Job Summary YWCA NENY Rapid Rehousing program serves individuals with varying levels of mental health diagnoses, women recovering from addiction, those recently released from incarceration, individuals living in extreme poverty, chronically homeless individuals and other marginalized populations. Reporting to the Rapid Rehousing Coordinator, the Case Manager is responsible for a caseload of approximately 30 participants/households who are homeless or at risk of homelessness due to domestic violence. Supervisory Responsibilities: None FLSA Category: Non-exempt Compensation: $19 - $21.50 Hourly. Responsibilities: • Conduct intake screening with client once referral received to determine housing needs., • Assess client’s physical and mental needs and refer for assistance as needed., • Assist with household budgeting and lease management for apartments. Refer individuals for financial counseling as needed., • Provide ongoing case management as needed after family has found housing, to help maintain safety, security and mediate landlord tenant dispute as needed. Case Management may be located in client’s home (within the capital region) or YWCA NENY office., • Encourage and provide guidance to clients with services such as social services, SSI/SSDI, finding employment, continuing education, obtaining childcare etc., • Participate in apartment inspections with clients., • Maintain files and case notes (empower database data entry) for individuals/families in the program., • Participate in conferences, staff meetings, trainings., • Knowledge and experience managing program budget and expenses., • Knowledge and experience with NYS tenant and/or landlord rights., • 2-4 year’s experience working with individuals needing assistance with low-income housing preferred., • Associates degree in Human Services or related field preferred., • Clean NYS Driver’s license and ability to use own vehicle for work related travel., • Prolonged periods of sitting at a desk., • Ability to use computer and necessary agency software., • Able to lift up to 25 pounds without team assistance. Job Description Residential Nurse Educator PART TIME. 20-30 HOURS BASED ON CANDIDATE’S AVAILABILITY. No on-call. No supervision of Approved Medication Assistive Personnel and No Personal Care Medical advocacy and case management. The Residential Nurse Educator serves as a resource for the clients in four community residences and the apartment program ensuring that significant medical needs are appropriately addressed with identified clients. Our programs focus on transition to independent living. Staff members support clients in learning to take their medications independently. Specific duties included: • Meet with new admissions (approximately 35 clients a year) and review medical history. Ensure that all clients are linked with a Primary Care Physician and have their annual physical., • Ensure that an appropriate plan of care is established and monitor the medical status of those identified clients who are medically compromised and/or who have significant physical health needs (approximately 15 clients)., • Attend medical appointments with clients who are experiencing chronic conditions, need assistance communicating their concerns to the physician or who may have difficulty interpreting/understanding what the physician may be trying to convey to them., • Act as a liaison with the medical providers when needed to obtain needed prescriptions, follow-up with questions or concerns, schedule appointments, etc., • Coordinate post discharge care as needed for clients who have been hospitalized for medical reasons or who have had surgery., • Consult with program staff to address noted issues of concern regarding clients’ medical status and provide education related to case specific medical issues/needed follow-up care and monitoring., • Facilitate the agency’s Health & Safety Committee which reviews and makes recommendations for those individuals who have more complex medical and/or psychiatric needs., • Provide training to staff on related medical topics., • Meet with clients who are reporting “minor” medical issues, displaying patterns of missing/refusing medications or who are high users of the Emergency Room. Job Description Service Coordinator at Certified Apartment Program Responsibilities of the Apartment Counselor are to provide services to adults who have a diagnosis mental illness and living in individual apartments throughout the Schenectady community. The goal is to work with the client in stabilizing at in his/her apartment while also providing rehabilitative services so he/she can transition to the next level of independence. Specific duties included: • Providing supportive counseling to assist in stabilizing., • Everything we do with our clients is verbal including verbal intervention skills. There is no personal care and no physical restraints., • Crisis prevention skills and coping skills., • Verbal crisis intervention., • Medication supervision by bringing the medication to the clients’ apartment for refills based on the step system and number of days the clients is eligible to have the medication. Although we don’t watch clients ingest the medications, we do a count at the visit to make sure the correct amount of medication is in the medication organizer. The ideal candidate displays the utmost professionalism Pay rate: $22.00 - $25.00 Monday - Friday Hours: 8:30 am - 5:30 pm Responsibilities include but not limited to: • Communicates the benefits of the apartment community, responding to leads by phone, in-person, and writing, • Screens prospective residents and leads tours of apartments, • Assists in supervising maintenance, security, landscaping, and cleaning apartments when necessary and at the direction of the Property Manager as well as coordination of contractors and vendors, • Assists with reviewing inventory and maintenance purchases as needed, • Assists the Property Manager in fiscal planning, staff supervision, physical property management, rent collection, and resident relations/complaints and directly oversees the occupancy and recertification process, • Detail-oriented and able to accurately prepare and document rental and resident paperwork, leases, and reports, • Fills in for Property Manager when he/she is off-site or as delegated, • Coordinates tenant move-ins / outs as well as assisting in turnover schedules with the maintenance supervisor and Property Manager, • Actively collect rents and control delinquencies, determines and recommends legal action for any tenant/lease defaults, and works with internal and external legal teams on lawsuits and other legal matters, • Prepares expense reports and other administrative forms, • Ensures tenant lease compliance, communicate any issues to the Property Manager, • Three years of Property Leasing experience is preferred, • Excellent customer service, sales, and problem-solving skills., • Professional use of verbal and written communications., • Must have a valid, clean driver’s license and have reliable transportation., • Certification - CLS or TCS preferred To APPLY please submit an online application Need help with your resume?
