Beach 21st is a building which was developed and is operated by Community Builders. Beach 21st is a residential building located in the Far Rockaway neighborhood of Queens. The project consists of 23 residential units, 5 singles and 18 families with CAMBA, Inc. providing the on-site services Position: Case Manager Reports To: Program Manager Location: 1047 Beach 21st Street, Far Rockaway, NY 11691 What The Case Manager Does: • Maintain professional relationships with clients and client confidentiality., • Practice Universal Precautions/Standard Protocol & Procedures., • Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information., • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building)., • Act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • Schedule appointments for tenant with referral organizations., • Escort tenants to appointments (educational, medical, social service, etc.), • Assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf., • Follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • Evaluate actual living conditions of tenants through home visits., • May prepare marketing materials for the program., • May prescreen tenants over the telephone for eligibility and may schedule intake appointments., • Bachelor’s degree (e.g., B.A., B.S.W.), • Must be fingerprinted and obtain “Criminal History Check “(CHC) with Justice Center for the Protection of Vulnerable People with Special Needs Ability to maintain fingerprint clearance throughout the duration o employment., • As a Mandated Reporter, you must immediately report suspected incidents/cases abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at 1-855-373-2122 and complete a CAMBA Incident Report, • Flexible hours may be needed occasionally to provide shift coverage until 8 pm. Compensation: 40,000-46,250 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. Job Description Title: Operations Manager Location: New York City metropolitan area Pay Range: $32.41 - $32.96 per hour (est. $59,000 - $60,000 per annum) FLSA Classification: Full-time (35 hours per week), Non-Exempt Role Summary The Operations Manager oversees the daily operations of their Transitional Housing Program, ensuring a safe, supportive, and well-managed environment for clients and staff. This role is responsible for supervising facility operations, coordinating program services, and ensuring compliance with operational standards imposed by the organization and/or authorized government bodies. Primary Job Responsibilities/Duties The primary job duties and responsibilities of the Operations Manager shall include, but not necessarily be limited to the following: • Manage day-to-day program operations, facilities management, and client support systems., • Supervise the Program’s Residential Aides, Shift Supervisors, and other Operations staff (if any) to ensure their timely and appropriate service delivery to clients., • Provide other Operations staff with ongoing coaching, training, and performance evaluations to promote professional growth and accountability., • Provide employees and clients with guidance concerning relevant policies and procedures, including but not limited to conflict resolution and de-escalation techniques, to promote safety and compliance., • Participate in the development of operational goals, objectives, and procedures for the Program, making recommendations for changes to existing standards and procedures., • Serve as the Program’s main advocate and daily representative in communications with third parties concerned with or hired to provide maintenance, security, and/or cleaning or housekeeping services., • Work closely with the Program’s Director of Operations/Security to ensure compliance with applicable health, fire safety, and emergency preparedness standards, as well as any contractual obligations related to the Program., • Work closely with the Program’s Director of Operations/Security to ensure compliance with local, state, and federal rules and regulations, including those promulgated by the City of New York’s Department of Homeless Services (“DHS”) and other authorized regulators and government bodies., • Conduct regular audits of program documentation, case notes, and operational reports to maintain program integrity., • Respond to and manage crises, incidents, or emergencies in coordination with other NAICA leadership and staff., • Establishing and implementing team schedules and work tasks for the team., • Prepare and submit operational reports, incident logs, occupancy reports, and other documentation as required., • Foster a positive and collaborative work culture that prioritizes client well-being and staff safety., • Maintain ongoing communication with all Operations department supervisors/directors concerning any issues., • Report directly to the Program’s Director of Operations/Security, and indirectly to its Site Director, about all activities, needs/concerns and incidents within the facility., • Support the implementation and monitoring of Standard Operating Procedures (SOPs) across the program. Physical Requirements • Ability to sit at a desk, work on a computer, and perform repetitive tasks for prolonged periods of time., • Ability to regularly engage in physical activity including bending over, lifting, reaching, grasping, standing, and walking., • Must be able to grasp, pull, lift and carry up to 25 pounds., • Must be able to travel to multiple NYC sites as needed. Work Environment / Schedule Requirements • Office setting with regular exposure to computer screens and moderate noise levels., • May be required to work evenings or nights, weekends (Saturdays and/or Sundays), and holidays when Program needs require., • This position follows a regular weekly schedule. However, the Program operates twenty-four (24) hours per day and three hundred sixty-five (365) days per year, so variable or additional hours may occasionally be necessary to effectively fulfill the responsibilities of the position, based on organizational needs, deadlines, or events., • Staff must continue working past their shift unless relieved by another staff member or authorized to leave by a supervisor. Job Description JOB DESCRIPTION JOB TITLE: Safety Security Officer FLSA: Non-Exempt DIVISION: Operations REPORTS TO: Shift Supervisor LOCATION: Queens, NY SALARY: $19.65 hourly SUMMARY: The Safety Security Officer is responsible for ensuring the safety and security of all clients, staff, and the property around the facility in a homeless shelter. Operations within our facility are 24 hours. This is a relief position that requires face-to-face relief with mandatory overtime. This position reports to the Shift Supervisor and Security Manager of the Facility. DUTIES & RESPONSIBILITIES: • Perform daily inspections of all clients’ rooms, • Escort clients to appointments as needed, • Interact with and monitor clients' behavior, maintaining good rapport by providing excellent customer service. Encourage clients to comply with the facility's policies and procedures., • Control access at the main entrance by screening all clients and visitors, utilizing magnetometer/x-ray machine to detect and prevent weapons from entering the site., • Perform Crisis Intervention to de-escalate potential crises and document all such situations and interventions., • Make hourly rounds throughout the entire facility, including all hallways, stairwells, floors, bathrooms, laundry rooms, kitchen, and office spaces., • Respond to incidents, • Assist with client intakes, • Perform daily pack-ups and logging of client property belonging to AWOL clients, • Store and remove client property once the mandated storage time has expired, • Maintain a clean and organized storage area, • Complete logs, incident reports, and all other required documentation., • Make calls to police, fire department, and other first responders as directed., • Perform administrative and receptionist duties at the operations desk; monitor video surveillance cameras, and the fire prevention/detection alarm notification system., • Provide duties related to fire incidents, evacuation of the building when necessary, utilizing Fire procedures., • Ensure the safety and security of all assigned equipment., • Mandatory overtime Required • Perform other tasks as assigned.
