ABOUT CJA The New York City Criminal Justice Agency (CJA) is a not-for-profit service and research organization that operates in partnership with the New York City Mayor's Office of Criminal Justice. CJA is the City's main pretrial services agency, combining operations, pilot projects, and research under one roof. CJA utilizes procedural justice practices to provide pretrial services to over 100,000 persons arrested annually in New York City. Information collected about these arrestees and subsequent case processing is maintained in the agency's internal and externally shared database systems, which are used to conduct descriptive and evaluation research on arrestee characteristics, case processing and court outcomes, and on issues and potential reforms to criminal justice policy in New York City. CJA's programs operate in the criminal courts and detention facilities 24 hours a day seven days a week within the 5 boroughs of NYC, employing over 300 employees, citywide. POSITION SUMMARY Reporting to the Clinical Supervisor, the Client Care Coordinator will provide community-based support to clients in their neighborhood or preferred location throughout NYC. Clients receiving community based engagement will meet the program's eligibility criteria of high risk or high need and require a support team to meet with them where they're at to facilitate early intervention. The Client Care Coordinator is an integral part of the Clinical Services Department with Queens Supervised Release, which strives to support clients in improving behavioral health, promotes recovery and rehabilitation, and provides the highest quality care and support in a responsive and effective manner to high needs and high-risk clients who can be served in the community by creating new opportunities for success. PRIMARY RESPONSIBILITIES • Coordinating care, treatment and services within multiple health care systems including public health insurance and Medicaid entitlements., • Creates proactive care plans with a supportive community-based team for mental health and substance abuse services as well as overall physical health concerns., • Assist clients in securing medical care, entitlement and other community support that promote integrated physical and mental health, safety, well-being, and recovery., • Communicates with client, client caregivers, and/or families (if applicable). Fitness Program, • Pet Insurance, • Legal Assistance, • Optum Financial Service through ConnectYourCare, • Benefit Advocacy Center through Gallagher MAJOR FUNCTIONS: The Bronx Street Outreach Coordinator oversees Odyssey House’s Bronx Street Outreach Program, ensuring effective delivery of low-threshold, trauma-informed harm reduction services to people who use drugs (PWUD) and individuals with co-occurring needs. The Coordinator manages daily program operations, supervises outreach staff, coordinates referrals, and ensures fidelity to harm reduction principles and regulatory standards. Description of Duties and Responsibilities. ▪ Supervise, train, and support two Peer Outreach Workers, including bi-monthly individual supervision and monthly group debriefings. ▪ Oversee daily outreach operations, including coverage of assigned zones, coordination of warm handoffs, and real-time problem-solving in the field. ▪ Ensure consistent delivery of harm reduction supplies, wound care, naloxone training, overdose prevention education, and benefits navigation. ▪ Manage referral pathways with internal services and external partners, including Damian Family Care Centers, Montefiore Medical Center, Acacia Network, Bronx Community Solutions, The Fortune Society, and St. Ann’s Corner of Harm Reduction. ▪ Track outreach activities, service referrals, and program metrics using agency platforms (e.g., AWARDS, Net[1]Health, Search Treatment Referral Management Portal). ▪ Conduct quality assurance reviews, support continuous quality improvement, and ensure compliance with OASAS and NYSDOH guidelines. ▪ Facilitate staff training in trauma-informed care, harm reduction, overdose response, motivational interviewing, and cultural humility. ▪ Collaborate with agency leadership and community partners to align outreach strategies with emerging overdose data and community needs. Qualifications for Position: 1. Bachelor’s or Master’s degree in social work, counseling, public health, or a related field is preferred., 2. Candidates are preferred to have 3-5 years of experience in street outreach, harm reduction, or SUD treatment,, 3. Supervisory experience strongly preferred., 4. CASAC, CASAC-T, LMSW, LMHC, or LMFT is required, or the candidate must be eligible to obtain such credentials within 6 months of hire. Skills and Knowledge Required: Expertise in harm reduction, overdose prevention, and benefits navigation, with a strong foundation in trauma-informed care and motivational interviewing. Strong communication, leadership, and crisis intervention skills are required, along with the ability to supervise staff, ensure quality assurance, and manage program operations. Knowledge of community resources, referral systems, and data tracking platforms is expected, and bilingual ability is strongly preferred. Amount of travel and any other special conditions or requirements: Local travel required, valid NYS driver’s license strongly preferred. Fitness Program, • Pet Insurance, • Legal Assistance, • Optum Financial Service through ConnectYourCare MARJOR FUNCTIONS: The Peer Specialist is a fully integrated outreach team member who provides linkage and referrals services to recipients with mental health and co-occurring disorders. Because of their personal life experiences with mental health wellness and co-occurring recovery, which demonstrates self-sufficiency, the Peer Specialist provides expertise that other professional training cannot replicate. The Peer Specialist will advocate, counsel, educate and find specific resources to refer/link the enrolled recipient(s) to reach their individual goals. The enrollment into the program for referrals/linkages will remain in place for six weeks. Once that timeframe has been reached the individual recipient case will be closed out, however there could be exceptions if the recipient(s) needs further assistance in reaching their individual goal. SPECIFIC DUTIES & RESPONSIBILITIES: • Provide outreach to the community at large in an effort to locate, intake and refer/link recipients to mental health and other services; as outlined in the Recovery/Action Plan (10 recipients a month)., • Keep charts updated weekly. Weekly contact with the recipient needs to be documented in the chart in regard to progress, lack there-of or change in objectives, interventions and goals., • Support the recipient(s) in maintaining focus and accomplishment of their identified goals., • Organize and facilitate focus topic wellness group. In these groups’ attendance will be taken and only unduplicated recipients will be counted as a contact monthly., • Assist the Outreach Coordinator or be able to provide presentation of the program at potential or existing sites., • Provide street outreach (tabling) and individual contact in regard to our services as offend as necessary (evenings & weekends)., • Promote/develop the recipient’s self-advocacy skills., • Meet monthly with Outreach Coordinator for supervision., • High School Diploma or G.E.D., • Certified Recovery Peer Advocate (CRPA-P/CRPA/CRPA-A) certificate highly preferred but not required., • Must be able to work a flexible work schedule., • Must be proficient in Microsoft products (Outlook, Word, Excel, Teams, etc.), • Drivers license Odyssey House is an equal opportunity employer maintaining a non-discriminatory policy on hiring of its personnel.
