Job Description Callen-Lorde Community Health Center is dedicated to providing high-quality, sensitive, and non-judgmental healthcare to LGBTQ communities and people living with HIV. We are seeking a dedicated and compassionate Medical Provider to join our team as a Primary Care Provider (PCP). As a PCP at Callen-Lorde, you will play a vital role in delivering comprehensive care, making a positive impact on the lives of our diverse patient population. To learn more about our mission and vision, visit As a Primary Care Provider, your responsibilities will include: • Screening, diagnosing, and treating Callen-Lorde patients., • Providing care in a sensitive and non-judgmental manner., • Making appropriate internal and external referrals., • Offering comprehensive care based on pre-arranged schedules (20 min appts f/u and 40 min new pt appts), • Documenting care promptly and following charting protocols (EHR: EPIC), • Ensuring appropriate follow-up for ordered care, including referrals and test results., • Providing care to patients living with HIV. Medical providers new to HIV and other specialized care will participate in didactics, precepting, and case conferencing in order to build competency., • Contributing to continuous quality improvement efforts., • Maintaining familiarity with clinical guidelines in various areas of care.
