As the Senior Living Physical Therapist, you will provide age-appropriate services for adult to geriatric patients, including the following: comprehensive assessment, direct treatment to individuals and groups, supervision of treatment delivered by the PTA or tech, development of a home exercise program, patient/family instruction, participation in team and family conferences, evaluation and selection of adaptive equipment, participation in PI programs, and documentation of services according to department and facility standards. This position involves direct patient care. Primary Responsibilities: • Evaluates the patient to determine rehab potential, identifies needs, establishes an individualized treatment plan of care, and sets goals to be used in the team plan of care, • Sets initial STG's and LTG's, • Utilizes equipment in a proper and safe manner, maintains equipment inspection, maintenance and cleaning logs, reports broken or faulty equipment, • Maintains equipment logs for required equipment in the outpatient clinic, • Follows medical regimen developed for the patient. Required Qualifications: • Current Physical Therapy licensure in state of practice, • Current CPR certification, • Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation $77,400 - $171,000 annual total cash target pay $44.65 - $98.65 per visit point $37.21 - $82.21 hourly rate Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. Job Description The Mental Health Association is seeking Per Diem Mobile Crisis Outreach Responders for our new program, 988 Mobile Crisis Outreach Team, operating out of Essex County The Per Diem 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shifts are on an as-needed basis for this position. Specifically seeking holiday and weekend coverage. Qualified applicants for the position of Per Diem Crisis Outreach Responder will have, at minimum: • A Bachelor’s Degree in a mental health related field from an accredited institution with two years post bachelor’s professional experience required. Previous construction experience is preferred., • Professional Engineer (PE) license, • 40-hour OSHA HAZWOPER certification, • Registered Professional Engineer (PE), • Valid Driver’s License and possess reliable transportation to access job sites, • Able to work with mathematical concepts such as probability and statistical inference, and fundamentals of solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations., • Proficient or familiarity with computer design software (Quake, WEAP, L-Pile, Finite Element Analysis, etc.) While performing the duties of this Job, the employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; talk or hear and taste or smell. The employee must frequently lift and/or move under 50 pounds. The employee is frequently required to use hands or fingers to handle or feel objects, tools, or controls. Approximately 10% of the work is conducted outdoors. Compensation: Base salary $150,000–$205,000, + mileage reimbursement, benefits, paid time off, tuition reimbursement, professional development opportunities, 401(k), and bonus potential.
