Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 255-unit residence will provide housing for 153 individuals and families referred by the New York City HIV/AIDS Services Administration (HASA) and the rest 102 units will be apartments for moderate-income households. The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population. Job Description Bilingual Case Manager of Functional Family Therapy ESSENTIAL DUTIES AND RESPONSIBILITIES RiseBoro’s Functional Family Therapy- Adult program is a therapeutic model that operates within its Clinical Services unit and is a strength-based, evidence-informed, culturally responsive intervention program that, at its core, aims to reduce community and interpersonal violence by engaging people in a change process that nurtures hope and strengthens relationships. Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Reporting to the PRS Intervention Supervisor, the PRS Intervention Case Manager will provide therapeutically focused intensive case management and supportive services to unaccompanied immigrant children released from federal custody. Attracting and retaining the best talent is vital to our continued growth and we are proud to offer an excellent benefits package including: • medical, dental, vision available the first day of employment,, • generous paid time off including 20 days of vacation, 12 days of sick leave, 12 paid holidays,, • 12 weeks of fully paid parental leave and up to 12 weeks of fully paid FMLA leave, DUTIES: • Conduct weekly in person therapeutic support for clients, focused on crisis intervention, safety planning and family preservation., • Provide therapeutically centered ongoing intensive case management for especially vulnerable children with special circumstances (i.e., medically or psychologically vulnerable children, family conflict or crisis, education-related issues), • Maintain regular and timely communication with supervisor around any issues concerning the child's safety, stability, or well-being in the placement., • Submit detailed and thorough documentation in compliance with Global Refuge policies and procedures., • Utilize clinically focused interventions as a framework for intensive case management support and social work best practices for ongoing services., • Travel by car or public transportation within an assigned region or, in emergent circumstances, occasionally outside that assigned region., • Participate in ongoing supervision, • Other duties as assigned. QUALIFICATIONS: • Master's degree in social work, psychology or other relevant behavioral science in which direct clinical experience is a program requirement; or a bachelor's degree with at least five years of experience in a clinical setting., • Licensed, or eligible for licensure., • Submit detailed and thorough documentation in compliance with Global Refuge policies and procedures., • Utilize social work best practices for thorough assessments of child's progress with a focus on therapeutic support and family preservation., • Ability to travel to home visits and other service agencies to serve the client, either by own vehicle or by public transportation (train, plane, etc.)., • Availability of a vehicle that meeting safety and mechanical requirements for travel within an assigned radius., • Maintain regular and timely communication with supervisor around any issues concerning the child's safety, stability, or well-being in the placement., • Strong analytical and problem-solving skills., • CBT certification, trauma focused CBT preferred, • Commitment to Global Refuge's core mission and values and an ability to model those values in relationship with colleagues and partners; commitment to empowering refugees and migrants. Special Position Requirements: • Must be willing and able to clear a criminal background check, • Must be willing and able to request and clear Child Abuse and Neglect background check in all states lived within previous 5 years, updated as required., • Must obtain a negative TB Test yearly., • Must be willing and able to obtain FBI fingerprint check every 5 years., • Spanish language fluency required, • Must have valid driver's license and MVA check free of major infractions for the last 7 years.
