Conduct routine safety rounds in all facilities where CHS employees are located while identifying potential environmental safety hazards to the Department of Corrections (DOC) for mitigation. The Occupational Safety & Environmental Health Coordinator will engage with staff to identify safety gaps or environmental health concerns, provide on-going support and follow-up for staff involved in workplace violence incidents and/or who have suffered occupational injuries. This position serves as a central resource for safety education, incident response, and environmental risk mitigation across all CHS sites and services. Using the problem-oriented progress notes, written to match each problem documented on the Problem List and organized using the SOAP format., • Coordinates the nursing care provided for patients within a given unit by:, • Assigning appropriate nursing staff for patient care activities such as admissions, sick call, treatments, medications, punitive segregation rounds, follow-up, etc. Problems in staffing must be communicated to the On-Site Director of Nursing., • Utilize therapeutic communication skills and effective interviewing techniques to elicit pertinent historical information and to make appropriate referrals as required. Screen patients with mental health problems in an immediate manner and makes appropriate referral as required., • Making rounds to patient care areas for the purposes of assessing, planning, implementing, documenting, and evaluating unit nursing care delivery. Assist OS-DON in staffing needs. Provides guidance and assistance to nursing personnel in the delivery of patient care., • Assuring continuity of care on other shifts through written and/or verbal communication. Communicates special care needs of particular patients to the psychiatrist, On-Site Director of Nursing or the On-Island Administrator., • Collaborates with the OS-DON in the orientation of new staff on the unit. The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicareabout 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. Duties & Responsibilities Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Unit Chief is the mental health administrative lead in each facility or designated area, including general population and therapeutic housing units. Under the direction of the Director of Mental Health Administration, this full-time position coordinates, and helps manage, mental healthcare delivery in one or several of the jail facilities in New York City. This includes ensuring provision of services within the timeframes and quality benchmarks outlines by CHS MH policies, providing administrative support to staff who are assigned to their clinical area, maintenance of patient records and compliance with monitoring requirements. S/he will act as the principle administrative liaison between mental health disciplines, as well as pharmacy, medical and nursing. Title: Housing Retention Specialist Location: Bronx, New York Reports to: Assistant Director/Housing Retention Coordinator Pay Range: $27.00 - $27.47 per hour FLSA Classification: Non-exempt Status: Part-time (< 35 hours) Role Summary: The Housing Retention Specialist will execute supportive services within NAICA’s "light touch" supportive housing program. Their interventions aim to facilitate long-term housing stability, assisting formerly homeless families in averting eviction and avoiding a return to shelter. Managing a caseload of up to 20 households comprising formerly homeless individuals and families, the Specialist will conduct bi-weekly home visits post-move-in. As households acclimate to their new residences, service levels will be tailored accordingly, ensuring appropriateness for each household. Emphasizing prevention, efforts will focus on sustaining benefits/subsidies, preventing rent arrears, and facilitating referrals to community-based services for seamless community integration. Additionally, the Housing Retention Specialist serves as the conduit between social services and property management staff. Summary Job Description: Performs professional-level casework duties in providing services for children, biological, and foster parents or planning resources under the care of the agency; performs related work as required in providing permanency planning to expedite the safe discharge of children from the foster care system. Principle Responsibilities: • Investigates cases referred to the agency to determine need and proper method of care; interviews the individual, the family, community and governmental agency personnel, and any other resource person who might provide information necessary for a proper psycho-social diagnostic evaluation and the preparation of a treatment plan; works with other divisions of the agency to evaluate the progress of cases and to carry out the treatment plan., • Works with public welfare agencies; the courts, and others to assist individuals with financial, legal, medical, and personal problems., • Attends periodic staff conferences; confers with supervisors weekly and as needed; prepares dictation, reports, and correspondence on assigned cases and maintains records of the status of each case through established channels., • Expected to respond to case emergencies after hours or on weekends., • All progress notes and FASP will be recorded online in Connections by the caseworker., • Every action or event must be recorded by the caseworker no later than 72 hours after the action or event and must be forwarded to the immediate supervisor for review. The supervisor must review the corrected progress note no later than 3 days after it is received., • When a caseworker is going on vacation, progress notes for all events must be submitted to supervisor by 5:00 PM, 2 weekdays before vacation begins., • The Director will monitor the timeliness of progress notes and the review process., • Caseworkers, Supervisors, Directors, and Secretaries will attend all mandated trainings regarding case recording., • Supervisors will schedule weekly conferences with caseworkers to specifically discuss each case and to review caseworkers’ interventions Qualifications: • Good knowledge of the principles and practices of social casework., • Good knowledge of the laws, rules, and regulations governing the care and treatment of children., • Experience related to services to families and children, preferably medical or child welfare related, • Excellent written and verbal communication skills in order to effectively provide direction and counseling to clients and maintain all required documentation., • Ability to establish effective working relationships with clients, relatives, and other individuals and to obtain and verify information needed through interviews and investigation., • Ability to prepare clear, accurate, and comprehensive case records and to reach logical conclusions based on pertinent regulations and information.
