. Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. 1. Completion of an acceptable internship in an approved hospital., 2. A license to practice medicine in the State of New York., 3. Certification by American Board in field of specialization or a faculty appointment of Associate Professor or higher in a medical school recognized by the American Medical Association., 4. Eight (8) years of satisfactory full-time paid experience in field of specialization including at least three (3) years in a supervisory or administrative capacity.(Training and experience required for certification will be credited toward required specialized experience.) Duties & Responsibilities Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Unit Chief is the mental health administrative lead in each facility or designated area, including general population and therapeutic housing units. Under the direction of the Director of Mental Health Administration, this full-time position coordinates, and helps manage, mental healthcare delivery in one or several of the jail facilities in New York City. This includes ensuring provision of services within the timeframes and quality benchmarks outlines by CHS MH policies, providing administrative support to staff who are assigned to their clinical area, maintenance of patient records and compliance with monitoring requirements. S/he will act as the principle administrative liaison between mental health disciplines, as well as pharmacy, medical and nursing. As a member of the facility management team, the Unit Chief will collaborate closely with the Department of Correction (DOC) leadership and CHS Operations in order to ensure service delivery and meet patient needs. A major component of this position will be providing effective, inspiring, and clinically-informed administrative leadership and organization. Responsibilities Include: • Manage the daily operations of the program and site; ensure that patient and program goals are met in accordance with established H+H policies and procedures, • Ensure adequate multi-disciplinary mental health staffing in the facility, • Coordinate therapeutic services; oversee the referral, intake and assignment of cases, • Participate in relevant quality improvement initiatives; assure completion and accuracy of all data completion, report progress and plan of action in monthly QI meeting, • Provide orientation and training to all new hires, • Provide administrative and clinical support to mental health staff; proactively identify and resolve performance issues and manage personnel issues with discipline supervisors, • Support staff development; teach and ensure completion of departmental trainings designed to enhance the skills of staff relevant to their position, • 10% of time must be devoted to direct clinical care, some of which may include intake evaluation, suicide watch assessment, and clinical follow up’s as needed., • Organize and facilitate monthly mental health staff meetings with other MH discipline leadership, • Attend and actively participate in multiple interdisciplinary meetings per week, both within the mental health department and with other services, including DOC, • Supervise the collection and appropriate dissemination of statistical reports, staffing patterns, bed utilization, timesheets, payroll activities and HR paperwork., • Maintain on-call availability for urgent or emergent administrative issues that arise off-hours, • Other duties as assigned by the Director of Mental Health Administration or her/his designee Minimum Qualifications 1. The Quality Assurance (QA) Radiological Technologist position encompasses responsible, administrative and clinical activity of varying degrees of latitude. Clinical level judgement is needed in the daily supervision of radiological functions and patient care, and departmental projects. Responsibilities include: • Review exam orders, consult with radiologist, schedule patients and arrange for transportation as indicated., • Identify patient and document all information pertinent to final report in the RIS and on images (PACS)., • Prepare all patients (infants, children, adolescents, adults and geriatric) for radiological procedures requested by the clinician. Arrange for proper protection of patients, public and staff from infectious diseases/materials., • Perform all general diagnostic imaging and routine examination in CT and Angiography., • Assists in performing daily checks of radiographic imaging equipment, immobilization devices and other necessary accessories., • Enters data and maintains documents and logs as required and mandated by the department QA guidelines and various regulatory agencies., • Maintains adequate stocks of supplies and materials needed for use in the department., • Checks equipment for defects prior for use and assures proper use of such equipment through instruction and review., • Report any unexpected patient incidents in accordance with the occurrence report protocol., • Maintain and exercise all mandated radiation safety regulations. Maintain basic care of all the imaging equipment. All malfunctions are to be reported to management staff whether mechanical, electrical or suspect and service request documentation completed. Maintain a clean and safe work environment for both patients and staff., • Perform other duties as deemed necessary (by supervisory or administrative staff) in the event of emergencies or disasters., • Continue professional growth and development of job-related skills on an ongoing basis. Participate in continuing education activities., • Ensure all examinations are accessioned, documented and sent to PACS for reading. Monitor PACS and RIS incomplete worklists to ensure all exams are completed., • In accordance with HIPAA (the Health Insurance Portability and Accountability Act), staff members will exercise due diligence in ensuring patient privacy and confidentiality., • Identifies activities that the network has implemented to promote patient safety and takes all necessary precautions to ensure a safe environment for patients, visitors and staff. In addition to the Radiology Technologist job functions, the QA Technologists is responsible for the following: • Organize and file maintenance reports for each building in regards to dentistry and radiology x-ray equipment and ensure dentistry and x-ray equipment follow the Department of Health codes, policies and procedures., • Participates in quality assurance activities, including coordination, control and maintenance of technical equipment., • Establish a daily/weekly/monthly quality control routine for monitoring, evaluating, and maintenance of X-ray equipment to ensure optimal performance and stability., • Documenting and maintaining records for the quality control program in accordance with applicable regulations, legal requirements, accrediting agencies and recommendations from equipment manufacturers., • Performs research and participates in special projects involving evaluation of equipment and delivery of service., • Work with our radiologists to obtain updated documentation of primary diagnostic monitors., • Distribute/collect dosimeter badges from each employee/facility that works with x-ray equipment., • Recommends area assignment of personnel to coordinate activities within the department with other activities and services and represents the department in interdepartmental operations and meetings., • May be assigned duties and responsibilities of X-ray Director in regards to radiology department workflow. Minimum Qualifications: Assignment Level III 1. For Supervisory Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and two (2) years of Radiologic Technologist experience performing duties consistent with Assignment Levels I and II, one (1) year of which must have been in a supervisory capacity., 2. Position Overview Under the general direction of the Director of Safety & Environmental Health, or designee, the Deputy Director of Safety & Environmental Health is responsible for developing, implementing, and monitoring adherence to CHS policies and procedures to ensure a safe working environment. This role oversees occupational safety training and on-going education to staff while working collaboratively with Human Resources, Facilities, Clinical Leadership, and external regulatory bodies to mitigate safety risk and reduce potential environmental hazards. Key Responsibilities • Implement and maintain safety standards and protocols that comply with local, state, and federal regulations. This includes conducting routine safety inspections and risk assessments to identify potential hazards., • Conduct in-service education to healthcare personnel on safety procedures, including but not limited to situational awareness, workplace violence prevention, and emergency response actions., • Represent CHS as needed in internal or external committees and workgroups where Safety representation or perspective is required., • Compile workplace violence and staff injury reports for distribution to required entities., • Assist in maintaining workplace violence, occupational injury, and other safety-related databases and logs., • Analyzes data to identify trends and areas for improvement in safety practices., • Respond on-site to serious incidents of workplace violence, occupational injury, or environmental hazards, to gather factual information and develop a detailed report of findings., • Assist in conducting post-incident well-being checks for employees who were involved in a workplace violence incident or who may have sustained a severe work-related injury., • Conduct and prepare written summaries of safety rounds performed at Rikers Island, community warehouses, outposted sites, and court-based facilities., • Aid in establishing methods to improve safe clinic operations while reducing the risks associated with workplace violence incidents, environmental hazards, and overall staff safety., • Performs regularly scheduled and unannounced walkthrough of clinic facilities and other work areas to identify, document and address potential workplace concerns in a timely manner., • Assist in facilitating monthly and quarterly workplace violence and safety meetings with key stakeholders., • Other relevant assignments as required. A Master’s degree from an accredited college or university in Public Administration, Public Health, Business Administration, or a related discipline, and one (1) year of full-time experience in an administrative capacity in business management systems, general administration, health care or medical/patient systems administration; or,, 2. Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. This full-time position reports either directly or through a Program Director to the Clinical Director of Psychiatry in the Division of Correctional Health Services and is responsible for the clinical oversight, management, and innovative development in specific mental health treatment areas, including standard mental observation (MO) units, specialty MO units and general population clinic settings in one of the 12 jail facilities in New York City. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Clinical Supervisors who are qualified for and interested in providing care on the PACE or CAPS units (specialty MO units) for individuals with serious mental illness will report to a Program Director for direct supervision. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. Conduct routine safety rounds in all facilities where CHS employees are located while identifying potential environmental safety hazards to the Department of Corrections (DOC) for mitigation. The Occupational Safety & Environmental Health Coordinator will engage with staff to identify safety gaps or environmental health concerns, provide on-going support and follow-up for staff involved in workplace violence incidents and/or who have suffered occupational injuries. Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. JAIL POPULATION REVIEW PROGRAM The Jail Population Review Program is an ambitious initiative in New York City that is led by the Center’s New York Legal Policy (NYLP) Team–court practitioners, clinical staff, and research and policy experts focused on developing and implementing critical operational and policy reforms and improving coordination between justice system stakeholders. The Jail Population Review Program reviews incarcerated individuals for opportunities for pretrial release or case resolution. The team provides critical information to prosecutors, defense attorneys and judges at the earliest possible juncture so they can engage in meaningful dialogue before an individual spends weeks or months in jail. THE OPPORTUNITY This postdoctoral fellowship in forensic psychology is a collaboration between the Center and Columbia University that focuses primarily on forensic assessment with an opportunity to provide psychotherapy to individuals re-entering the community from incarceration. Postdoctoral fellows conduct forensic assessments mainly for the Jail Population Review Program. The Jail Population Review Program provides a number of clinical consultation services, including independent clinical assessment, review of medical records, and psychological testing when indicated. The postdoctoral fellowship is a one to two-year, full-time program providing advanced training at the intersection of psychology and law. Fellows gain supervised experience conducting psychological evaluations in the criminal court context, primarily brief and comprehensive clinical assessments of incarcerated people, as well as violence risk assessments (general violence, intimate partner violence, and sexual violence) using the Structured Professional Judgement (SPJ) approach, criminogenic risk and needs assessments, and mitigation psychological evaluations. Fellows also conduct psychological evaluations and testing to assist the courts in developing treatment plans for those involved in court-mandated treatment, such as mental health courts or other alternative-to-incarceration programs. In collaboration with Columbia University, fellows may conduct psychotherapy with one participant of the Columbia Reentry Initiative under the supervision of Columbia’s faculty or audit a graduate level forensic psychology course at Columbia University (Forensic Assessment or Correctional Psychology). The Columbia Reentry Initiative at the Dean Hope Center for Educational and Psychological Services at Teachers College is a no-cost, evidence-based and trauma-informed psychological treatment program designed to support adults reentering to the community following incarceration. Postdoctoral Psychologists attend weekly multidisciplinary case discussions and monthly didactic seminars covering topics such as case law relevant to forensic psychology, report writing for legal audiences, cultural considerations in forensic work, and other topics related to forensic assessment. Postdoctoral Psychologists report to the Supervising Forensic Psychologist and receive clinical supervision from the Supervising Forensic Psychologist and the Senior Director of Forensic Psychology in order for the candidate to become licensed within 18 months of employment. Of note, this is a postdoctoral position and therefore a full-time position after licensure is not guaranteed. The Postdoctoral Psychologist will work with stakeholders, including judges, defense attorneys, prosecutors, special jurisdiction courts, ATI programs, Correctional Health Services, and the Department of Correction. SIJC operates diversion, alternative to detention and incarceration, supervised release, reentry, mental health and wellness, workforce development, peer-led restorative justice and leadership development programs. The Justice Center’s youth justice initiatives provide judges, prosecutors, and defense attorneys with alternative community sentencing options that hold individuals accountable in a way that is meaningful, proportionate, and restorative. The goal is to provide participants with individual and group services that help them to avoid future contact with the justice system and promote swift case resolutions that bolster public confidence in justice. SIJC is seeking a Youth Engagement Specialist who will support as a Mentor in working with youth justice initiatives participants ages 13 to 18. Reporting to the Youth Justice Initiatives Program Manager, the Youth Engagement Specialist will develop and oversee mentorship programming activities—including group workshops and individual mentorship sessions. These activities will take place virtually, on-site, and in the community. The Youth Engagement Specialist will work individually and with a team to assist participants in addressing the individual and societal factors that may impact their behavior. Job Description Director of Veterans Services The Director of Veterans Services is responsible for the leadership, development, implementation, and oversight of all veteran-focused programs and services at NJRC. The Director will ensure that all court-involved veterans receive comprehensive support services designed to promote stability, wellness, employment, education, housing access, behavioral health support, and successful community reintegration. This position requires strong leadership, program management, partnership development, and knowledge of issues affecting veterans. The Director will work collaboratively with internal departments, veteran groups, Veterans Administration, community organizations, courts, correctional institutions, government agencies, and service providers to expand and improve services for veterans. Essential Duties and Responsibilities Develop, manage, and oversee all veteran services programs and initiatives for veterans. Ensure programs comply with all federal, state, local, and grant requirements. Build and maintain partnerships with: Veterans Administration agencies, NJ State agencies servicing veterans, courts, probation and parole departments, correctional facilities, veteran-serving organizations, workforce development agencies, healthcare providers, and housing organizations. Supervise veterans services staff and program coordinators. Monitor program performance, outcomes, and reporting requirements.
