This is an excellent opportunity to join a supportive and integrated adult social care team delivering high-quality safeguarding and statutory services. Purpose of the Role To support the delivery of health and social care functions for adults and carers across Wirral. The role plays a key part in delivering Personalisation and Inclusion Pathways, ensuring compliance with the Care Act 2014 and embedding strength-based, asset-based practice and social inclusion. You will assess, access and implement a range of social care services, providing advice, guidance and safeguarding support to adults with complex physical health, mental health needs, older people and carers.
