Job Description COMMUNITY HEALTH NAVIGATOR - HRSN PROVIDER Position Title: Community Health Navigator – HRSN Provider Program: Access360 Type: Full-time / non-exempt Pay Range: $22.06 - $27.05 Location: 40 N. If you would like to learn more about us, please visit our website at: Position Overview: The Community Health Navigator – HRSN Provider works closely with the Social Care Network (SCN) Lead, Healthy Alliance, and is part of a collaborative, community-based initiative aimed to provide support and connection to services to address unmet Health Related Social Needs (HRSN’s) for individuals and families residing in the Capital District Region of New York State. The Navigator will act as a resource for individuals and their families by being an agent for health-related social needs screening and assessment and a vital linkage to community-based services in order to improve individual and community level health and wellness outcomes. The Navigator will also provide Enhanced Care Management for qualifying individuals. Essential Position Requirements, Specifications & Duties: • Ability to work: Monday-Friday, 9:00am – 5:00pm w/ flexibility as needed to accommodate client availability., • MSW or Master’s Degree in human service field preferred; BSW or Bachelor’s Degree in human service field AND at least two years’ experience in the human services or mental health field, required., • A Clean and Valid New York State Driver’s License (actively licensed at least one year) is required., • Staff in driving positions must be able to meet agency’s minimum requirements of our insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire., • Ability to work with clients with mental health issues, substance abuse and developmental disabilities., • Proficient computer skills: Microsoft Office Suite & Google Business Workspace Suite, and use secure Internet practices., • Ability to effectively navigate electronic health records, adding documentation with high accuracy., • Previous experience working in a human services agency is a plus!, • Capable of exercising ethical, independent judgment and discretion while handling sensitive data with strict confidentiality., • This position has regular and substantial unsupervised or unrestricted contact with clients, and is required by law to be fingerprinted for a criminal history record check under NYS Department of Health., • Ability to pass background checks, including fingerprinting., • Accept referrals from Healthy Alliance and/or other local partners and conduct outreach with clients within 2 business days., • Conduct AHC Health Related Social Needs Screening for referred clients and members of their households to assess for unmet medical and social needs, and document screening outcomes in the designated Electronic Health Record (EHR)., • Meet with clients in community settings., • Navigate clients and their household members to needed medical, mental health and social supports within the community., • Competitive Pay with an Excellent Benefits Package, • Health Insurance options*: Medical, Dental and Vision, • $600 Well-being Reimbursement Benefit*, • Generous Combined Leave Time (CLT) and Paid Holidays*!, • $500 Employee Referral Bonus*, • We are a 501(c) (3) agency, which is a qualifying employer for eligible staff under the Public Service Loan Forgiveness Program*, • Pension Plan with Generous Agency Contribution*, • 403b Retirement Savings Plan*, • Life Insurance* – Automatic Benefit at no cost to employee, • Paid Training, including TCI and CPR/AED/First Aid Certification & Update courses, if role appropriate, • Tuition Reimbursement* & Travel/ Mileage Reimbursement*, • Professional Development & Career Growth Opportunities, • The Comfort of a Business Casual Environment EEO Statement: St. Catherine’s Center for Children is committed to creating a diverse environment and are proud to be an equal opportunity employer.
