Job Description Apicha Community Health Center aims to improve community health by providing access to comprehensive primary care, preventive health services, mental health, and supportive services. The center is dedicated to delivering high-quality, culturally competent services that enhance the quality of life. Position Summary Provides professional nursing care for Apicha Community Health Center patients, adhering to established clinical standards and practices. The purpose of this position is to provide nursing care to patients of all ages, gender, cultures, and background; provide physical and psycho-social support to patients and their significant others; welcome and create an inclusive environment for LGBTQ patients; support and guide members of the clinical team. As a member of the nursing team, work collaboratively with medical providers and all other members of the multidisciplinary team in managing care for a population of patients. The major duties of the position include, but are not limited to: Direct Patient Care • Conducts an assessment of patients’ physical and psychosocial needs including pain assessment, allergies and objective data., • Maintains reports of patients’ medical histories, adheres with protocols, rules and regulations to maintain complete medical records., • Administers prescribed medications, vaccines, and treatments, and reports all critical lab values immediately to the providers., • Assess and implement emergency treatment when needed., • Triage urgent patient care requests (phone, walk-in, portal communication), as well as making appointments when appropriate. Utilize critical thinking, and sound clinical judgment to appropriately determine the next level of care., • Promotes wellness and healthy behaviors through teaching and education of patients and caregivers., • Implement procedures to improve the quality of service and maintain patient flow efficiency., • Adhere to patient confidentiality and Apicha corporate compliance policy. Collaboration • Works collaboratively with nursing team, addresses issues and concerns with supervisor with a positive problem-solving approach., • Evaluates the effectiveness of the care given by interdisciplinary care team and communicates findings to the team., • Assist with population management by identifying patients lacking preventive health screenings; recommended periodic testing; follow-up appointments (lost to follow-up); treatment or follow-up for actionable test results., • Collaborates with Clinical Nurse Manger to provide seminars and in-house trainings on addressing patient health care concerns, infection prevention and teaching family caregiver’s strategies on how to manage their loved ones’ conditions. Compliance • Adhere to patient confidentiality/HIPAA regulations and Apicha corporate compliance policy., • Maintain reports of patients’ medical histories, monitors and reports changes in their condition and adheres with protocols, rules and regulations to maintain complete medical records., • Act in a professional manner, reflecting the mission of Apicha Community Health Center demonstrating compliance with all policy, procedures and federal/state regulations as outlined in the Employee Handbook. Patient Education • Provide patient education, and information to patients and caregivers, focusing on self-care appropriate to their age and identified learning considerations and needs. Exam Room Oversight • Monitor medications, vaccines and medical supplies and report inventory to Nurse Manager., • Anticipate and oversee preparation of examination rooms with equipment and supplies required for visits. Assists providers with examination and procedures as requested., • Maintains hygienic and safe working environment in compliance with healthcare standards and procedures and adheres to Infection Control efforts. Self-Development • Participate in ongoing trainings in cultural competency with special consideration including but not limited to underserved populations and the LGBTQ people across the age continuum., • Maintain nursing competence through participation in continuous professional education and the appropriate learning experience. Qualifications: • Bachelor's degree or Associate of Applied Science in Nursing is required., • 2 year's experience with medical surgical unit and/or pediatrics or adult triage preferred., • Must have a current Registered Nurse License in New York State., • Must have the ability to effectively communicate verbally and in writing., • Computer literate, documentation experience with EMR, preferably eClinicalWorks but not required., • Bilingual (preferably Spanish speaking or South Asian or East Asian or South-east Asian)., • BLS certified preferred. The substitute will fill in for head teacher absences to provide coverage-based instruction and social-emotional support to students. Our classrooms typically consist of eight or fewer students. Teachers have the opportunity to receive mentorship, coaching, and support from the clinicians as well as academic administrators. Supports provided may be offered in a residential, day, home, or other setting. MSPs assist and support individuals to identify and achieve their Personal Outcome Measures TM (POMs). MSPs motivate individuals to develop their vocational, nutritional, social, and personal skills to achieve the highest possible level of independence. All supports are offered in positive settings, that are the least restrictive possible. DEMONSTRATIVE DUTIES Medical Support Professionals provide observations, evaluations, and reports to other team members to ensure and/or record an individual’s progress toward their personal goals, in accordance with their individual support plans. In doing this, they guide, assist, and teach individuals to advance their skills to foster an independence based on their needs, preferences, and desires. PROGRAM PLANNING • Provides information, documentation, shares observations and offers insights into the development of individual support, plans and implementation of chosen POM’s. • Documents all services and support provided to individuals supported based on them Individualized program plan(s) and enter information into computerized system. • Tracks and documents individual’s progress on goals. • Assists the individuals in planning and preparing meals while following dietary menus, eating protocols and serving all meals in a family style manner. • Reviews and initials communication log(s) upon arriving at assigned work site, documents all activities and information in the daily log. • Administers and documents Medication Administration per AMAP training and doctor’s orders; teachers, observes, reports problems and progress in self-administration of medication programs; assist individuals in achieving more independence in administration of medications. • Notifies program nurse and manager/supervisor of any changes in medication, individual’s condition, or any other health related concerns. • Follows all OPWDD and Organization’s Policies and Procedures regarding reporting injuries, incidents, or other related events. • Identifies and responds to unexpected, unplanned, and emergency situations as they occur. notifies appropriate management and follows the Agencies Policies and Procedures.
