Job Summary: The Case Manager is responsible for supporting the Program Social Workers in the provision and coordination of casework services for youth of the Runaway Homeless Youth program Job Responsibilities: • Provide casework services to residents of the shelter including referrals for entitlements, housing, and services, accompanying residents on appointments and fostering/facilitating relationships with other services providers to meet the needs of the residents of the program., • Maintain up-to-date case record documentation and reports including progress notes court reports, and all documentation required by city and state governing bodies., • Enter case information into DYCD’s PTS and other demographic Systems. Provide timely communication to DYCD, • Developing, updating, following up on service/treatment plans. Job Description PRIMARY FUNCTION/PURPOSE: Provide direct case management services including assessment, counseling, crisis intervention, referral, and advocacy to ensure an effective social service support plan is in place. This role is responsible for working at our Union Hall location in Jamaica, Union Hall Street to enable the successful relocation of families to permanent housing. MINIMUM QUALIFICATIONS: • Bachelor’s degree is preferred, and at least an Associate’s degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, or Human Services is required, • At least one (l) year of experience in social services, counseling, crisis intervention, family services, preventive services, or housing services is required, • Experience facilitating groups for homeless adults, specialized knowledge of substance abuse issues and treatment, and/or family dynamics treatment experience is required, • Bilingual language ability is preferred, • Excellent written and verbal communication skills ESSENTIAL DUTIES AND RESPONSIBILITIES: • Utilizing the Department of Homeless Services (DHS) Client Assistance and Re-housing Enterprise System (CARES), complete family’s intake assessments, service plans/independent living plans, housing plans, next step conferences/transfers, incident reports, progress notes, etc., • Utilizing the DHS Client Assistance and Re-housing Enterprise System (CARES), review accessible information from the Human Resource Administration, Administration for Children Services, Department of Probation, Department of Homeless Services, etc., • Utilizing the DHS Client Assistance and Re-housing Enterprise System (CARES), ensure that all information is updated in the system and documented in a timely fashion as required by DHS, • Provide advocacy and referral services to ensure clients receive appropriate benefits, entitlements, and services, thereby enhancing their transition to permanent housing while minimizing the length of time families are homeless, • Coordinate services to stabilize the family unit, deal with problems of substance abuse, or improve housekeeping, parenting, and financial management skills so the goal of permanent housing may be realized, • Assess and identify the clients' needs, formulate appropriate plans of service, monitor and document the progress of service plans, and document all significant interactions with/on behalf of clients to ensure the effective delivery of service to families while ensuring compliance with State, City, and Agency regulations, • Develop and maintain appropriate helping relationships with clients to assist them in making the most of their residency at the facility and facilitate their active involvement in the achievement of service goals, • Provide appropriate individual counseling and crisis intervention services to families to help stabilize and minimize the negative impact of the family's displacement, • Maintain familiarity with off-site and on-site program resources for homeless families and refer families to appropriate resources to ensure that they receive the most beneficial services, • Facilitate groups to address family issues, enhance life skills, and assist families in achieving self-sufficiency and a successful transition to permanent housing, • Coordinate client services provided by other social services staff to ensure that families receive the necessary and appropriate services in a timely fashion and to ensure compliance with State, City, and Agency regulations, • Facilitate effective communication among staff, both within the Casework Unit and with other staff members; ensure the timely and accurate transfer of relevant information to administrative staff, such as client admission and/or discharge, and changes in public assistance cases, in order to ensure the smooth functioning of the family and compliance with regulatory agencies, • Perform other duties and special projects as assigned, including serving on-call for a one-week period each quarter, • Assist in transferring clients in and out of the program when required OTHER REQUIREMENTS (including Physical Demands): • May be required to work long hours and a varied schedule, • Traveling throughout various locations in the New York City area, • Lifting and moving objects weighing up to 30 pounds several times a week, • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions EEO Statement – Children’s Rescue Fund is an Equal Opportunity Employer. Location: 244 W 54th Street New York, New York 10019 We’re hiring Event EMS Supervisors to work with the Manager of Operations to ensure that all Field Supervisors, Support Services Department, Field Personnel, vehicles, and equipment operate in an efficient and effective manner. This person must be able to educate, comply, and enforce company policies and procedures. Our Event EMS Supervisors are responsible for making managerial decisions that affect the positive outcome of the company’s operations and taking appropriate disciplinary action when required. Responsibilities: • Resolve issues/inquiries from field personnel regarding work assignments, vehicles, equipment, etc., • Respond on an ambulance as part of the EMS system to meet compliance standards and provide quality care., • Develop coordinated teamwork between field crews as part of an integrated operation – clinical performance plan., • Investigate and resolve incoming customer service inquiries from fire departments, hospitals, nursing staff, and patients pertaining to field employee performance and patient care., • Approve or disapprove shift exchanges when proper notice of request has been given after normal business hours and forward the changes to the scheduling coordinator., • Coordinate EMS system availability of ambulances during disaster situations or MCI incidents and assist as directed by the incident commander., • Respond as necessary to Haz-Mat calls, calls involving unusual circumstances, or calls with more than two units responding., • Resolve scheduling conflicts that may occur during shifts to ensure continuation of the highest level of service and forward those changes to the scheduler., • Maintain knowledge of all company policies and regulations ensuring consistency in interpretation of such when dealing with employees and the public., • Maintain knowledge of current EMS Protocols and Regulations that govern the EMS system., • Maintain an “open door” policy to field employees and encourage crewmembers to discuss any issues and make suggestions for improvement., • Participate with new employee orientation., • Coordinate with the field training officers and clinical manager to identify clinical opportunities for improvement and provide follow-up as necessary., • Act as a liaison between field personnel, Manager of Operations, and the General Manager., • Meet with each Supervisor to schedule platoon meetings quarterly, with their crews while off duty at a designated restaurant. We currently have five part time career opportunities available to join the HKNC team as a Direct Support Professional in our group home located in Port Washington, NY. The Direct Support Professional is responsible for providing support to the residents in the areas of communication, personal management, home management, money management, community skills, vocation, leisure and the use of behavior as communication. This is an in-person position located out of our group home in Port Washington, NY. Available Shifts: • 16 hours per week. Saturday & Sunday, 8am-4pm, • 16 hours per week. Saturday & Sunday 4pm-12am., • 16 hours per week. Sunday & Monday. 4pm-12am., • 32 hours per week. Tuesday-Friday, 4pm-12am, • 24 hours per week. Friday-Sunday 4pm-12am Pay Rate: $22.9440/hour. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. • Provide consistent instruction in the following areas: personal grooming, communication, home management, wardrobe management, community skills, vocation, leisure and behavior support, under guidance of the Program Advisor and Assistant Program Advisor., • Participation in programming which consistently promotes an atmosphere conducive to the growth of all residents and encourages maximum independence and interdependence in all areas of living., • Provides consistent regular checks on all consumers during shift, ensuring safety and well-being., • Assists consumers with safe evacuation in the event of an emergency at the home, work site or in the community. Participates in regularly scheduled drills to prepare for such emergencies., • Accompanies and serves as a guide and/or a facilitator of communication for consumers and professional staff members, when assigned. To accomplish this, it may be required to use personal vehicle on occasion., • Becomes CPR and First Aid certified. Dispenses consumer medications after passing the AMAP course.
