Program Description: PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve. They help clients navigate through what can be a complicated and confusing application process, step by step. Each year, we help over 22,000 people apply for or renew free or low-cost health insurance coverage, 14,000 people to apply for SNAP, and 35,000 eligible pregnant and nursing women and children up to five years of age to enroll in the New York State WIC (Women Infants and Children) Program. Position Summary: Under the general supervision of the ABD Sr. Program Manager and Director of Health Insurance Enrollment, Home Visitor Facilitated Enrollers will engage the aged, blind, and disabled (ABD) populations through home visits, outreach, and at community-based sites. This role will engage the ABD populations who are homebound and assist clients with the application process for public health insurance through home visits 5 days a week (including weekends per request) across the 5 NYC boroughs. Home Visitor Facilitated Enrollers will educate consumers about insurance options, benefits, length and terms of coverage and application process as well as about managed care, health plans, and primary/preventive care. They will also provide guidance to consumers on required documentation and assist consumers with health insurance applications. Schedule may vary due to program needs JOB SUMMARY: Provide high-quality, culturally competent child development and parent support services with an emphasis on the role of the parent as the child's first and most important relationship. The Home Visitor will also assist with the completion of the child health requirements and other required screenings and assessments while supporting the larger system of Family and Health/ Nutrition Services. For the start-up phase for 2 years this will be an evolving position. Interim roles and responsibilities will be adjusted and this position requires an individual to be flexible. Under the direct supervision of the Education and Disabilities Coordinator, the Home Visitor shall be responsible for: • Maintaining a caseload of (12) participants, • Providing 90-minute weekly home visits to support parents/guardians in their roles as primary caregivers and to facilitate the child’s individual development within their home environments, • Providing regularly scheduled home visits with enrolled pregnant women to ensure access to comprehensive prenatal and postpartum care and to provide prenatal education on topics such as fetal development, labor and delivery, postpartum recovery (including maternal depression), and the benefits of breastfeeding., • In collaboration with the Education Coordinator and assigned staff, planning and implementing developmentally appropriate, parent/child focused Group Socializations twice per month, • Assisting parents in identifying strengths and goals leading to school readiness, • Working in collaboration with families to identify goals for themselves, their families and their child, and developing objectives for meeting those goals for the Family Partnership Agreement, • Record Keeping: Enters child and family information into the approved child tracking system database (COPA & ACS/DOE systems). Assist with collecting child health medicals and dentals along with entering health related data into COPA. Submits reports as needed and requested, such as monthly summary tracking reports, etc. Maintains and provides data for the annual Program Information Report (PIR).
