The In-Home Family Therapist will provide Trauma Systems Therapy (TST) Model of treatment to children, youth, and families who are involved in, or at risk of involvement in, the child welfare system, legal system, and/or hospital system. Education, Knowledge, Skills & Experience Required: • Master’s degree in counseling, social work, or related field., • LCSW, LPC, LMFT or comparable license preferred. License eligible required., • Two or more year’s related experience and/or training required., • Ability to conduct a structured intake interview and make appropriate diagnoses., • Experience providing behavioral health treatment to traumatized populations, including proficient use of Motivational Interviewing., • Provide Trauma Systems Therapy (TST) Model of treatment to children, youth, and families who are involved in, or at risk of involvement in, the child welfare system, legal system, and/or hospital system. Closely coordinate care with other team members, including TST Skills Coach and Care Manager partners., • Work with consumers will regularly take place in the community or in the consumer’s home. Non-clinical duties may be conducted across a hybrid work setting. Must be able to provide evening sessions at least three evenings per week (last session offered is 7pm)., • Understand how trauma impacts the lives of the people being served, so that every interaction is consistent with the recovery process and reduces the possibility of re-traumatization., • Demonstrate ability to work with families with diverse backgrounds, including awareness of cultural differences., • Learn and creatively implement the Trauma Systems Therapy (TST) treatment model with all clients, families, and TST Teammates., • Outreach, screen, and effectively plan for sessions to engage client to meet agreed upon therapeutic goals and objectives., • Collaboratively create goals and objectives with client, family, and TST Teammates related to addressing trauma histories., • Exhibit effective assessment and diagnostic skills., • Identify risk factors including lethality, and effectively safety plan with client, family, and TST Teammates., • Work to increase and maintain client self-sufficiency by improving client’s adaptive level of functioning and use of community supports and natural supports., • Exhibit strong communication and collaboration with a multidisciplinary team to address barriers to treatment and advocate for client needs. Communicate daily, or as needed, with TST Teammates partnered on shared cases., • Coordinate care with internal and external service providers. Provide regular updates to other professionals involved with the case., • Provide psycho-ed about the trauma system to improve stakeholders’ understanding and support of client and family behavioral health needs., • As schedule allows, attend client court hearings as well as social services, school, probation, medication, and other community-based meetings. Must have strong understanding of professional boundaries in these settings., • Refer and help connect clients to needed resources (housing, medication, transportation, education, state and federal benefits, etc.), • Attend team meetings, supervisions, and mandatory in-services as scheduled., • Participate in planned individual and weekly group supervision by coming prepared with an agenda. Report high risk/problem cases and utilize a problem-solving approach., • Participate in trainings and staff development activities to enhance professional growth and follow individualized training plan., • Complete all TST Model paperwork required for fidelity., • Maintain a shared client file with TST Teammates to document, conceptualize, strategize, and prioritize planned interventions., • Efficiently navigate Electronic Health Record System., • Maintain timely and appropriate treatment documentation., • Effectively document medical necessity, treatment progress, safety risks, and other required information., • Comply with all corporate compliance and HIPAA standards, including submitting paperwork on time and meeting Jefferson Center’s productivity standards. Salary Grade 51 - Master’s Unlicensed Clinician $57,000 - $64,000* Salary Grade 52 - (Licensed Clinician LCSW, LMFT, LPC) $70,000 – $77,200* Additional Salary Information*: • The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week.
