Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. They participate in clinical reviews, incident reviews, human rights committee meetings, and other meetings as directed. As schedule allows, the Client Coordinator serves as primary chair for Life Plan meetings. They review documentation and provide technical assistance to program management and staff. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES The employee must be able to interact with individuals and staff professionally and ethically, understand and implement service plans, and complete required documentation clearly, legibly, and on time. Previous experience working with individuals with IDD/mental illness is required, as well as experience managing teams. Competency in Microsoft Word is required. Experience working in a Day Support Program or Special Education environment is preferred. Required: • Associate Degree in a Human Services field with 4–6 years of OPWDD experience, OR, • High School Diploma/GED with 7–10 years of OPWDD experience., • Experience handling confidential information, multitasking, problem-solving, taking initiative, working independently, and meeting deadlines., • Successful completion of required trainings (SCIP, AMAP, CPR/First Aid). The Queens Community Justice Center is seeking a Mentor for its Plus+ Mentoring program. Plus+ Mentoring is a transformative mentoring services program for young people on the ICM level of probation between the ages of 13 and 18 who reside in Queens. Job DescriptionBenefits: • 401(k), • Dental insurance, • Free uniforms, • Health insurance, • Paid time off, • Parental leave, • Training & development Responsibilities Patient Communication & Support • Answer incoming phone calls, take accurate messages, and direct calls appropriately., • Respond to inquiries from patients, vendors, insurance companies, and healthcare providers., • Provide patients with information regarding appointments, procedures, and treatment plans., • Assist patients in navigating the healthcare system and resolving concerns with empathy and professionalism.Patient Registration & Scheduling, • Request and obtain medical records for new patients., • Scan and upload medical records and insurance cards into the electronic health record system., • Register new patients in Aria and Practice Fusion., • Schedule and confirm appointments for consultations, simulations, procedures, treatments, and follow-up visits., • Coordinate imaging appointments and request necessary authorizations., • Notify patients of appointment details (date, time, and location)., • Request and track CDs of imaging studies., • Verify patient insurance coverage on a regular basis, particularly for patients undergoing treatment.Administrative Support, • Fax consult letters, follow-up notes, and summaries to referring physicians., • Prepare and send medical records to other providers or entities upon receipt of a signed release of information., • Support general office functions, including data entry, filing, and record maintenance.Qualifications, • Education: High School Diploma or equivalent required., • Experience: 2+ years of medical office experience, preferably in a specialty clinic or hospital-based department., • Language Skills: Bilingual Russian or Spanish preferred., • Technical Skills: Proficiency in Microsoft Office Suite (Word, Excel, Outlook). Experience with Electronic Health Records (EHR), especially Aria and Practice Fusion, preferred. Strong computer and typing skills required., • Communication Skills: Excellent verbal and written communication skills, with strong interpersonal and customer service abilities., • Organizational Skills: Highly organized, detail-oriented, able to effectively prioritize tasks and manage multiple deadlines., • Customer Service: Demonstrated ability to remain calm, professional, and effective in high-stress situations., • Teamwork: Capable of working independently and collaboratively in a fast-paced clinical setting.

Salary for this position is $62,000-$68,000 annually Job Summary • Behavior Intervention & Assessments & Analysis, • Collaboration with BIS staff to conduct functional behavior assessments, to develop behavior support plans and data collection systems, • Assist with Life Plan process and individual goal development, • Review and provide feedback to QIDPs on preparing and developing Staff Action Plans (SAP), • Review and maintain documentation and provide reports and recommendations to supervisors, • Ensure data is taken correctly and individual’s progress is monitored daily, • Assist with training development and protocols and provide staff training across all multiple residential locations, • Provide training and support to BIS and QIDP staff for development of proactive and reactive strategies /SCIP-R interventions and implementation, • Assist BIS staff in conducting in-service training, • Travel to multiple sites in NYC and Queens is required and Clinical Coordinator will be required to fill in for absent BIS/QIDP, as needed, • Maintain individual and family confidentiality, • Qualifications are subject to change in accordance with government regulations, • Master’s degree required in Psychology, social work or related field; BCBA Preferred, • 3-5 years professional/clinical work experience, • 2 years experience supporting adolescents and/or adults with autism spectrum disorders/developmental disabilities, • 2 years management experience required, • Understanding of Applied Behavior Analysis strategies and principles, highly preferred, • Knowledge of federal, state and local regulatory requirements, • Excellent interpersonal, critical thinking, problem-solving and communications skills, • Ability to work independently with minimal supervision, • Ability to define problems, collect data, establish facts, and draw valid conclusions QSAC has terrific benefits that go beyond health and retirement.
