CSS’s Health Initiatives Department runs a number of statewide programs that help New Yorkers find and use their health insurance or otherwise access health care, including: Community Health Advocates (CHA); the CSS Navigator Network (CNN); Community Health Access for Addiction and Mental Healthcare Project (CHAMP); the Independent Consumer Advocacy Network (ICAN); the Managed Care Consumer Assistance Program (MCCAP); the NY State Office of the Aging (SOFA) program; the Independent Intellectual and Developmental Disabilities Ombudsprogram (IDDO) and the Facilitated Enrollment for the Aged Blind and Disabled program (FE-ABD). RESPONSIBILITIES: • Assist the Aged, Blind and Disabled populations enrolling in and recertifying in Public Health Insurance., • Provide direct services to clients, including intake, engagement, assessment, service plan development,, • referral, advocacy, exit summaries and follow up., • Monitor and document client progress toward service plan goals., • Document all interactions with, or on behalf of, clients., • Maintain familiarity with program resources available on- and off-site., • Coordinate client services with other social services, mental health, medical, employment, educational,, • childcare, and other providers., • Conduct regular case conferences with clients and service providers as required., • Complete program reports monthly, or more frequently as required., • Report to, and meet with, supervisor monthly., • Conduct home visits and other fieldwork, as required., • Perform additional duties as assigned. Program Description: PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve.
