Job Description JOB SUMMARY: Under the general supervision of the Program Director or Designee, functions as part of a team providing client-centered, strength-based rehabilitation and recovery services to assigned consumers that relate to the care, nurturing, treatment, education, socialization, habilitation, recovery/healing and support of adults diagnosed with a mental illness and in some instances, co-occurring substance abuse disorder. These tasks focus on supporting, instructing and assisting recipients of mental health services to develop the skills needed to attain personal goals, live, be educated, work, and socialize successfully in the community environments of their choice. Based on this information, completes and maintains consumer treatment plans to include quarterly service-plan reviews, assessments, medical records, and changes in treatment and/or medications either manually or electronically as instructed., • Provides supportive counseling in 1:1 or group formats that assist the consumer to modify attitudes and behaviors as needed, • Maintains up-to-date, accurate individual case records on each consumer and develops measurable and objective service plans that maximize consumer rehabilitative abilities in accordance with regulatory guidelines. Continually documents consumer treatment progress. Reviews service plans and performs follow-up to determine quantity and quality of services provided. Ensures that plans include consideration of the consumer’s cultural and ethnic background, customs, needs, beliefs, and primary language., • Facilitates individualized services to the consumer that meet the diverse needs of the consumer and focus on the discharge-planning goal. Document case notes regarding significant and untoward events and contacts with mental health providers, including ICM’s and AOT’s., • Models appropriate behavior and provides rehabilitation and recovery services to consumers that incorporates skills training in all areas of psychiatric rehabilitation, including the 11 restorative services. Uses cognitive-behavioral techniques (e.g. role-plays, skills training, plans of action) that assist the consumer in meeting service plan goals Examples of skills training include self-advocacy training, ADL (Activities of Daily Living) skill building, medication management training, socialization skill enhancement, and conflict resolution training. Job Description PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. The Center for Justice Innovation operates Project Reset, New York City's pre-arraignment diversion program, in all five boroughs and the Midtown Community Justice Center operates the program in Manhattan. In collaboration with the District Attorney’s Offices, the Mayor’s Office of Criminal Justice, the defense bar and the New York Police Department, Project Reset seeks to create a proportionate response to low-level crime outside of the traditional criminal justice process. Project Reset-eligible community members receive the option to participate in a one session program in advance of their court date. Upon completion of the session, the District Attorney’s Office will decline to prosecute the case. The Midtown Community Justice Center also offers Rapid Reset, which offers the same diversion opportunity to Project Reset-eligible people who show up to court. Midtown Community Justice Center is seeking a Project Reset Social Worker. Reporting to the Project Reset Program Coordinator, the Social Worker will facilitate psychoeducational group-based curricula and provide individual counseling and case management to Project Reset and Rapid Reset participants. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC is seeking a Clinician, Supervised Release Program (SRP). Reporting to the Clinical Director, the Clinician, SRP will be licensed a social worker and mental health practitioner responsible for providing therapeutically-informed support to participants. This position will leverage exemplary clinical and interpersonal skills to provide clinical support to participants enrolled in the program. Specifically, the Clinician, SRP will facilitate CBT-informed sessions for all Tier 2 participants, provide short-term counseling for any participant identified as a strong candidate for mental health support, and external referrals to therapy, as appropriate. This position provides hours towards LCSW accreditation. The Clinician, SRP will not carry a standard SRP caseload. Rather, they will work closely with the case management and specialty in-house referral teams to case conference about participants' strengths and needs and coordinate tailored support as appropriate. The Clinician, SRP will work with other SRP teams to ensure access to, completion of, and accurate data entry for CBT and other therapeutically-informed sessions to ensure these sessions count toward participants’ compliance status. Job Description About the Role The Trauma Recovery Center (TRC) of Rising Ground understands the importance of removing barriers to care and providing trauma-informed services. Through this dedication, the TRC aims to transform services for survivors of violent crime. Rising Ground’s TRC is designed to help victims of violent crimes overcome barriers to accessing mental health treatment, health care, and legal resources in the acute aftermath of trauma. The TRC supports recent victims of violent crime, including survivors of physical assault, sexual assault, gunshot wounds, stabbings, domestic violence and human trafficking. The Trauma Recovery Center facilitates healing for survivors of trauma, violence, and loss through an innovative model that blends comprehensive care, advocacy and outreach. The Clinical Social Worker/Therapist of the Trauma Recovery Center (TRC) will provide clinical intakes, assessments, planning and implementing individual and group psychotherapy, assertive outreach, and clinical case management services, using evidence-based practices that are effective for the service population. The ideal candidate will bring a passion for social justice and anti-oppressive practice, ample experience providing clinical services to survivors of trauma, and a desire to work collaboratively with a team of creative and committed advocates. The Clinical Supervisor is responsible for client case record reviews, performance improvement and quality improvement to include all required monthly reports. KEY ESSENTIAL FUNCTIONS • Responsible for the daily operations and performance of the OTP program, • Manage OTP clinic operations to budgeted and planned results., • Work closely with staff via weekly supervision to ensure the completion of performance goals. Conducts training on clinical subjects, charting, fire safety plan, fire drills, emergency plans and procedures, building security, volunteers and interns, and site-specific exposure control plan., • Manages employee accident reporting and initial submission of workers’ compensation claims., • Monitors employee morale and the climate of the work environment to stop or eliminate sexual harassment or any form of employee harassment or employee discrimination., • Responsible for taking complaints of sexual harassment, informing the Human Resources Department of the complaint, and investigating the complaint (as directed)., • Monitors employee accrual totals, approves or recommends approval of employee requests for time off, and notifies the Department of Human Resources when an employee should be placed on or taken off Family and Medical Leave (FMLA).
