Job Description POSITION SUMMARY Provides advanced medical care to patients enrolled in the End-Stage Renal (ESRD) Program. Utilizes clinical expertise to manage acute and chronic conditions related to renal disease, with a focus on dialysis care, care coordination, and improving patient outcomes. JOB RELATIONSHIPS REPORTS TO: Medical Director SUPERVISES: Provides clinical direction to RNs, LVNs, CMAs/CNAs DUTIES/RESPONSIBILITIES 1. Provides comprehensive medical services including assessment, diagnosis, and treatment of acute and chronic conditions related to renal disease, 2. Manages patients with End-Stage Renal Disease, including those receiving dialysis, in collaboration with the interdisciplinary care team, 3. Performs history and physical examinations and develops individualized treatment plans, 4. Orders, interprets, and evaluates diagnostic tests, including labs and imaging, 5. Prescribes medications and treatments in accordance with licensure, protocols, and regulatory guidelines, 6. Coordinates care with nephrologists, dialysis centers, specialists, and other healthcare providers, 7. Provides patient education on disease management, dialysis treatment, medication adherence, diet, and preventative care, 8. Monitors patient progress and adjusts treatment plans as needed to ensure optimal outcomes, 9. Performs minor procedures within scope of practice, including wound care and other clinical interventions, 10. Initiates and tracks specialty referrals, consultations, and follow-up care, 11. Ensures continuity of care across outpatient, inpatient, and dialysis settings, 12. Documents patient care accurately in the electronic medical record (EMR), 13. Participates in quality improvement initiatives and adherence to ESRD program standards, 14. Assists in maintaining compliance with applicable healthcare regulations and clinical guidelines, 15. Collaborates with administrative and clinical staff to ensure appropriate resources and supplies are available, 16. Provides input to improve clinical workflows and patient care processes, 17. Performs other related duties as assigned. PREFERRED EDUCATION/EXPERIENCE/LICENSURE REQUIREMENTS EDUCATION: Graduate of an accredited college/university program EXPERIENCE: Minimum two (2) years of clinical experience; experience in nephrology, dialysis, or chronic disease management preferred.
